Observed stained mattress cover in room 357. Cover was removed and replaced. Spoke with staff about removing linens with set in staining. **15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest.

15A NCAC 18A .1824(i) Single-use articles may be used if discarded after each use. Single use articles may be stored and handled to prevent contamination. Observed single-use cups stored on floor of laundry chute room on 3rd floor. Needs to be stored off the floor to prevent contamination.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed pink residue on shower heads in several rooms. Observed caulking loose around tub in room 335. Observed pitting in tub in room 105. Needs cleaning/repair. REPEAT Note: 15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests. Observed no sanitizer on house keeping carts or at dispensers but large boxes of sanitizer in floor 2 storage closet. Manager stated facility has switched from their sanitizer and the labeling is now a sanitizer and disinfectant.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed stained mattress in 366, torn mattress cover in 255, stained mattress and mattress topper in 217, and stained mattress topper in 353. Needs cleaning/removal. REPEAT.

15A NCAC 18A .1828 (d) Housekeeping carts shall be kept clean and stored to protect items from contamination. Observed mops/brooms stored on housekeeping cart with dirty ends hanging over clean laundry. Needs to be stored to prevent contamination.

15A NCAC 18A .1825 (c ) A handwashing sink, located to allow use by employees handling clean and soiled linen, shall be provided. This requirement shall be effective one year after the effective date of this Rule. Observed no handwashing procedure or handsink installed in laundry room for employees handling soiled linens. Today, per Housekeeping Mgr, facility will implement washing hands in utility sink prior to handling clean linens.-0-

Observed ripped shower curtain in Rooms 354, 323, 218 and 201. Observed initials carved in desk in 323. Clean and repair these areas. -0-

Observed buildup on walls outside Rooms 352 and 353, hole in Linen storage wall at Room 318, wall damage in storage closet on 3rd floor, wall damage in original hotel side elevator. Observed water damaged ceiling on original side 3rd floor at hallway window. Per Housekeeping Mgr, this repair is scheduled. -2- REPEAT
