4-903.11 (A) Store cleaned equipment, utensils, linens in a clean, dry location; where they are not exposed to splash, dust, or other contamination; and at least 6 inches above the floor. Observed boxes of single services bowls and cups being stored directly on the floor in storage room. CDI - PIC removed boxes off the floor.

Solution bleach-out test strips. CDI - PIC voluntarily discarded solution.

4-302.14 Provide a test kit that accurately measures sanitizer concentrations. -Pf Observed no bleach test strips during inspection. CDI - PIC was given bleach test strips and educated on how to properly measure strips.

6-501.12 Floors, walls, ceilings including the attachments such as soap, towel dispensers; light fixtures; and heat/ac vent covers shall be clean. Caution should be used to minimize food exposure. Observed food splash on ceiling near bathrooms and dry storage room. Increase frequency of cleaning.

6-403.11 (A)Areas designated for employees to eat and drink, shall be located so that food, equipment, linens, single service/use items are protected from contamination Observed personal food co-mingled in low boy freezer with establishment foods. CDI - PIC voluntarily discarded food.

6-301.12 Provide paper towels or approved alternative for hand drying at each handsink. -Pf Observed no paper towels at handwashing sink near register. CDI - PIC provided paper towels.

6-501.111 Keep the premises free of insects, rodents, and other pests. Observed small insects crawling along ceiling baseboards in storage room. 6-501.112 Remove dead or trapped birds, insects, rodents, and other pests. Observed dead insects under refrigeration equipment in storage room.

3-304.14(B) Hold in-use wiping cloths in properly concentrated sanitizer between uses. Observed several blue wet wiping cloths stored on drainboard of 3-comp-sink. CDI - PIC voluntarily discarded wiping cloths.
