15A NCAC 18A .1826(a) Handwashing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. *** Observed no soap available at hand sink in laundry room. Ensure to provide all hand washing supplies at all hand sinks at anytime.

General Comment: 15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. *** Observed disrepair drawer in 318, disrepair strip on the counter and stained bathtub in 314, peeling ceiling in bathroom in 111. All furniture and fixtures in guest rooms shall be clean and good repair.

5-501.110 Refuse, recyclables, and returnables shall be stored in receptacles so that they are inaccessible to insects and rodents. *** Observed trash and debris on the floor, the side and behind the dumpster. Area need to be cleared and cleaned up.

General Comment: 15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to aoid contamination of food and food contact surfaces, linen, and single-use articles. *** Observed linen boxes stored on the floor in 1st floor storage room. Items need to be stored off the floor at least 6 inches at anytime.

Observed burn marks on counter under microwave in room 315 and deeply stained/burned counter spaces bathroom in room 130. Observed bathroom stoppers missing in multiple rooms (134, 219, 321) Cabinet in room 326 starting to peel. Under sink shelving in room 219 cracked. Bath tub in room 110 peeling. Sink in room 209 with toothpaste. Observed boxes of single-service articles on the floor in the pantry. Items were recently delivered. Ensure items are placed on shelving and stored above the floor.

Repeat: Observed sanitizer bottles at 50ppm Quat. CDI- Ran all of the air bubbles out of the line and refilled the bottles with the correct strength. Ensure employees are using the test strips to test their bottles. 4-501.114 Maintain sanitizer at correct concentrations when being used to sanitize. -P

No Points Deducted: Observed no back flow preventer on the shower wand in room 201. If one is built in to the shower head, provide the information during the next inspection. 5-202.14 Provide a backflow prevention device on water supply systems. -P

-Observed stained mattress pads and covers in rooms 201, 212, and 218. Each were removed from the bed and taken to the laundry room. -Observed a used paper towel in the refrigerator in room 201. Room was already cleaned. -Observed a bottle of vitamins in the cabinet in room 107. No Points Deducted: Observed a strong pet urine smell in room 212. Deep clean carpet. 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

Observed 2 sanitizer bottles at 0ppm chlorine, and 1 cart without sanitizer. The lavatory, its handles, faucet, and vanity shall be washed using an all purpose cleaner and rinsed daily if the room has been occupied. A sanitizer shall be applied to the lavatory and vanity using a method such as a wipe-on chemically-treated disposable sanitizing towel or equivalent. The sanitizer shall be left to air dry. Sanitizing solutions shall consist of iodophors, quaternary ammonium or other chemicals or methods that are equivalent to the action of 50 parts per million chlorine solution. 15A NCAC 18A .1809 (b).

-Observed a large amount of ants on and around the dumpster. Treat and clean the dumpster. All garbage and trash shall be collected and stored in covered containers in such a manner as not to create a nuisance. 15A NCAC 18A .1814 (a). -Observed the dumpster door open. Keep closed.
