15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. - Observed items (chemical bottles, coffee cups and lids, broom) on housekeeping carts stored mix and match, chemicals touching coffee cups, broom hanging over coffee cups. Ensure to keep chemical or dirty items clearly separate and away from clean items on the carts at anytime.

15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to aoid contamination of food and food contact surfaces, linen, and single-use articles. - Observed water bottle cases and a box of coffee lids stored on the floor not on the shelves or off the floor in store storage room 2nd floor. Ensure to keep all clean supplies off the floor at least 6 inches at anytime.

15ANCAC 18A. 1824(k) A test kit or other device that measures the concentration in mg/L of sanitizing solutions shall be provided by the lodging establishment. Observed no test strips for quat sanitizer in lodging establishment. Hotel manager ordered test strips during inspection.

Observed loose toilet seat in room 129 and sofa cushion with minor stains in room 129. Observed damaged hinges in top drawer of dresser in room 327. Observed broken body lotion holders in rooms 319 and 430. REPEAT

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed minor stains on two pillows and both mattresses in room 401. Red marks and a few small holes on the curtains in the bed room of 401.

15A NCAC 18A .1828 (e ) Carts used for soiled laundry shall be labeled or identified for soiled laundry use only. Observed two carts used for soiled laundry without labels in laundry room per laundry room employee.

Stain on sofa bed mattress in room 225. Satin on curtain in room 108. Spot cleaning needed, and keep clean.

15A NCAC 18A .1825(a) Water, plumbing and waste shall comply with Chapter 5 of the food code as amended by Rule 15A NCAC 18A .2655. 5-202.13 Provide an air gap between the water supply inlet and flood level rim of the plumbing fixture or equipment at least twice the diameter of the water supply inlet. -P Observed ice machines in all the floors except the first floor without an air between water supply inlet and flood level rim of the plumbing fixture. Needs to be repaired.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed stains on the couch in rooms 118,127. Needs to be cleaned.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed dirty pillow covers and sheets in rooms 101,108,115. Needs to be cleaned.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. Observed chemicals stored in same place on the carts with linens. Needs to seperate the storage.

Interior of ice container in room # 303 is chipped and not cleanable. Facility will discard.

Sheets on bed in room # 115 stained with a red stain.

Doors to dumpster were open. CDI- Closed. Keep door closed.

15A NCAC 18A .1825 (c ) A handwashing sink, located to allow use by employees handling clean and soiled linen, shall be provided. This requirement shall be effective one year after the effective date of this Rule. Observed hand sink in laundry room was removed. Replace missing hand sink in laundry before October 1st, 2018.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed torn drape hem in room 202. Burnt microwave door, room 411. Trash below chair cushion, room 411. Most hallway tables had trash/personal belongings in them.

Observed chemicals and non-clean items stored with guest room supplies on house keeping carts. 15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items.

Observed multiple ice buckets damaged and collecting residue. One house keeper was not aware to wash the ice bucket and lid with soapy water prior to using sanitizer. When properly fitting disposable food grade liners are provided, ice buckets shall be treated as non-food contact surfaces and shall be washed, using an all purpose cleaner suitable for food contact surfaces, and rinsed. The lid used with the lined ice bucket shall be washed, rinsed and subjected to an approved sanitizing treatment in place. 15A NCAC 18A .1811 (b).

-Repeat Non-Compliance: Observed no sanitizer on 4 of the 6 housekeeping carts that were inspected.After cleaning the lavatory, faucet, and vanity, a sanitizer shall be applied to the lavatory and vanity using a method such as a wipe-on chemically-treated disposable sanitizing towel or equivalent. The sanitizer shall be left to air dry. 15A NCAC 18A .1809 (b).-Observed no test strips to test the quat sanitizer.A method or equipment for testing the sanitizer shall be available and used to insure minimum prescribed sanitizer strength. 15A NCAC 18A .1809 (b).

Repeat Non-Compliance: Observed guest related items (tissues etc) on the 3rd floor in the storage closet.Cabinets, shelves, and storage rooms shall be constructed to facilitate cleaning and kept clean. 15A NCAC 18A .1813 (b).

Observed: No sanitizer on the 4th floor housekeeping carts.Sanitizing solutions shall consist of iodophors, quaternary ammonium or other chemicals or methods that are equivalent to the action of 50 parts per million chlorine solution. 15A NCAC 18A .1809 (b).

Observed: Ice machine chutes on the 3rd and 2nd floors collecting residue/discoloration.Ice Machines, equipment, utensils, and the room or area in which the machines are located shall be kept clean and in good repair. 15A NCAC 18A .1811 (c).
