General Comment: 15A NCAC 18A .1828 (e ) Carts used for soiled laundry shall be labeled or identified for soiled laundry use only.- Observed yellow/blue linen carts in laundry room labeled “Dirty Only”, but is using for clean linens. The cart that labeled “Clean Only” is using for dirty linens. Ensure to clearly use correct cart labels for correct type of linens. Do not mix and match dirty and clean. Keep them clearly separate at anytime.

REPEAT 15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to aoid contamination of food and food contact surfaces, linen, and single-use articles.- Observed boxes of supply (cup, toilet tissue,..) stored on the floor in storage 5th floor. Ensure to store and keep all clean supplies off the floor at least 6 inches at anytime. Points remain the same at this time.

Ensure to provide all hand washing supplies at all hand sinks at all time.

Observed no sanitizer test kit in the laundry room at the time of inspection. Manage provided at the later time of the inspection. Recommend for each stationary (kitchen or laundry room) to obtain their own test kit.

Observed rip and tear on both sides of a desk chair in most of guest rooms. Repair/Replace and maintain in repair. Observed minor food debris in microwave in room 508, 320, 205. Need cleaning and keep clean. Observed burned mark on bedside desk in room 401 also observed lots of lady bugs on the carpet, Need vacuum and keep clean. Observed flickering light above the sink and refrigerator in room 320, repair or replace.

Observed boxes on single used articles (cups, paper towels, pillows..) stored directly on the floor in storage room of 4th floor. Recommend to keep everything off the floor at least 6 inches above the floor to prevent any possible contamination.

Observed carpets stains throughout the rooms 418, 506, 517, 316.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed torn sheet in room 410.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. Observed personal items (cell phone and drink) on housekeeping cart on 5th floor. Observed broom on 4th floor housekeeping cart stored with Clean linens. Relocate the broom and personal items to prevent contamination.

4-501.114 Maintain sanitizer at correct concentrations when being used to sanitize. -P Observed sanitizer not at the correct strength and no test strips on site. Therefore the sinks are not being sanitized correctly.

Room 501 had tiles cracked and needed to be resealed around the jacuzzi tub-also black glue residue on tiles. Cracked wall neare the tub. Old door jam has left a hole in the doorway. Room 410 wall damage by the window. Shower liner coming away from the wall-needs to be resealed. Room 304-water stain in cabinet underneath the sink. Room 114 had water damage in the corner of the wall near the shower.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed mattress pad stained in room 511.

15A NCAC 18A .1828 (d) Housekeeping carts shall be kept clean and stored to protect items from contamination. Observed a red bull sitting directly on top of single use coffee grounds. Needs to be stored to prevent cross contamination.

Observed 2 leaks in ceiling inside laundry room in need of repair (one leaking into can wash and other into 30 gallon trash can). Observed water from washing machine dumping into strainer box that is rusted out puddling into floor of laundry room; (small leak, should be able to repair without replacing). Observed wall beside dryers in need of repair inside laundry room.

5-501.15 Maintain refuse, recyclables, and returnables receptacles with tight-fitting lids, doors, or covers. Observed dumpster having both lids and doors open; lids are broken in half in need of replacing. 5-501.115 A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean. Observed old refrigerator unit stored within garbage enclosure.

Floors, walls, and ceilings located in Lobby, Halls, Stairs, and Vending Areas, shall be kept clean and in good repair. 15A NCAC 18A .1808 (b). Observed damaged wallpaper in hallways. Stained ceiling tiles in 1st floor vending area. Chipped paint on room doors. (Hotel is in process of being re-painted)

Ice Machines, equipment, utensils, and the room or area in which the machines are located shall be kept clean and in good repair. 15A NCAC 18A .1811 (c). Observed ice machine on 3rd floor with residue in the chute. Needs cleaning.
