Observed shower liner missing in bathroom 1. Observed rust line from handles on wall of shower and pink build up around drain. Observed black build up and wear on the bottom to the tub in Bathroom 2. Needs cleaning/repair.

15A NCAC 18A .1610 (b) The residential care facility shall provide each resident with soap and individual hand-drying towels. These hand-drying towels will be stored separately after being used. Observed no individual hand-drying towels or paper towels in bathrooms. Needs paper towels.

15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair. Observed build up at bottom of shower curtain in boys bathroom and some rust staining in shower. Needs cleaning/replacement of shower curtain.

15A NCAC 18A. 1618 (a) all food service equipment and utensils shall be kept clean and in good repair. Observed black build up surrounding the lining of inside of dish machine unit (black build up resembles microbial growth). EHS advises facility to ensure no active growth is occurring and to clean/repair issue. REPEAT.

15A NCAC 18A .1621(d) the residential facility shall have gloves, personal protective equipment, disinfectant, individual disposable towels, and a coagulating agent onsite for employees to use and a written procedure for employees to follow when responding to a vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that employees shall take to minimize the exposure of employees, residents, guest, food, and additional surfaces to vomitus or fecal matter. New rule, EHS emailed facility required procedure plan. REPEAT.

15A NCAC 18A.1614 (a)All solid wastes shall be kept in leak proof, non-absorbent containers. Waste containers shall be kept clean and in good repair. (b) Outside waste containers shall be kept covered with tight fitting lids when not in use. Observed lid missing on 1 outside waste container. Observed debris/litter behind waste container.

15 NCAC 18A. 1617 (a) Furnishings, including furniture, curtains, draperies, and blinds, shall be kept clean and in good repair. Observed cabinet flooring and siding with black build up next to dish machine resembling microbial growth. EHS advises facility to ensure no active growth is occurring and to have issue repaired. Observed pink throw pillow in women’s room 1 with minor staining, EHS advises facility to clean pillow.

15A NCAC .18A .1617 (b) clean bed linen in good repair shall be provided for each resident and shall be changed when no longer clean. Observed minor stained on fitted sheet in men’s room 2.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed floor tile cracking in bathroom located on “side 1”. Observed touch up paint needed to baseboards and door frames sporadically throughout facility.

Observed large stain on floor near entrance in living area. Observed shredded carpet thresholds present at living room & 3 rooms. Observed damaged exterior door threshold plate.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed touch up paint needed to wall at dining room and inside room #1 closet. Observed evidence of water damage on baseboard & wall behind toilet in restroom #2. Ensure that leak is not ongoing.

Observed sofa and chair in living room starting to crack and are worn. Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). Observed one pillow case soiled on resident bed.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Clean floor in activity room where debris is present. Observed stains on floor in living area.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Walls throughout are damaged and need to be repainted. Observed debris on wall in one resident room. It was cleaned during inspection. Window sills have dust and debris. Need to be cleaned. Observed water damaged ceilings above laundry room and one resident room.

Observed loose toilet seat in the bathroom on the left side of the home. Needs to be tightened. Observed tile with build-up in the shower in the bathroom on the right side of the home. **Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed mattress torn in bedroom #1. Observed pillow torn in bedroom #4. Needs to be replaced. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed tile loose in the bathroom on the left side of the home. Needs to be repaired. **All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed baseboard tiles coming from the wall in the bathroom on the right side of the house. Observed wall with staining in bedroom #4. Observed window sill paint chipping in room #5 and wall damage in room #6. Observed water stained ceiling in the laundry room. Observed black build up on tile in bath tub in female bathroom at the end of the hall. Observed build up on shower head in female bathroom at the end of the hall. Observed torn mattress in Cindy bedroom and missing handles on dresser near window. Observed damaged cushion on chair in TT bedroom and pillow stain. Observed damaged rocking chair in Gabriel room. Observed build up on shelving in closet in male bathroom. Observed couch cushions damaged in Living Room. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed wall damage in female bathroom on the right and damaged tile behind toilet. Observed wall damage in TT bedroom. Observed wall damage in Gabriel bedroom, build up on the back side of the door and build up baseboard tile at entrance. Observed wall damage behind toilet, and wall staining in male bathroom. Observed wall damage behind table and under back window in kitchen and in Living Room. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed cabinet door off hinges in kitchen. Operator in transition of products to different cabinet. Observed residue build up on curtains (green) at end of hall left bedroom. Observed debris under covers in back right bedroom. Observed blinds collecting residue at kitchen sink and throughout facility. Keep all furniture items clean and in good repair. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed hall vent in restroom (end of hall) collecting heavy debris build up in need of cleaning. **Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Check vent connection to dryer, laundry room with excessive lint. Lint on floor, walls, light fixture, shelving, etc. Possibly a leak at kitchen sink, water pooling in cabinet.**Each resident will be provided soap and individual towels. Observed dirty blinds in bedrooms and common areas. Moldy blinds in men's bathroom. Damaged drawer of dresser in male bedroom. Soiled cushions for dining room chairs.**All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).General Comment : Where is the potty chair going to be cleaned? Cannot clean in sink or bath tub.

Observed soiled walls several areas including closets. Rubbed/abraded window sill ledges. Rubbed paint on walls. Damaged ceiling in men's bathroom. Several soiled window areas.Holes in wall where toilet paper dispenser was replaced. Ceiling stained in hallway and walk-in pantry. Damaged casing in men's bathroom.**The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed some cabinet interiors with debris where pots and pans were stored. **All containers and clean utensils shall be stored in a clean place. 15A NCAC 18A .1618 (f).

Observed caulk around the shower handrail fixtures with milde, and some rust, and the washing machine rim with collected detergent and debris.**Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed the patio umbrella in disrepair, and the patio deck need pressure washing (mildew/algae) and re-painting.**All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed the floors under neath bed with dust and debris, the floor in the activity closet with debris, and the pantry floor was dirty.**All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed the light fixture with collected dryer lint in the laundry area, and the bathroom floor vents with chipping paint.**Ventilation and lighting equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Observed no thermometer in the kitchen refrigerator. **Provide and accurate thermometer to ensure adequate food temperatures.**An indicating thermometer shall be located in each refrigerator. See rule .1620(a).

Observed a shower curtain liner with mildew in one of the bathroom.**Toilet, hand washing, bathroom, and laundry facilities shall be kept clean and in good repair. See rule .1610(a).

Observed the furniture with dust accumulation in most of the resident's rooms.**All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. See rule .1617(a).Observed some pillow cases were stained/soiled on the beds, and a mattress pad was torn/damaged on one resident bed.**Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. See rule .1617(b).

.Observed adult diapers were stored on the floor of the exterior closet, and the floor was wet.**Rooms or spaces which are provided and used for storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. See rule .1616(a).

Observed the bathroom floor grout was dirty.**All floors shall be easily cleanable and shall be kept clean and in good repair. See rule .1607.

Observed the window sills throughout the home with dust/debris accumulation.Observed one wall in a resident's room was damaged behind the rocking chair.**The walls and ceilings of all rooms and areas shall be kept clean and in good repair. See rule .1608
