Per Manager Chad, this area was damaged after the last freezing weather when snow accumulated in the area. Fill in grout. Observed hard crack around the perimeter of ceiling at crown molding in Arosa Room bathroom and holes in walls in Solarium closet where iron holder was once located and at wall under hand towel bar in Arosa Room restroom where another wall fixture was located. Fill in these areas. -0-

15A NCAC 18A .1826(a) Handwashing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. Observed no soap and paper towels at handsink located in laundry room.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed minor carpet staining throughout facility.

15A NCAC 18A .1827(a) The premises shall be kept free of litter and items unnecessary to the operation or maintenance of the lodging establishment, such as equipment that is nonfunctional or no longer used.

15A NCAC 18A .1825 (c ) A handwashing sink, located to allow use by employees handling clean and soiled linen, shall be provided. This requirement shall be effective one year after the effective date of this Rule. Per discussion with only housekeeper present during time of inspection the sink located in the laundry room is a chemical dump sink and that she does not wash her hands in that sink. EHS also observed no soap and paper towels at that sink which supports the housekeeper's comment. EHS advised facility that a hand sink shall be provided in the laundry area or next inspection points will be taken. NO POINTS TAKEN.

Observed slits in chair located in carriage house lower level to the right of main entrance. Observed staining on carpet near night stains in room Euclid. Observed staining on carpet in Mt. Vernon Observed brown residue build up inside of tub basin in room Kingston bathroom. Observed rust build up around the sink handles in carriage house lower level room to the right of entrance bathroom. REPEAT VIOLATION.

15A NCAC 18A .1824(f) Sanitizers used for sanitization of kitchenware and food-contact surfaces shall be maintained as required in Part 4-5 of the Food Code as amended by Rule 15A NCAC 18A .2564. Observed housekeeper saying that sanitizers were only left on ice bucket lids for 12-15 seconds before wiping off. Ensure that sanitizers are allowed 60 seconds of contact time as required by manufacturer.

Observed chemical sanitizer in house keeping bucket on second floor measuring 0 ppm quat and 0 pmm chlorine. **Sanitizing solutions shall consist of iodophors, quaternary ammonium or other chemicals or methods that are equivalent to the action of 50 parts per million chlorine solution. 15A NCAC 18A .1809 (b).

Observed hot water in hawthorn suite at 133F. **Hot water in guest rooms shall be provided at a temperature of no less than 116°F (46.7°C) and no more than 128°F (53.3°C). 15A NCAC 18A .1810 (e).

House keeper on the second floor was questioned on how ice buckets were cleaned, she stated Lysol was used on the buckets. Lysol is a disinfectant and is too strong to use on a food contact surface. Also, there was no sanitizer available on the 2nd floor for sanitizing the ice buckets. **When properly fitting disposable food grade liners are provided, ice buckets shall be treated as non-food contact surfaces and shall be washed, using an all purpose cleaner suitable for food contact surfaces, and rinsed. The lid used with the lined ice bucket shall be washed, rinsed and subjected to an approved sanitizing treatment in place. 15A NCAC 18A .1811 (b).

Observed stain on couch cushion, and cleaning needed under couch cushions in Solarium room. Cleaning needed under Hawthorne room and library couch cushions. Observed cleaning needed on the gasket of the refrigerator in the second floor sitting area. Cleaning is needed in the drawers of the coffee furniture in the second floor sitting area. Observed stained mattresses in the Solarium and Dilworth room. Cleaning needed on the land-line phones near the mouth area in the Solarium and Arosa rooms. **Furniture, fixtures, carpets and other accessories shall be kept clean and in good repair. 15A NCAC 18A .1812 (d).

Observed boxes of single-service cups being stored on the closet floor on the 2nd floor. Observed a folding mattress being stored in the Hawthorne suite closet, underneath a sewage pipe and food service equipment under sewage lines in the basement. Provide protection from possible contamination by installing a drip pan. Observed chemicals being stored on shelf above clean items in basement. Observed clean resident care items being stored with chemicals under the vanity in the second floor common restroom. **A separate storage area shall be provided for building and ground maintenance equipment and supplies. 15A NCAC 18A .1813 (d). **Clean linen and supplies shall be stored in cabinets, or on shelves in linen and supply storage rooms. Cabinets, shelves, and storage rooms shall be constructed to facilitate cleaning and kept clean. 15A NCAC 18A .1813 (b).

-Observed the sanitizer bottles that were being used at 0-50ppm Quat. Per the bottle, food contact surfaces shall be cleaned with 200ppm strength quat. -Observed a bottle of bleach solution that measured above 1000ppm chlorine. (The bottle was labeled incorrectly). Sanitizing solutions shall consist of iodophors, quaternary ammonium or other chemicals or methods that are equivalent to the action of 50 parts per million chlorine solution. 15A NCAC 18A .1809 (b). After cleaning the lavatory, faucet, and vanity, a sanitizer shall be applied to the lavatory and vanity using a method such as a wipe-on chemically-treated disposable sanitizing towel or equivalent. The sanitizer shall be left to air dry. 15A NCAC 18A .1809 (b).

Observed the water at 131F-137F in the 2 buildings that are separate from the main building (where the office is). Hot water in guest rooms shall be provided at a temperature of no less than 116°F (46.7°C) and no more than 128°F (53.3°C). 15A NCAC 18A .1810 (e).

-Repeat: Observed bedding that is not laundered between guests (per a housekeeper) on the floor. Do not contaminate clean linen. This was general commented on last inspection. -Observed several rooms with stained sheets, mattress covers, and comforters. Bed linens, including sheets, pillow cases, blankets and bedspreads, shall be kept clean and in good repair. 15A NCAC 18A .1812 (c).

Repeat: Observed carpet wavy and or fraying in several rooms. The floors, walls, and ceilings of bedrooms, closets, and storage areas shall be kept clean and in good repair. 15A NCAC 18A .1812 (d).

Observed cracked window and dusty ceiling fan in Mt. Vernon room.**Windows and glass doors shall be kept clean and in good repair. 15A NCAC 18A .1812 (a).**In the absence of functioning windows, air conditioning and artificial lighting must be kept clean and in good repair. 15A NCAC 18A .1812 (a).

Observed loose carpeting (wavy) in several area. Chipped paint on sills in several areas.**The floors, walls, and ceilings of bedrooms, closets, and storage areas shall be kept clean and in good repair. 15A NCAC 18A .1812 (d).General Comment #25: Do not let bedding or pillows that are not to be laundered between guests touch the floor when the sheets are stripped. **Do not contaminate clean linens.
