REPEAT 15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair.- Observed bathtub along edges may need new caulking. Keep in good repair, so that when it is cleaned, it looks clean. (325, 101). Observed dusty bathroom vent in 304. Cleaning needed and keep clean. No point deduction due to improvement.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items.- Observed trash, and bag of trash stored above and next to clean supplies (cups) in the hanging hook pocket to the cleaning cart. Ensure to keep clean supplies and dirty items clearly separate and away from each other at anytime to prevent any possible cross contamination at anytime.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.- Observed missing ceiling tiles on 1st floor due to maintenance issues, leaks from above. Once dry out. Replace the ceiling tiles and keep in place.

15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair. Observed microwave with food debris in room 210. Recommend higher frequency and more thorough cleaning. -0 points-

Observed some pink and black mildew around the bathtub and along grouting wall. Need cleaning and keep clean. Observed rip and tear on desk chair in room 319. Repair or replace. Observed dirty air filter in room 215. Need cleaning and keep clean.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed damaged wall around the sprinkle on 3 floor. The building is in the process of checking/inspecting all sprinkles in the building according to housekeeper.

15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to aoid contamination of food and food contact surfaces, linen, and single-use articles. Observed boxes (towels, linen, single use article ..) in all storage rooms stored directly on the floor. Recommend to store all items off the floor at least 6 inches or on the crates or in shelving. To prevent any possible contamination.

REPEAT 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed shower rods damaged pulling away from walls inside guest rooms (#210, #321) in need of repair.

NO POINTS DEDUCTED 15A NCAC 18A .1825 (c ) A hand washing sink, located to allow use by employees handling clean and soiled linen, shall be provided. Observed no hand washing sink available in laundry room, Hand hygiene program will be utilized to meet rule compliance.

REPEAT 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed water damaged ceiling tiles in hallways for 1st and 2nd floors in need of replacing and repair. (points not doubled for repeat, due to general comment issued on the prior inspection).

Also observed stains on the mattress in room 322 and 102. Observed stains on pillows in rooms 102, 312, and 322.

Observed stained pillows in rooms 304, 313, 324, and 202. Observed stained shower curtains in 324, 202, 104, and 304. Observed cracked shower chair in 324 and shower chair in need of cleaning in 224. Observed caulking needed on the back of the vanity in 224. Observed holes in the mattress on 224 and stained mattresses in rooms 202, 224, and 104. Observed food left in refrigerator of 324. **15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair.

REPEAT Observed wall damage in second and third floor housekeeping laundry shoot closets. Observed water stained ceiling tiles in first and second floor hallway. Observed water damaged ceiling in laundry room. **15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

Observed stained sheets in rooms 304, 313, 324, 202, and 104. Observed stained towels in rooms 304, 313, 324, and 202. Observed stained mattress pad in 313, 324, and 202. Observed holes in the mattress pad of 104 and in the fleece blanket in 313. **15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest.

REPEAT Observed dumpster with holes in the bottom. Have dumpster replaced. Observed ground under dumpster to be sand. Resurface dumpster storage surface. Observed debris collecting in dumpster enclosure area. **5-501.111 Storage areas, enclosures, and receptacles for refuse, recyclables, and returnables shall be maintained in good repair. **5-501.11 An outdoor storage surface for refuse, recyclables, and returnables shall be constructed of a nonabsorbent material, be smooth, durable, and sloped to drain. **5-501.115 A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean.

Observed several spray bottles of chemicals in laundry room and under hand sink in dining room without labeling. **15A NCAC 18A .1827( c) Household cleaning agents such as bleaches, detergents, and polishes shall be used and stored according to manufacturer's recommendations.

Observed stained and missing ceiling tiles in 100 and 200 hallways. Floors, walls, and ceilings located in Lobby, Halls, Stairs, and Vending Areas, shall be kept clean and in good repair. 15A NCAC 18A .1808 (b).

Observed shower basins damaged and stained throughout facility. Observed damaged caulk on vanity throughout facility. Observed Bath, lavatory, and toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .1809 (a).

Observed refrigerator and chemical buckets stored in dumpster blind. Rubbish, litter and other items not used in the operation of the establishment shall not be permitted to accumulate on the premises. 15A NCAC 18A .1814 (b).
