Observed rusted refrigerators and shelving throughout the lodging facility. Observed dirty stained curtains on all the windows. One face cover missing and one face cover broken on the outlet in Rm# 112. Observed stained chairs and loveseat in all rooms.

Repair loose tile in Rm# 214 and 217 and repair holes in the wall throughout to include public toilet in the office area. Observed old food debris on the stairs in the back. Observed mildew on the ceilings and walls of several bathrooms. Observed mildew on the ceiling in several rooms Rm#112; 126 & 214. 15A NCAC 18A .1826 (g) Perimeter walls and roofs shall protect the lodging establishment from the weather, and the entry of insects, rodents, and other pests. Observed holes in the exterior wall of the building; damaged wood siding in the back of the building stairwell storage room. Remove all stored broken appliances and electronics from blocking the walk way on the second floor.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed stained on the top cover in serveral rooms (see below) Observed tiny bugs on sheets in Rm# 105; 116; 119; 121; 126; & 217.

Observed trash on stairs and at the doorway of serveral guest rooms. Observed broken storage in the hallway leading to the laundry room. PIC stated that it will be removed the next day.

Observed no sanitizer on housekeeping cart. Housekeeper was in the process of cleaning rooms. Ensure sanitizer is on cart at all times. **15A NCAC 18A .1824(h) Sinks in guest rooms shall be sanitized before washing multi-use utensils.

Observed build up on shower head in room 108. Observed filter vents in need of cleaning in room 122. **15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair.

Observed wall stain near the toilet in room 108. Observed black build up on walls and ceiling in the bathroom in room 110. Observed baseboard missing around tub in room 217. Observed fan cover missing in bathroom in room 217. Observed wall damage in room 110. **15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

Observed several roaches (dead and alive) during the inspection. Observed signs of bed bugs (black spots and eggs) behind head boards and around outlets in room 207. Observed no Quat sanitizer test strips. Provide. A method or equipment for testing the sanitizer shall be available and used to insure minimum prescribed sanitizer strength. 15A NCAC 18A .1809 (b).

Repeat: Observed moldy ceiling and wall in bathroom, room 106. **Floors, walls, and ceilings located in lavatories and baths shall be cleanable, durable, and shall be kept clean and in good repair. 15A NCAC 18A .1809 (c).

Repeat: Observed A/C filters dirty, several rooms. **In the absence of functioning windows, air conditioning and artificial lighting must be kept clean and in good repair. 15A NCAC 18A .1812 (a).

Observed stains and/or holes in the bedding (comforters, sheets, pads, etc) in several rooms. Bed linens, including sheets, pillow cases, blankets and bedspreads, shall be kept clean and in good repair. 15A NCAC 18A .1812 (c).

Observed chemicals not labeled and stored over & next to clean linen. Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. 15A NCAC 18A .1813 (e).

Observed furniture, box springs, mattresses outside by the dumpster and in the (old) pool area. Remove all. Rubbish, litter and other items not used in the operation of the establishment shall not be permitted to accumulate on the premises. 15A NCAC 18A .1814 (b).
