Observed tub needing to be resurfaced or re-glazed in room 411. 15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair.

Observed black residue/spotting on ac vents in rooms 302 and 315. 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

15A NCAC 18A .1824(i) Single-use articles may be used if discarded after each use. Single use articles may be stored and handled to prevent contamination. Observed coffee cups and coffee cups lids not individually wrapped. Dispose of any non-wrapped single service between guests. Protect unwrapped single service in storage rooms.

15A NCAC 18A .1825 (c ) A handwashing sink, located to allow use by employees handling clean and soiled linen, shall be provided. Observed hand sink in laundry room (2 bay utility sink) (hand sink is left side bay, chemical dispensing right side) full of empty spray bottles and no paper towels in dispenser. Keep sink (left side) empty of all items and keep soap/paper towels stocked.

15A NCAC 18A .1825(a) Water, plumbing and waste shall comply with Chapter 5 of the food code as amended by Rule 15A NCAC 18A .2655. Observed ice machine on 1st floor with internal drain clogged and backing up into bin. Leak at external plumbing of same ice machine on dripping onto floor. Staff says this continually happens. Have ice machine repairman/rep of ice machine company properly repair ice machine so operates as designed.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed floor under ice machine 1st floor soaking from leak with sludge build up behind and under machine. Get ice machine properly repaired. Staff lounge has stained and missing ceiling tile. Repair whatever is leaking. Replace ceiling tiles.

15A NCAC 18A .1828 (b) If bed covers are not cleaned between successive guests, the upper sheet shall be folded under the mattress at the lower end and folded over the bed cover a minimum of 6 inches. Observed coverlet not changed between guests. Top sheet is folded 6 inches over coverlet, but then coverlet is folded back over top sheet so there is not 6 inches of separation.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. Observed housekeeping carts with clean linens in contact with staff shoes, staff beverages, brooms, lint rollers, spray bottles. Coffee cup lids unprotected on carts. (Coffee cups on carts individually wrapped).

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Clean stain from drip on carpet on 4th floor hallway. Vacuum dirt debris on stairwell. Repair locking mechanism on door on 2nd floor.
