15A NCAC 18A .1826(a) Handwashing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. Employee restroom lacked hand drying device. Paper towels added.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Wall next to dish machine in bad repair. Sink in room 108 not attached to drainboard.

Observed chair cushion peeling in room 327.

Observed dust on ac filters and black residue on ceiling of bathroom in room 104. 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

Hoses observed in mop sink, below flood level rim at time of inspection. 5-202.13 Provide an air gap between the water supply inlet and flood level rim of the plumbing fixture or equipment at least twice the diameter of the water supply inlet. -P

Repeat. Observed stained and torn fabric on couches in rooms 321, 304, 218 and 227. Chair slightly ripped fabric in recliner piece in room 304. Some bed bug mattress covers observed with stains in rooms 218, 227 and 127. Small hole in cabinet over sink area in room 127. Broken arm on couch in room 321 and chipped table in room 321. 15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Approved sanitizer not available or being used in required areas. Ensure approved sanitizer (checked with test strip) is applied in correct areas. Toilets and sinks on main floor in need of cleaning. 15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests.

Trash area floors in need of cleaning. Vent in main restroom broken (making noise) and in need of cleaning. Some staining on carpet outside of 1st floor trash room. 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

Repeat. Observed boxes of clean linen stored on floor of storage in conference room, storage room 3rd floor and main laundry room. 15A NCAC 18A .1828( c) Clean linen and supplies shall be stored in cabinets, or on shelves in linen and supply storage rooms. Some of these items are to be donated or washed before using in hotel. Keep off floor, on shelves.

Bottle of employee water stored on cart. 15A NCAC 18A .1828 (d) Housekeeping carts shall be kept clean and stored to protect items from contamination. Keep stored separate from other items. Manager moved and corrected.

Observed multiple improperly labeled spray bottles. Bottle had blue chemical labeled as quat sanitizer. One bottle reused or labeled "bleach" with different chemical. Multiple unlabeled bottles of chemicals. 15A NCAC 18A .1827( c) Household cleaning agents such as bleaches, detergents, and polishes shall be used and stored according to manufacturer's recommendations.

15A NCAC 18A .1828( c) Clean linen and supplies shall be stored in cabinets, or on shelves in linen and supply storage rooms. Observed boxes of pillows and toilet paper stored on floor of 3rd floor storage room and clean linen and pillows stored on floor of main laundry room.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. Observed 4 housekeeping carts with soiled brooms/dusters stored dirty side up over clean linen, towels, and customer items. Either store dirty side down or cover with trash bag when not in use.

15ANCAC 18A. 1824(k) A test kit or other device that measures the concentration in mg/L of sanitizing solutions shall be provided by the lodging establishment. Observed no test strips available

15A NCAC 18A .1827( c) Household cleaning agents such as bleaches, detergents, and polishes shall be used and stored according to manufacturer's recommendations. Observed improperly labeled spray bottle. Bottle had window cleaner labeled as quat sanitizer.

Observed heavily stained mattress cover in room 225. 15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest.

Replace stained and damaged gasket on refrigeration unit, clean interior of refrigeration unit , clean stain on oven door and thoroughly remove all item (guest personal belongings) from cabinet area in room 307. Repair laminate on kitchen cabinet door in 321. 15A NCAC 18A .1824(e ) Nonfood-contact surfaces of equipment shall be clean.

Remove tape from towel rack in 310 and repair. Repair corner of couch on arm rest and thoroughly clean mattress cover in 210. Clean/dust top of hair dryer in 225. Repair broken towel holder in 224. Clean under couch cushion in 129. Replace missing light shield over light fixture in 129. 15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair.

Clean or replace (1)ventilation register in breakfast foyer area. Repair small hole on exterior siding of motel on back pool side of motel. 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. 15A NCAC 18A .1826 (g) Perimeter walls and roofs shall protect the lodging establishment from the weather, and the entry of insects, rodents, and other pests.

Observed clean linen stored on top of cleaning supplies on house keeping cart. Observed employee personal drink stored on and near clean linen on house keeping cart. Arrange items on the house keeping cart so clean linen and utensils are not touching cleaning supplies. Store employee personal belonging away from clean linen. 15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items.

Observed a chair stored behind dumpster within dumpster corral. Observed an old couch stored behind dumpster corral. Observed old pallets stored through walk-way of back wall. Remove old chair, couch and pallets from premises. 5-502. 12 Refuse, recyclables, and returnables shall be removed from the premises by way of portable receptacles or a transport vehicle.

Repeat Non-Compliance: Observed the water temperature in all the rooms inspected today 132F-133F. Hot water in guest rooms shall be provided at a temperature of no less than 116°F (46.7°C) and no more than 128°F (53.3°C). 15A NCAC 18A .1810 (e).

-Observed stained mattress cover in room 308 & 204. -Observed debris on the pullout mattress in room 204. -Observed hair in the bed and pillow cases in room 308. Bed linens, including sheets, pillow cases, blankets and bedspreads, shall be kept clean and in good repair. 15A NCAC 18A .1812 (c).

Repeat Non-Compliance: -Observed crumbs and debris in the drawers & refrigerator in room 308 (kitchen). -Observed the laminate on the kitchen cabinet doors peeling in 204 &118. -Observed damage to the kitchen counter top in room 308. -Observed stained pillows in rooms 308 & 204. Furniture, fixtures, carpets and other accessories shall be kept clean and in good repair. 15A NCAC 18A .1812 (d).
