Observed hot water reach 131F and 128F in bathrooms #1 and #2, respectively. **15A NCAC 18A .1611 (d) Residential care facilities shall provide water heating facilities. Hot water shall be provided at temperatures between 105F and 116F at handwashing and bathing facilities.

Observed chair in the living room in need of repair. **15A NCAC 18A .1617 (a) Furnishings, including furniture, curtains, draperies, and blinds, shall be kept in good repair. Mattresses shall be kept in good repair.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed walls beside desk and behind the bed headboard in the bedrooms collecting dust and debris. Need cleaning.

Replace toilet seat in bathroom with shower/tub fixture. Observed laminate worn and wearing off. Clean corner of wall in walk-in shower and area around tub/shower fixture where microbial growth is growing in caulking or remove old caulking and recaulk areas using a microbial resistant caulking. Repair or replace damaged leg on table in living room area. Replace damaged mattress in resident room. Clean pillow in resident room or replace pillow. Monitor old furniture and recommend replacing old furniture where varnish is worn and wearing off exposing bare wood on furniture. Replace broken window blinds in resident room. Clean linen when visibly soiled. Observed damaged pillow cover on resident pillow. Discard and repalce damaged pillow case. Clean top of trash can lid in bathroom near kitchen. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Secure panel on base of med room door. Tightly secure loose corner moulding at entrance to kitchen. Clean doors throughout facility near door handles.
