Observed stained couch in room 519 and stained and torn desk chair in room 315. Points not escalated due to less items and different items observed from previous inspection.

15ANCAC 18A. 1824(k) A test kit or other device that measures the concentration in mg/L of sanitizing solutions shall be provided by the lodging establishment. Observed no quat sanitizer test strips.

15A NCAC 18A .1826 (d) Light intensity shall be at least 20 foot candles at 30 inches above the floor in areas for handwashing, warewashing, equipment storage, utensil storage, and in toilet rooms. Observed lighting intensity <20 foot candles in housekeeping closet on 2nd and 3rd floor.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed separating veneer on mini-fridge cabinet in rooms 525 and 225.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed loose baseboards in rooms 409, 318, and 309. Observed missing electrical wall plate in rooms 419 and 309. Observed hole in wall by door in housekeeping closet on 2nd and 3rd floor. Points not escalated due to different items observed. Damaged wall in 3rd floor housekeeping closet was repaired but hole has reappeared.

Observed dust on ac filter in hall near 4th floor housekeeping closet and in room 524. Observed wall damaged beside shelving in 3rd floor housekeeping closet.

Ice machine on 3rd floor in need of cleaning, observed with mold/mildew buildup on chute. 4-601.11(A)* Equipment food contact surfaces and utensils shall be clean to sight and touch. -Pf. No points taken.

Some cabinets in guest rooms observed with scrapes and not in good repair. One stain on mattress in room 425. Two mattress found with hair in room 224 and 500. Overall, improvement made from last inspection notes. Different staff using different products on sinks, vanities and showers. Some reagents (Terrifico, supposed to be quat ammon sanitizer) being used on sinks and vanities did not have any active sanitizer when tested. 15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests. 15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair.

Vents throughout (in hallways and in guest rooms) establishment observed soiled and in need of cleaning. Hole in wall by night stand in room 224. Hole in wall in housekeeping room on 5th floor. Walls in restrooms with some tears, in need of repair. Manager stated these restrooms are being remodeled soon. 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

Labeling faded on carts in laundry room and cart labeled "soiled" being used for clean laundry. Per housekeeping, this cart has been sanitized. Manager labeling carts. Some boxes of new clean linens stored on floor in main laundry (repeat of general comment from last inspection). 15A NCAC 18A .1828 (e ) Carts used for soiled laundry shall be labeled or identified for soiled laundry use only. 15A NCAC 18A .1828( c) Clean linen and supplies shall be stored in cabinets, or on shelves in linen and supply storage rooms.

Two housekeeping carts observed with personal items stored over and on top of clean room supplies. 15A NCAC 18A .1828 (d) Housekeeping carts shall be kept clean and stored to protect items from contamination. Overall, carts were clean and items separated. No points taken.

Litter, appliances and items around dumpster and outside of dumpster enclosure. 15A NCAC 18A .1827(a) The premises shall be kept free of litter and items unnecessary to the operation or maintenance of the lodging establishment, such as equipment that is nonfunctional or no longer used. No points taken

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed stained mattresses in rooms 511, 324, 224, 101, and stained pull out mattress in room 407. Also observed debris under mattress in room 401 and several strands of hair on cleaned shower curtain and bathtub in room 525.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed cleaning needing to be done on ac filters on 5th floor hallway, room 511, 4th floor hallway, and room 324.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed stained bed skirts in rooms 511 and 122.

15A NCAC 18A .1828 (a) Bed and bath linen shall be provided for each guest and shall be changed between successive guests. Observed stained pillow covers, sheets and hair in rooms 122, 124, 126. Needs to be properly cleaned and changed between uses. The rest of the rooms were properly cleaned.

15A NCAC 18A .1828 (e ) Soiled laundry shall be handled and stored separately from clean laundry using separate, cleanable carts or bags. Observed clean linen stored in dirty container. Needs to be rewashed and stored properly.

5-501.16(A) An inside storage room or outside storage area and enclosure shall be of sufficient capacity to hold all refuse, recyclables, and returnables that accumulate. Observed refuse accumulated on the floors of storage area. Needs to be cleaned and placed in a storage or receptacle.

Observed minor cleaning needed in the cracks of drink holders on sides of refrigerator doors in several rooms including 523, 409, and 411. EHS advised using a small bristled brush to remove build-up. **15A NCAC 18A .1824(e ) Nonfood-contact surfaces of equipment shall be clean.

Observed re-caulking needed on the back of hand sink back splash in rooms 501, 307, and 516. Observed food crumbs that had been left under basket drawers in room 411. Observed desk chair with a split where stuffing is exposed in room 318. Observed staining on desk chair in room 201. Observed crumbs from cookie by door to stairwell on 2nd floor carpet in need of cleaning. **15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair.

Observed ceiling in need of repair by the window in 305. Observed side of entry door in need of repair in 305. Observed water stained ceiling tiles in fitness center and in 1st floor hallway. Observed dumpster lid to be open. Trash collectors came this morning. GM and houseman typically close this back after every trash pick-up. **5-501.113 (B) Maintain tight fitting lids or doors on outside receptacles.
