Observed red stained bathroom counter in room 425. Observed missing/broken kitchen cabinet in room 310.

REPEAT 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.- Observed carpet throughout the facility on every floors need vacuuming/shampooing, keep clean.

REPEAT 15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair.- Observed leftover foods inside refrigerator 431, soiled microwave in room 206. Cleaning needed and keep clean.

REPEAT 15A NCAC 18A .1826(a) Handwashing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. Observed no hand towels in dispensers or other hand drying devices in both restrooms lobby hallway. Observed no paper towels in a dispenser in staff common room. Ensure to refill the dispenser and or provide paper towels on the side at all hand sinks at all time.

Observed stained sofa bed mattress and broken screen window in room 320. Repair or replace needed. Observed dusty AC filter at the end of hallway first floor, missing both of filter in room 430. Observed new caulking needed around bathroom sink in 225. Points remain the same.

General Comment: 15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to aoid contamination of food and food contact surfaces, linen, and single-use articles.- Observed toilet tissue boxes stored directly on the storage 4th floor. Keep all clean supplies off the floor at least 6 inches. Observed dusty HVAC storage room floor, cleaning needed and keep storage floor clean.

15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair.- Observed soiled water fountains (pink/black mildew) in the fitness room, need detail cleaning and keep clean. Observed soiled microwave need cleaning in room 227, and 107.

15ANCAC 18A. 1824(k) A test kit or other device that measures the concentration in mg/L of sanitizing solutions shall be provided by the lodging establishment.- Observed the facility without test strips for sanitizer solution use. Obtain the test strips.

Ensure to provide all hand washing supplies at all hand sinks at all time.

Observed dusty AC filter at the end of hallway every floors, need cleaning and keep clean. Observed hair strains on top of toilet and microwave, dust and debris under cabinet in room 227. Observed dust and debris on sofa bed in room 415, detail cleaning needed. Points remain the same.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items.- Observed housekeeping cart stored clean items and cleaning bucket next/touch each other. Ensure to keep clean and dirty separate from each other to prevent any possible cross contamination.
