15A NCAC 18A .1826(a) Handwashing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. *** Observed no paper towels is not available at the dispenser at a hand sink in laundry room. Ensure to provide all hand washing supplies at hand sink at anytime.

General Comment: 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. *** Observed holes on wall in bathroom 412 need repair. Observed leaks and disrepair ceilings above ice machines all floors, repair is in progress.

General Comment: 15A NCAC 18A .1828 (e ) Carts used for soiled laundry shall be labeled or identified for soiled laundry use only. *** Observed soiled linens carts without labeling. Ensure to label carts especially dirty linen carts.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. *** Observed housekeeping carts, dirty items are touching clean items. Ensure to keep and separate clearly between clean supplies and dirty supplies. No cross contamination at anytime.

General Comment: 15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to avoid contamination of food and food contact surfaces, linen, and single-use articles. *** Observed clean supply boxes (toilet tissue, paper towels,..) stored on floor in storage rooms throughout. Ensure to keep/store all clean supplies off the floor at least 6 inches at anytime.

15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests. Observed housekeeping cart with no sanitizer. Housekeeping was actively cleaning room without sanitizer. Ensure sanitizer is used to sanitize all rooms in between guests.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed cracked tile on 2nd floor going into the hallway from elevator. Repair. REPEAT

15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair. Observed utensils/plates with food debris and rusted can opener in rooms 201, 209, 307, 310, and 407. Observed dirty ice bucket in room 301. Clean utensils and can opener.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed toilet seat in disrepair in rooms 108 and 307. Observed couch with stains in room 108, 201, 209, 307, 407, and 415. Observed curtain with damage in room 201. Observed black/pink buildup along caulking and on walls in shower in rooms 108, 201, 307, 310, and 407. Repair and clean.

Observed exterior of ice machine with build up on the 2nd floor. Needs to be cleaned. **15A NCAC 18A .1824(e ) Nonfood-contact surfaces of equipment shall be clean.

Observed wall damage in the housekeeping closet on the 2nd floor. Needs to be repaired. Observed water stained ceiling tile in the fitness room. Ensure leak is not ongoing and replace ceiling tile. **15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.

Observed boxes of pillows stored on the floor in the housekeeping closet on 4th and 2nd floor. Needs to be placed on shelving. **15A NCAC 18A .1828( c) Clean linen and supplies shall be stored in cabinets, or on shelves in linen and supply storage rooms.

Observed carpet burns in rooms 427 and 426. Needs to be repaired. Observed ottoman peeling and food debris in the sofa in room 427. Observed mattress with burn holes in rooms 426. Observed couch torn in room 308. Needs to be repaired/replaced. Observed mattress staining in room 209. Needs to be cleaned. **15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair.

Observed mattress cover and bed skirt torn in room 308. Items were removed. **15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest.
