Observed pink residue on shower heads in rooms 105 and 314. Observed dusty vents in a/c units in several rooms. Observed stained mattress in 212. Needs cleaning. REPEAT.

Observed window covering damaged in room 521. Observed head board in room 521 and 103 damaged. Observed foot stools in room 521, 413, and 315 damaged and torn.

Observed disposable cup lids not protected on housekeeping cart. Ensure that items are stored in a manner to prevent possible cross contamination. **15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items.

15A NCAC 18A .1824 (b) Equipment and utensils shall be kept clean and in good repair. Observed black build up inside of ice chute located on first floor. General cleaning is encouraged to ice machine chutes throughout facility.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed staining on couch in the following rooms: 522, 317, 310, 216, and 214. Observed staining on carpet in main hallways throughout building,

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed minor staining on bed linen in room 522.

5-501.115 A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean. Observed several large bags of trash stored outside of dumpster. Observed dilapidated luggage cart stored outside of dumpster.

15A NCAC 18A .1824(i) Single-use articles may be used if discarded after each use. Single use articles may be stored and handled to prevent contamination. Observed boxes of toilet paper stored on floor in storage rooms.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed wall damage in laundry area and scuffs on walls in hallways on first floor.

Observed mattress and mattress cover stained in 520 also shower curtain with pink residue(bottom), pink residue in showerhead in 410. Stains on couch in 218,219,319,321,and 409. Stained carpets in 219, stained decorative pillows in 409, coffee stain on table in 409. Observed wall damage above AC unit in 219. Observed water stained ceiling tiles in exercise room. Observed torn blinds 410 and 218. 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

Observed stained pillowcase covers in 409 and stained sheets in room 218. 15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest.

Observed sanitizer solution (quaternary ammonia)too weak on two house keeping carts on 5th floor. Provide properly mixed sanitizer at all times so vanity within rooms can be cleaned and properly sanitized. Sanitizer in other sanitizer bottles on house keeping carts was reading to the correct strength 200PPM-400PPM. Make sure sanitizer dispenser unit is primed before filling sanitizer bottles. Sanitizing solutions shall consist of iodophors, quaternary ammonium or other chemicals or methods that are equivalent to the action of 50 parts per million chlorine solution. 15A NCAC 18A .1809 (b).

Store cleaning supplies and personal belongings below and away from clean linen and single service articles on house keeping cart. Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. 15A NCAC 18A .1813 (e).
