Observed wall peeling at 7th floor elevator.

15A NCAC 18A .1828 (d) Housekeeping carts shall be kept clean and stored to protect items from contamination. Observed 3 housekeeping carts with duster stored beside Single service cups. CDI- Rearranged. Keep items separate.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed minor wall damage near curtains in rooms 2121 and 1713. Observed minor carpet stain in hallway near room 1619.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed stained mattress covers in rooms 1818, 1617,1618, 602. Observed stained pillow and couch cover in rooms 1702.

15A NCAC 18A .1828( c) Clean linen and supplies shall be stored in cabinets, or on shelves in linen and supply storage rooms. Observed clean linen stored with employee food, jackets and bags in storage rooms on floors 21, 19, 16, 14, 12 and 6.

5-202.13 Provide an air gap between the water supply inlet and flood level rim of the plumbing fixture or equipment at least twice the diameter of the water supply inlet. -P Observed ice machine drain from ice machine on 6th floor with no air gap.

Observed laminate damaged on pull down door under sink in rooms 1212, 1109, 810, and 604. Observed stained headboard in 1509. Observed broken shower head in 1711. Observed loose toilet seat in 1822. Observed stained window shades in 2111, 1920, 1907, 1511, and 1403. Observed holes in shades in 2219, and 2111. Observed build up on the shower wand in 2219 and 1109. Observed paper bags remaining from food in dresser drawer of 1212.

Observed book bags, jackets, and purses stored on top clean linen in carts of the housekeeping closets on floors 18, 16, 14, 12, 9, and 7. Observed iPad from Engineering stored on clean towels in linen storage on 19th floor. CDI: Personal items removed.

Observed dirty glass from a room stored with linen on housekeeping cart on floor 7.

15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair. Observed multi-use glassware in bathrooms have a flat coaster. A glass cap that protects the lip of the glass is required.

15A NCAC 18A .1828( c) Clean linen and supplies shall be stored in cabinets, or on shelves in linen and supply storage rooms. Observed pillows touching sewer line and plunger stored on shelving in hallway linen closets.
