Observed touch up paint needed in bedroom 4. **15A NCAC 18A .1608 (b) Wall and ceiling attachments, such as light fixtures, fans, and vent covers, shall be kept in good repair.

Observed outside dryer vent cover in need of repair. **15A NCAC 18A .1609 (b) Ventilation equipment shall be kept clean and in good repair.

Keep walls, ceilings and attachments in good repair; Observed numerous walls scuffed within resident rooms in need of repair.

Keep facility free of pest, Observed live roach inside cabinets of kitchen.

An indicating thermometer shall be located in each refrigerator. 15A NCAC 18A .1620 (a). Observed no thermometer in refrigerator. CDI - Thermometer was placed in refrigerator.

Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed loose toilet seat in bathroom. Needs tightening.

/ All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed cabinet doors in kitchen having smudges / soil surfaces in need of general and detailed cleaning.

REPEAT The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed wall inside bedroom 2nd to last on the left side of hallway having damage in need of repair.

Observed the bottom of the shower curtain with pink buildup in the 1st restroom on the left. Clean or change. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

-Repeat: Observed chairs in the bedrooms in disrepair (vinyl torn off). Replace or repair. -Observed heavy debris and buildup under the chair cushion in the 3rd room on the right. Clean. -Observed stains on the mattress in the 3rd room on the right. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). -Observed clean comforter stored on the floor in the closet in the 2nd bedroom on the left. Store according to the rule below. Clean linen shall be stored and handled in a sanitary manner and separate from soiled linen. 15A NCAC 18A .1617 (c ).

Observed the wall around the mop sink in disrepair. Repair. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

An indicating thermometer shall be located in each refrigerator. 15A NCAC 18A .1620 (a). No thermometer inside refrigerator

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed debris on floors around beds, furniture, and laundry room equipment.

Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Observed heavy build-up in hvac vent (back right room).

Observed raw meat and eggs stored on top shelf in refrigerator. Keep all raw foods stored below ready to eat foods (bottom shelf). locate thermometer, place in warmest part of refrigerator(front). All foods, while being stored, prepared, served, and during transportation, shall be protected from contamination. 15A NCAC 18A .1620 (a). An indicating thermometer shall be located in each refrigerator. 15A NCAC 18A .1620 (a).

Observed tears/rips on couch and chair in living room. Repeat. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed pink residue and black markings in tub(female) bathroom. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed crumbs in beds in various rooms. Observed cushion damaged on couch. Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed wall damage in last bedroom and beside couch. Observed holes in ceiling in laundry room. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed rust on HVAC vents in bedrooms. Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Replace damaged shelving under two compartment sink in kitchen. Replace broken/damaged gasket on refrigeration unit in kitchen or replace refrigeration unit. Clean interior top of microwave. All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).

Replace damaged shower head on bath/tub shower fixture. Thoroughly clean shower head wand in walk-in shower. Clean hand railing and area around hand railing in bath/tub shower fixture. Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Replace or refurbish damaged cushion on couch in main living room area. Thoroughly clean stains on resident pillow or replace stained pillow. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Replace missing floor paneling in bathroom near toilet. Clean floors throughout where needed. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Thoroughly clean wall around light switch in residents rooms. Touch-up paint where walls were patched. Replace damaged door to activity closet. Repair door framing to closet in living room and bathroom door framing. Repaint metal door framing starting to rust. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed raw hamburger meat and raw egg products stored above other food products in refrigeration unit. Observed bowl stored in grit container. Store raw meat and raw egg products below and away from other food products. Use scoop or utensil with handle to dispense food products. All foods, while being stored, prepared, served, and during transportation, shall be protected from contamination. 15A NCAC 18A .1620 (a).

Repair or replace broken dish washer. Repair or replace damaged shelving in kitchen pantry closet. Observed up-right freezer unit beeping and water on the floor near unit. Have some one to check and repair freezer unit immediately! Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ). FOOD PRODUCTS IN FREEZER UNIT IS FROZEN SOLID!

.Thoroughly clean shower curtain and shower head in bathrooms. Repair bar in shower. Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Repair or replace couch cushion in common living room area. Repair damaged shelving in linen/storage closet. Replace broken/damaged mattress on resident bed. Replace torn/damaged linen on resident bed. Wipe/clean base of bed framing in resident room. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Thoroughly clean floor in hallway area along edge of wall and in med room closet. Repair (1)floor panel in bathroom. Thoroughly clean carpet in resident room or replace carpet. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Repair wall beside base of can wash/ behind washer machine. Clean/repaint walls throughout in common living room area, kitchen, bathroom and resident room. Clean and repaint back door. Caulk/seal top of tile shower wall and ceiling. Repaint bathroom door framing. Highly recommend sealing/staining bare wood bathroom doors. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. General Comment: #1. Provide back flow prevention device on shower wand in bathroom with bath tub/shower combination. Replace trash can with hole present. Observed large furniture beside storage building. House manager informed me that she had called the City of Charlotte and had already scheduled a large item trash pick up. Repair light fixture in kitchen area or replace blown light bulbs. Lakethia Mungin present at time of inspection.

Observed shower curtain and area around hand railing in bath tub soiled in 1st bathroom on left side of hallway. Observed base of shower wall, shower head and shower chair and shower chair straps soiled in 2nd bathroom on left side of hallway. Thoroughly clean or replace shower curtain. Clean wall in shower, area around hand rail in shower/tub, shower wall and shower chair.Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Repair drawers on resident furniture in rooms. Tighten loose pulls on drawer. Replace covering on resident bed---> last room on left side of hall. Clean underside edge of dining room table. Observed old food debris around underside edge of table. Replace chair in living room with worn arm rest. Clean window blinds throughout where needed. Wipe/dust top of fixtures/picture in resident room. Observed light dust on top of furniture/fixture. Clean top of bed base where mattress rest. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed carpet in residents room soiled. Thoroughly shampoo carpet in residents room! Tightly secure loose floor panel in 1st bathroom on left.All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Touch-up paint wall in living room area, area around door framing leading into kitchen area, area around door framing leading into dining room area. Patch hole in wall in 1st bathroom on left side of hallway and repaint. Patch area on wall behind door in resident room and repaint. Repair base of wall near base of shower in 2nd bathroom on left side of hallway and repaint. Observed sheet rock tape popping loose in 2nd bathroom on left side of hallway. Tightly secure loose sheet rock tape and repaint. Highly recommend applying wood sealant/varnish on wooden bathroom door. Observed varnish/sealant worn off of wooden bathroom doors. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. General Comment: Wipe/dust ceiling return air ventilation near entrance to hallway. Ms. Earther Burris present at time of inspection.

Observed underside of handrail in shower soiled. Thoroughly clean handrail in shower.Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed worn/torn chair in living room area. Replace damaged chair in living room. Observed two resident beds missing flat sheet. Fitted sheets were on bed and a bed spread. Provide residents a flat sheet also.All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Observed storage directly on the floor in living room area. Store items on shelving or in a plastic container to allow easy cleaning of the floor.Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

Patch small holes in wall throughout where needed. Clean and repaint walls throughout where needed. Repair lower section of wall near entrance to walk-in shower.The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.General Comment: Recommend storing opened packages of dry food products in sealable container, labeled and kept clean(flour, corn meal). Replace blown light bulb in light fixture in bathroom. Clean out gutters. Mr. Mike Powers present at time of inspection

Observed cookies and crackers improperly stored in kitchen.** Food shall be properly stored to prevent contamination.

Shower chair with black and pink residue around joints. No soap availble in 2nd restroom.** All bathroom fixtures and facilities shall be kept clean and in good repair. Soap and paper towels shall be provided.

Observed a few pillows, mattress and sheet with stains. Chair with large hole in seat cushion in residents room. Some linen stored on floor in hallway linen closet. **All furniture, mattresses, pillows, curtains and other furnishings shall be kept clean and in good repair. Clean linen shall be stored and handled in a sanitary manner.

Observed carpet stained in most residents' rooms.** Keep floors clean and in good repair.

Observed some wall damage throughout facility.** Keep walls and ceilings clean and in good repair.

Most vents accumulating dust.** Ventilation equipment shall be kept clean and in good repair.
