15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed damaged walls near laundry chutes in storage rooms on floor 7 and 8. Points not escalated due to correction of previous violations.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed torn pillowcase in 719 and 721. Observed small ink spots on sheets in 720

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed yellow stain on mattress protector in room 1002. Observed damaged desk and curtains in room 909. Observed ripped desk chair in room 620.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed hole in wall in room 807. Observed carpet lifting from floor going into bathroom area in room 620. Observed low grout in the shower in rooms 1526 and 620.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. - Observed stained mattresses in rooms 1420, 1209,1119, crib mattress (storage), 707, 624. Observed Carpet stained in room 1420 REPEAT 15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests. Observed shower grouting in room 1011 shower stained. Observed shower head with pink residue in the following rooms: 1011 and 624. Clean and maintain.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed HVAC unit filters in the following rooms accumulating dust: 1420 and 1209. Observed floor under head board in room 1011 in need of cleaning. Clean and maintain. Observed bulb cover missing in the shower light in room 624. Replace.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed mattress cover stained in the following rooms: 1119, 1011, and 624.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items.Observed chemicals stored above clean linens on house keeping cart. Separate to prevent cross contamination.

15A NCAC 18A .1827( c) Household cleaning agents such as bleaches, detergents, and polishes shall be used and stored according to manufacturer's recommendations. Observed blue and pink chemicals in different spray bottles (floor 15); Observed chemical in unlabeled bottle in storage room (floor 7)

Observed stain on the mattress in room 1409. Ensure that mattresses are cleaned once visibly soiled. **15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair.

Observed wall damage in the storage rooms on the 14th, 11th, 9th and 7th floors. Needs to be repaired. **15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

Observed cups stored on the housekeeping cart not protected. Ensure that cups and lids are covered while on the carts to protect from possible contamination. **15A NCAC 18A .1828 (d) Housekeeping carts shall be kept clean and stored to protect items from contamination.

Observed staining on mattresses in rooms 1227, 1119, 923, 807, 716 and 623. Observed stained mattress pad in Room 1516 and 1227. Observed cigarette burn in mattress pad in room 1516. REPEAT

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed water stains on ceilings in 6th floor conference room and wall and ceiling stains in room 716.

Observed debris in vents of a/c unit in room 1420 and carpet on 15th floor in need of vacuuming. Recommend frequent cleaning of air vents and filters in all rooms and higher frequency of vacuuming in hallways and common areas.

Observed debris in and on heating and cooling unit covers in 621,1520 and 1411. Clean covers in and on heating and cooling units. Wipe/dust top of television in 1526. Clean corner of showers in 1401 and 727. Replace missing shower gasket in 1401. Clean ceiling ventilation in bathroom in 1201, 1220. Replace damaged ceiling ventilation cover in bathroom located in room 1220. Replace damaged window covering in room 809. Wipe/clean wall near entrance to bathroom in room 809.
