6-501.111 Keep the premises free of insects, rodents, and other pests. Observed small flying insects around can wash and 3-comp sink drain. Per PIC, EcoLab regularly services facility. Recommend more frequent service.

3-304.14(E) Store sanitizer containers used for wet wiping cloths off the floor and to prevent contamination of food, equipment, and utensils. Observed sanitizer bucket stored on floor under front counter. All other sanitizer buckets properly located. PIC moved bucket off of floor.

4-601.11(C ) Keep non food contact surfaces of equipment clean to avoid build up of dust, food residue or debris. 4-602.13 Non food contact surfaces of equipment shall be cleaned frequently to preclude accumulation of soil residues. Observed build-up of food and splash residue on bottom interior surfaces of refrigeration/freezer units. Observed build-up of food debris on door tracts of front glass display case. PIC to address cleaning frequency.

3-304.14(B) Hold in-use wiping cloths in properly concentrated sanitizer between uses. Observed 3 quat solutions holding in-use wiping cloths at 0ppm. CDI- PIC removed containers and refreshed them to above 200ppm. EHS confirmed.

4-501.18 Maintain wash, rinse, and sanitize solutions clean. Observed wash and rinse solutions in 3-comp sink unclean. CDI- PIC emptied compartments to refresh with clean solutions to w/r/s dishes and equipment.

6-301.14 Post a handwash sign at each handsink. Observed hand sinks without hand wash signs. CDI- provided PIC with hand wash procedure signs.

4-903.11(A)(C) Store single-use and single-service articles in a clean, dry location; where they are not exposed to splash, dust, or other contamination; and at least 6 inches above the floor. Observed boxes of single service items stored on the floor in the back of house kitchen.
