Observed cabinet stain under sink in room 1514.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed stained floors in the hallway on the 16th, 15th, 14th, 11th, 10th and 8th floors throughout. Observed carpet pulling away from the floor on the 12 and 11th floors. Observed closet door scuffed at room 1718.

15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests. Observed build up in corners and at edges of showers in 1021. Observed stained shades in 1712, and 1608. Observed stuck drawer in 1608. Observed wall damage from a removed fixture in rooms 1418 and 815 near the window and at the sink in room 1114. Observed chipping paint on door in 1220. Observed torn sheer in 1417. Observed laminate damaged near microwave in 1021. Observed worn edges on luggage rack in 1417 and 1114. Observed carpet damage in 1220, 1021, and 815. Observed loose toilet seat in room 815.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed bleach stained carpets throughout hallways in facility. Observed carpet damaged at transition in the 11th floor elevator lobby.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed stained mattress pad on pull out bed in 704. REPEAT VIOLATION Significant improvement from previous inspection as this was the only linen issue.

15A NCAC 18A .1828 (e ) Carts used for soiled laundry shall be labeled or identified for soiled laundry use only. Observed unlabeled carts used for dirty laundry in laundry area.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. Observed several housekeeping carts with clean linen stored on dirty side of cart. CDI: Linen separated. 15A NCAC 18A .1828 (d) Housekeeping carts shall be kept clean and stored to protect items from contamination. Observed 16th floor housekeeping cart with chemical spray bottles stored commingled with linen. CDI: Spray bottle removed.

15A NCAC 18A .1827(d) Sanitizing solutions shall not be stored in or dispensed from containers previously containing other poisonous or toxic materials. Observed bathroom cleaner-sanitizer stored in previous used chemical bottle at 16th floor housekeeping cart. CDI: Chemical removed.

Observed hair in the shower in room 1502. Needs to be cleaned. **15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests.

Observed mattress cover stained in rooms 1502 and 1208. Observed mattress cover torn in room 801. Linen were removed. **15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest.

Observed mattress staining in rooms 1706, 817 and 702. Ensure that mattress staining is removed. Observed vent filters in need of cleaning in rooms 1719, 1115 and 817. Observed minor wall damage in the storage closet on 15th floor. Needs to be repaired. **15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

15A NCAC 18A .1826(a) Handwashing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. Observed paper towel dispenser not functioning at handsink located in laundry room.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed burn mark outside on tub in room 702.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed carpet staining in the following areas: near rooms 1520, 1205, and 801. *NOTE: During time of inspection facility was operating at 30% capacity and floors 9,10, and 11 were not in service due to COVID-19.

15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests. Observed no sanitizer being kept on housekeeping carts on multiple floors. Sanitizer was checked and determined to be below 150ppm in spray bottles and coming from dispenser in laundry room. Sanitizer was filled up at proper conetration from kitchen. 15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed stain on duvet cover and mattress in rooms 908 and 702.

15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to avoid contamination of food and food contact surfaces, linen, and single-use articles. Observed storage on floor inside storage room closets on 9th and 16th floor.

15A NCAC 18A .1824(i) Single-use articles may be used if discarded after each use. Single use articles may be stored and handled to prevent contamination. Observed bottled water stored on floor in hallway storage rooms.

15A NCAC 18A .1825 (c ) A handwashing sink, located to allow use by employees handling clean and soiled linen, shall be provided. This requirement shall be effective one year after the effective date of this Rule. Observed hand sink in laundry needs to be accessible (not covered or blocked), the drainage needs to be repaired and soap and towels provided for hand washing after touching dirty.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed showers with black caulking around base and around shower door

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed blankets and pillows that not washed between guests being put onto floor or partially on floor in many guestrooms. Keep items that will be put back on the bed protected.

15A NCAC 18A .1828( c) Clean linen and supplies shall be stored in cabinets, or on shelves in linen and supply storage rooms. Observed boxes of supplies stored on floors in most closets. Keep storage organized and off the floor.

15A NCAC 18A .1827( c) Household cleaning agents such as bleaches, detergents, and polishes shall be used and stored according to manufacturer's recommendations. Observed a couple spray bottles of cleaner not labeled with contents.

Observed shower heads with build up on multiple rooms throughout (1716, 1605, 1521, 1408, 1221, 1114, 1001, 919, 804, 821). Repeat non-compliance. Bath, lavatory, and toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .1809 (a).

Repeat Non Compliance: Observed bottoms of shower curtains collecting pink residue in rooms 1020, 1419. Observed white calcium build up present on shower head in room 1512. **Bath, lavatory, and toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .1809 (a).

Observed storage on floor in closets throughout facility. **Cabinets, shelves, and storage rooms shall be constructed to facilitate cleaning and kept clean. 15A NCAC 18A .1813 (b). **Storage rooms or cabinets shall be provided for all supplies, linens, and equipment. 15A NCAC 18A .1813 (a).

Observed shower curtains with scale build-up and pink mold throughout.** Keep all fixtures clean and in good repair.

Observed no backflow preventers at shower wands in handicap accessible rooms. A backflow preventer is required where shower wands can fall below the flood rim of the bathtub.**Cross-connections are prohibited.

Observed trash below bed in room 1505 and 1215. Sticky kitchenette drawer and silverware tray in room 1505. Trash left in refrigerator in room 1505. Trash below chair cushion in room 1301. Broken sconce room 1110.**Furniture, fixtures, carpets and other accessories shall be kept clean and in good repair.

Observed a loose toilets seat in 1015. Shower heads collecting some pink hard water build-up in 819, 920, 1008, and 1501.Bath, lavatory, and toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .1809 (a).

Observed A/C unit filters with heavy accumulation of dust in 819, 1519 and 1619. In the absence of functioning windows, air conditioning and artificial lighting must be kept clean and in good repair. 15A NCAC 18A .1812 (a).
