Observed no paper towels or hand drying device at handsink in main laundry room. Paper towels were supplied to handsink during time of inspection. -General comment.

Observed carpet staining throughout facility on main walkways. Observed loose toilet seat in rooms 316 and 430. Observed laminate peeling on chair in rooms 103 and 305. Observed staining on lamp shade in room 103. Observed part of blinds torn in room 210. Observed part of ceiling tile torn outside of room 316. REPEAT VIOLATION.

Observed 1 sliding door opened to dumpster. 5-501.115 A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean. Observed several wooden pallets, a tire, and minor litter stored within dumpster area. REPEAT VIOLATION.

15ANCAC 18A. 1824(k) A test kit or other device that measures the concentration in mg/L of sanitizing solutions shall be provided by the lodging establishment. Observed no test strips to measure the strength of sanitizer as required in notated rule. -General comment.

Observed dysfunctional paper towel and soap dispensers at the hand washing sink in laundry room, it is causing no soap and paper towel to be available for hand hygiene. CDI- A roll of paper towel and small soap container provided at the time of inspection. Ensure to provide hand washing supplies at the hand sink at all time.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items.- Observed housekeeping cart need to be rearranged clean with clean, dirty with dirty. No personal food/snack on the cart. Keep the carts clean and organized.

REPEAT 5-501.114 Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place. Observed a dumpster missing a drain plug. Need a replacement.

Observed toilets need better cleaning in room 418, 334. Damaged chair in room 426 and damaged shelve in room 316, need repair or replace. Observed the base of cabinets under the sinks need cleaning and keep clean throughout. No point escalating.

15A NCAC 18A .1826 (i) Maintain premises free of insects, rodents, and other pests. Observed dead bug under a kitchenette sink in room 426. PIC stated the pest control treatment was done couple days ago. Need cleaning.
