Observed couch cushions in room 203 & 303 with a hole, and ripped. Observed sheer curtains in room 203 &109 ripped. Observed desk chair in room 203 & 109 with back cushion torn. Observed rubber black out curtain in room 203 & 109 badly stained. REPEAT

Repeat: Observed stained sheer and middle vinyl curtain lining on windows throughout facility. Observed stained mattresses in 503, 405, and 205. Observed torn/damaged pullout mattresses in 310 and 205. Repeat: Observed stained ceiling tiles and some with holes in them at the ends of each hall by the vending/ice/laundry and AC unit areas throughout. Observed hole in the wall by the laundry machines and ice machine on the 5th floor and in the 2nd floor laundry. Observed stained ceiling tiles on the 1st floor hall and in the fitness room. Observed the wallpaper peeling at the end of the hall on the 4th floor (stair case #1 side). 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

The presence of insects, rodents, and other pests shall be controlled to eliminate their presence on the premises and eliminating harborage conditions. Observed one decomposing rodent in bait trap on 2nd floor behind ice machine and standing water around ice machines on all floors. Observed live insects and dead insects in guest rooms and in hallways near AC units. -0-

Observed water stained/bulging ceiling tiles above ice machines on 1st, 2nd, 3rd, and 5th(missing tiles/leak) floors. Observed damaged window sill in room 307. Observed broken and damaged gaskets on refrigeration units in rooms throughout rooms on different floors. Replace damaged/split gaskets on refrigeration units in rooms. Damaged area on gasket was collecting trash and debris. Replace missing control knobs in refrigeration units in rooms throughout on different floor. Observed ink pen caps used as controls in refrigeration units in rooms on different floors. 15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair. 4-202.16 Non food contact surfaces shall be free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Observed ice machine chute and ice machine guard soiled throughout on all five floors of lodging establishment. Touch-furniture throughout in rooms with a furniture marker. Observed scuffed furniture throughout in rooms. Observed holes in wall behind doors in rooms where carpet was replaced and door stops not re-installed. Patch holes in walls behind doors and replace missing door stops. Observed shower heads throughout in rooms soiled. Thoroughly clean shower heads in rooms. Observed missing/damaged soap dish in shower of room 110. Replace missing/damaged soap dish in shower. Recaulk base of shower in room 110. Monitor and repair wall in room 218 and 204 bathroom. Replace damaged electrical outlet covers in rooms. Replace lamp covering in rooms stained with paint. Observed clean linen stored on uneasily cleanable surface(shelving) in house keeping cart storage room. Do not store personal belongings or lost and found items on or near clean linen. Do not store clean linen on bare wood shelving. Paint or apply wood sealant to any bare wood shelving where clean linen will be stored. Provide additional shelving to store boxes of new linen off the floor. 15A NCAC 18A .1828( c) Clean linen and supplies shall be stored in cabinets, or on shelves in linen and supply storage rooms.

Observed some build-up on larger white shower heads in bathrooms within rooms on 5th floor and 3rd floor. Thoroughly clean shower heads on bath fixture. Refurbish chipped spot on bath tub in room 516. Clean hair dryer and hair dryer holder in room 520. Caulk/seal back splash to lavatory counter in room 503. Bath, lavatory, and toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .1809 (a).

Observed water fountain mouth guard soiled on water fountain near laundry room/office area. Observed low water pressure on water fountain on water fountain near laundry room/office area. Thoroughly clean water fountain mouth guard. Increase water pressure on water fountain. Facilities for the dispensing of drinking water shall be of sanitary design. If drinking fountains are provided, they shall be of angle-jet type. 15A NCAC 18A .1811 (a).

Observed clean linen stored in a linen bin labeled soiled in laundry room area. Observed rooms being cleaned and comforters stored/placed on the floor while beds were being made. Store clean linen in clean linen bin. Do not store or place comforters on the floor while cleaning room and making beds. Soiled linen shall be so handled and stored as not to come in contact with clean linen. 15A NCAC 18A .1813 (c). Store linen in manner to prevent contamination.
