15ANCAC 18A. 1824(k) A test kit or other device that measures the concentration in mg/L of sanitizing solutions shall be provided by the lodging establishment. Observed expired test strips in laundry room. PIC contacted Ecolab for new test strips.

REPEAT 15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed stained pillow cases or sheets in rooms 812, 710, 714, and 715. Observed hair on sheets in 819 pillowcases 408 and towels in 715. Observed torn sheet in 612.

5-501.114 Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place. Observed missing drain plug on blue dumpster at the side of building.

Observed boxes of linen, single used items stored directly on the floor in storage, 2nd, 4th, 5th, 7th, 8th and laundry room. Store all items above the floor.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed microbial growth in several storage room areas on exterior of ventilation system and walls in 5th floor and 4th floor storage areas. Observed dust in vents in several restrooms .

Observed debris on utensils and/or in the container in rooms 306, 823, 15A NCAC 18A .1824(c )* All kitchen ware and food contact surfaces of equipment, excluding cooking surfaces of equipment, shall be cleaned and sanitized, as required in Parts 4-6 and 4-7 of the Food Code as amended by Rule 15A NCAC 18A .2654 after each use, air dried, and stored in a manner to prevent contamination.

Observed stains and/or holes on mattress skirts, bed mattresses, pull out couch mattresses and/or mattress cover in rooms 306, 417, 823, and 619. Observed rust in tubs in rooms 306, 823, and 619. Repeat: Observed microbial growth on the wall in the 4th floor storage room. Observed microbial growth in the AC/Heating unit in the 3rd floor guest laundry room. Observed wall paper peeling in the halls. Observed the carpet fraying in room 823. *Note: Hall walls on the 3rd-7th floors are under construction due to a past leak. 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

Hose observed in sink in laundry, below flood level rim, no backflow prevention or air gap. 5-202.13 Provide an air gap between the water supply inlet and flood level rim of the plumbing fixture or equipment at least twice the diameter of the water supply inlet. -P

Vents in room and on halls observed with dust and buildup. Under cabinets in rooms 314, 601, observed stains. Room 508 wall by shower peeling. 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

New mattress stored on floor in laundry room. No labeling on soiled linen carts throughout facility and in laundry area. Some housekeeping using soiled linen carts for clean sheets, others using same carts for soiled linen. Clean linen stored on floor in storage rooms and in laundry storage. Observed soiled employee jacket stored on clean linens. Multiple housekeeping carts observed with chemicals and employee personal items stored over and on on top of clean linens and items for guests rooms. 15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. 15A NCAC 18A .1828 (d) Housekeeping carts shall be kept clean and stored to protect items from contamination.

Observed employee jacket stored directly on new clean silverware being used for guests rooms. Observed personal bottle of water stored on clean plates in storage for guest rooms. Observed stained carpet on each floor. Observed paint peeling from the ceiling on 8th floor hall and in bathroom of room 410 , frayed carpeting in room 717, room 516 and room 314. Observed black mold on ceiling and around the molding at the base of the wall in the bathroom. Observed stained ceiling tile on the 8th floor hall. Clean A/C vents near laundry room on 3rd floor and on 8th floor near room 819 observed damaged vent cover .
