Observed trash piled up outside dumpster in dumpster enclosure. Needs cleaning.

15A NCAC 18A .1825 (c ) A handwashing sink, located to allow use by employees handling clean and soiled linen, shall be provided. This requirement shall be effective one year after the effective date of this Rule. Observed no handsink in laundry room for employees to use handling clean and soiled linen. Facility needs to install a handsink in laundry room or develop an approved hand hygiene written standard operational procedure approved by the regulatory authority. NO POINTS DEDUCTED.

Observed lids and doors open on dumpster. 5-501.110 Refuse, recyclables, and returnables shall be stored in receptacles so that they are inaccessible to insects and rodents. Observed bags of trash stored on floor / ground of hallways and near stairwells on each floor of building. 5-501.111 Storage areas, enclosures, and receptacles for refuse, recyclables, and returnables shall be maintained in good repair. Observed large amounts of trash piled up behind and beside dumpster in need of cleaning to prevent the harborage of pest and vermin.

15A NCAC 18A .1826(i) The premises and guest rooms shall be maintained free of insects, rodents, and other pests. The presence of insects, rodents, and other pests shall be controlled to eliminate their presence on the premises and eliminating harborage conditions. Observed bed bugs present in guest room #229; Room and adjacent rooms will be pulled from renting and treated by facilities pest control company; visit will be made to ensure treatment.

Replace missing ventilation covers in bathrooms throughout in different rooms. Ventilation covers have been purchased but not installed yet. Replace stained and peeling toilet seat in room 103 and 245. Replace damaged toilet tank cover in room 245 and 255 bathroom. Clean shower curtain and shower head in room 103. Tightly secure loose slip resistant material in base of bath tubs throughout in rooms. Repair base of tub around drain in 131 and 148. WIpe/clean top of hair dryer device holder in room. Repair/ refurbish chipped spot and burn mark on bath tub. Bath, lavatory, and toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .1809 (a).

Continue to touch-up furniture with furniture marker in rooms. Clean spots/stain on red chair. Replace broken glass in picture in room.. Defrost and clean ice from freezer section of refrigeration unit in rooms. Replace rusted shelving in refrigeration. Repair loose/damaged gasket on refrigeration unit in room. Replace refrigeration unit in room 131 with damaged interior door. Repair backs on television in rooms. Observed pillows stored on floor while room was being cleaned. Highly recommend storing pillow on furniture or chair while beds are being made. Furniture, fixtures, carpets and other accessories shall be kept clean and in good repair. 15A NCAC 18A .1812 (d).
