REPAET 15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair.- Observed soiled ice machines in the chute and basin on 1st, 3rd, and 4th floor, detail cleaning needed and keep clean. Observed rusted microwave in need of repair or replace in 202, 103.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair.- Observed stain spot on pillow protector in 525. New caulking needed around bathtub in 401, 316, 202, 103. Peeling paint on a bathtub surface in 414. Disrepair at the top corner of ceiling and wall in bathroom 202. Repair/replace needed. Keep all furniture and fixtures clean and in good repair.

REPEAT 15A NCAC 18A .1826(a) Handwashing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. Observed no paper towels at hand sink in laundry room. 5-205.11 A handwashing sink shall be accessible and only used for handwashing. -Pf- Observed hand sink in laundry room was completely blocked by carts. CDI- Carts were removed from the front of the sink. Ensure to always give access for hand washing at all time.

REPEAT 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed damaged wall in room 108. Observed stains on ceiling tiles in fitness room, need replacement if not active leaks. Observed several (108, 106, 210, 308, 408) bath tubs looking soiled/stained, may need new caulking/or touching up for clean look and easily cleanable. Point remain the same.

15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to avoid contamination of food and food contact surfaces, linen, and single-use articles.- Observed clean supplies boxes stored directly on the floor in closet 4th and 5th floor. Keep all supplies off the floor to prevent any possible contamination.

15A NCAC 18A .1828 (e ) Carts used for soiled laundry shall be labeled or identified for soiled laundry use only. Observed 1 soiled bin in laundry room not labeled.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. Observed clean towels stored under cleaning side of broom and swifter on housekeeping cart located on 4th floor. EHS educated lodging establishment on how to separate clean from soiled on laundry carts.

15A NCAC 18A .1824(e ) Nonfood-contact surfaces of equipment shall be clean. Observed liquid build up on shelving on refrigerator located in room 420.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed staining on couch cushions in rooms 407, 420, and 519. Observed heavy dust build up inside of wall vent located beside ice machine on 3rd floor.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed stained ceiling tiles in the following area; entrance to hotel, fitness center, and over ice machine located on 3rd floor. Observed walls damage/chipping in rooms 519 under picture next to bed, 502 wall near desk, 317 wall near desk and wall joint near bed. Observed storage closets throughout facility with damage walls. Observed storage closets throughout facility with sticky build up on floor and in need of cleaning.

5-501.113 (B) Maintain tight fitting lids or doors on outside receptacles. Observed 1 dumpster door opened during time of inspection.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed cigarette burns (holes) on couch inside guest room (#418); Observed debris collected on top of kitchenette cabinets inside guest room (#310).

5-501.15 Maintain refuse, recyclables, and returnables receptacles with tight-fitting lids, doors, or covers. 5-501.16(A) An inside storage room or outside storage area and enclosure shall be of sufficient capacity to hold all refuse, recyclables, and returnables that accumulate. Observed trash debris over-flowing trash bins inside facility near maintenance closets and guest laundry areas in need of cleaning. (corrected during inspection).

15A NCAC 18A .1824(h) Sinks in guest rooms shall be sanitized before washing multi-use utensils. Observed no sanitizer on any cart in housekeeping and no sanitizer made for the day. PIC explaiend they use disinfectant on handsinks in rooms. CDI-Educated PIC on using sanitizer for handsinks in guest rooms due to the use of washing utensils in them.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed wallpaper in most hallways peeling off and damaged. PIC explained they are renovating 3-4 floors and wallpaper will be repaired after construction. Observed 3 damaged ceiling tiles on 5th floor near room 501. Observed 5th, 4th, 3rd floor elevator wall, in front of elevator damaged. Elevator tile floor damaged. Observed water damage tile in lobby area on 1st floor. Observed mold build up in shower in room 207. Observed hole in wall in room 210.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed 3 hairs on clean bed in room 525. PIC explained they do a pre-check before room is rented out. CDI-PIC changed bedding and provided new

Observed one bottle of water on the top of the housekeeping cart, bottle was closed and had no water in it. It was next to single use items. CDI-Employee moved bottle to bottom away from single use items.
