15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair. Observed rusted microwave in room 321, 233, 127. Damaged/rusted shelving refrigerator in room 233, 204, 127, 321. Need repair or replace and keep in good repair.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed caulking needed to fill in a cap between bathtub and floor/wall in bathroom 301, 127.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed loosely/drooping exterior ceiling around building 3 rd floor. Need repair and keep in good condition. PIC stated that they are having someone to come and repair that issue. Observed damaged wall being patched need final touch up in room 204.

15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to aoid contamination of food and food contact surfaces, linen, and single-use articles. Observed boxes of pillows, linens, single used articles in storage room second floor stored directly on the floor. Keep items above the floor to prevent contamination.

4-603.14* Equipment food contact surfaces and utensils shall be effectively washed to remove or completely loosen soils by using the manual or mechanical means necessary. Observed guest ice machine chute in need of cleaning.

EHS reminded manager to ensure staff is properly trained to wash hands after handling soiled linen and before moving to clean linen. **15A NCAC 18A .1825 (c ) A hand washing sink, located to allow use by employees handling clean and soiled linen, shall be provided. This requirement shall be effective one year after the effective date of this Rule. **15A NCAC 18A .1826(a) Hand washing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.

Observed re-caulking needed on restroom vanities in rooms 304, 318, 238, 117,131,119, and 232. Observed lamp shades in several rooms with holes and in need of cleaning. Observed tub peeling in room 207. Observed cleaning needed on the base of the toilets in rooms 207, 131, and 318. Observed broken chair in room 238. Observed cleaning needed and stains on chairs in rooms. Observed window curtains with staining in 131 and 207. Observed debris left on the mattress pads of several rooms, EHS suggested having house keepers sweep out debris before making up beds. Observed stained sheets in rooms 207 and 238. **15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest.

Observed cleaning and touch-up paint needed on clean linen and clean supply storage shelving of laundry room. **15A NCAC 18A .1828 ( c) Cabinets, shelves, and storage rooms shall be in good repair and kept clean.

Observed personal drink being stored with clean linen and personal food being stored on top of clean supplies on house keeping cart of 3rd floor. Observed purse being stored with clean linen on house keeping cart of 2nd floor. **15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items.

Observed dumpster doors and lid left open. EHS advised checking dumpster after trash pick-up to ensure door and lids are shut. **5-501.15 Maintain refuse, recyclables, and returnables receptacles with tight-fitting lids, doors, or covers.

Observed vent cover with dust build up in room 340. Observed torn curtains in room 340. Observed shower rod cover damaged in room 339. Observed soap holder in tub cracked and heavy accumulation of build up in room 118. Needs to be repaired. **15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair.

Observed wall damage in rooms 340 and 339. Observed ceiling peeling over the tub and grout needed around base of tub in room 202. Needs to be repaired. **15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

Observed one roach in the dreesser in room 118. Pest management was on site upon arrival of inspection. **15A NCAC 18A .1826 (i) Maintain premises free of insects, rodents, and other pests.

Observed burn holes in covers in rooms 340 and 339. Ensure that once linen is damaged, it is replaced. **15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest.

Observed no sanitizer on the carts. Provide and use per the rule below. After cleaning the lavatory, faucet, and vanity, a sanitizer shall be applied to the lavatory and vanity using a method such as a wipe-on chemically-treated disposable sanitizing towel or equivalent. The sanitizer shall be left to air dry. 15A NCAC 18A .1809 (b).

Repeat: Observed residue and stains on the walls in 315, 328, and 220. Observed laminate floor damage in room 313. Observed the base of the tub area damaged, the restroom door damaged, and the restroom door along with the restroom ceiling with brown residue (able to be wiped off) in room 220. Observed the paint peeling on the walls and/or wall damage in several rooms. Floors, walls, and ceilings located in lavatories and baths shall be cleanable, durable, and shall be kept clean and in good repair. 15A NCAC 18A .1809 (c).

Repeat: Observed the window drapes soiled in several rooms that were inspected. Window coverings shall be kept clean and in good repair. 15A NCAC 18A .1812 (b).

Repeat: Observed holes and or stains on the blankets and mattress covers in several room inspected. Bed linens, including sheets, pillow cases, blankets and bedspreads, shall be kept clean and in good repair. 15A NCAC 18A .1812 (c).

Repeats: Observed rusted out microwaves (Inside), refrigerator gaskets with residue, soiled refrigerator shelves, damaged computer chairs, damaged desks, hair on furniture and inside a refrigerator, dead roaches in dresser drawers, and damage to the freezer door/shelf (inside) in multiple rooms inspected. Furniture, fixtures, carpets and other accessories shall be kept clean and in good repair. 15A NCAC 18A .1812 (d).

Repeat: Observed the dumpster lids open. Keep closed. All garbage and trash shall be collected and stored in covered containers in such a manner as not to create a nuisance. 15A NCAC 18A .1814 (a).
