Observed no paper towels or hand drying device at handsink in laundry room. (EHS did observed soap at handsink, points not escalated due to some improvement).

Observed burn markings on tub in rooms 117, 151 and 245. Observed backside of drapes ripped in rooms 105 and 234. Observed AC removable vent broken in room 138.

Observed dumpster sliding door opened during time of inspection. 5-501.115 A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean. Observed several bags of trash stored outside of dumpster area. Observed mattress stored outside of dumpster area.

Observed hair in the tub in room 217 which was cleaned. Ensure tubs are thoroughly cleaned when cleaning the bathroom. **15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests. Observed torn mattress and torn mattress in pull out sofa in room 217. Observed stained mattress in rooms 211, 129 and 244. Observed stained shower curtain in room 229. Needs to be cleaned. Observed housekeeping cart on the second floor in need or cleaning. Debris items need to be cleaned from cart. Ensure that carts remain cleaned and organized to reduce possibility of contamination. **15A NCAC 18A .1828 (d) Housekeeping carts shall be kept clean and stored to protect items from contamination.

Observed the flooring and ceiling in the hallways with residue/debris and in needed cleaning, ceiling leaks at both ends of the building near the 2nd floor storage room and stairwell, ceiling panels missing in the employee lounge, ceiling fan was dusty in the lobby, and a section of vinyl as missing on the north side stairwell. **Continue with all repairs and clean all exterior floors, walls, and ceilings.

Observed the ice machine interior shield (touches formed ice cubes) and the ice shoot with pink/black residue. **Ice Machines, equipment, utensils, and the room or area in which the machines are located shall be kept clean and in good repair. 15A NCAC 18A .1811 (c).

Observed the carpet was worn and had a burn in room 117, and some rooms with worn unraveling carpet. **The floors, walls, and ceilings of bedrooms, closets, and storage areas shall be kept clean and in good repair. 15A NCAC 18A .1812 (d).
