15A NCAC 18A .1609 (b) Ventilation equipment shall be kept clean and in good repair. Observed minor dust accumulation to wall and ceiling vents throughout facility.

15A NCAC 18A .1607 All floors shall be kept clean and in good repair.- Observed flooring underneath beds in rooms need detail cleaning and keep clean. Observed damaged/missing piece of floor tile in the kitchen. Repair needed. Keep flooring clean and in good repair.

15A NCAC 18A .1609 (b) Ventilation equipment shall be kept clean and in good repair.- Observed AC vents covered with dust. Detail cleaning needed. Filter may need to be replaced. Keep clean.

15A NCAC 18A .1610 (b) The residential care facility shall provide each resident with soap and individual hand-drying towels. These hand-drying towels will be stored separately after being used.- Observed no paper towels available at sink in the kitchen and bathroom sink in A hallway. Ensure to provide all hand washing supplies at all hand sink at all time.

General Comment: 15A NCAC 18A .1620 (d) All food shall be stored as required by Parts 3-302.11, 3-302.12, 3-305.11, and 3-305.12 of the Food Code, incorporated by reference at 15A NCAC 18A .2650 as amended by 15A NCAC 18A .2653.- Observed a bag of potato and BBQ sauce stored directly on the floor in pantry. Observed boxes of medication cups and diapers (clean supplies) stored directly on the floor in the garage. Ensure to keep all food items and clean supplies above the floor at least 6 inches at all time.

General Comment: 15A NCAC 18A .1621 (d) The residential care facility shall have personal protective equipment on-site for employees to use and a written procedure for employees to follow when responding to vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that should be taken to minimize the exposure to vomitus or fecal matter.-New rule- Observed the facility without vomiting and diarrhea written plan. EHS provided the poster and the facility will gather all supplies needed and have available for such an event.

Observed touch up paint needed in bedrooms 2 and 6. Observed baseboard in need fo repair in bedroom 5. **15A NCAC 18A .1608 (b) Wall (doors, windows and window trim) and ceilings and ceiling attachments (light fixtures, fans, and vent covers) shall be kept in good repair.

Observed torn blinds in bedroom 1. **15A NCAC 18A .1617 (a) Furnishings, including furniture, curtains, draperies, and blinds, shall be kept in good repair. Mattresses shall be kept in good repair.

Observed shower curtain with build up in bathroom 2. **Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed blinds torn in bedroom 1. Observed dresser knobs missing in bedroom 4. Observed sofa peeling in the living room. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed ceiling peeling in bedroom 2. Observed wall peeling in bathroom 2. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed wall damage behind the chair in the living room. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed eggs stored above ready to eat foods in the refrigerator. Spoke with staff about proper storage. **All foods, while being stored, prepared, served, and during transportation, shall be protected from contamination. 15A NCAC 18A .1620 (a).

Observed grout in the tub in the large bathroom in need of cleaning. **Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed torn mattress cover in bedroom 3. Needs to be replaced. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed touch up paint needed throughout the facility. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed box spring in resident room having debris collected on surfaces in need of cleaning. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed flooring underneath residents beds in need of cleaning; Observed flooring in resident rooms in need of mopping, sticky food debris or dried liquids collected on surface.

Effective measures shall be taken to keep insects, rodents, and other vermin out of the residential care facility and to prevent their breeding, harborage, or presence on the premises. 15A NCAC 18A .1615 (a). Observed numerous live ants inside resident bedroom (floor) in need of treatment,

Observed debris build-up in cabinets and drawers of kitchen. **Acceptable storage facilities include residential kitchen cabinets, which should be kept clean and free of vermin. 15A NCAC 18A .1618 (i).

Observed caulking needed in shower holes where railings used to be, cleaning needed on shower floor and in tub, shower head handle missing, and shower curtain with build-up in bathroom #1. Observed dried urine on toilet and toilet seat in bathroom #2. **Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed dresser with dust build-up and sheet with holes in bedroom #1. Observed stained pillows in several bedrooms. Observed debris collecting under mattress in bedroom #4. Observed mattress with smudges in bedroom #3. Observed red couches with stains and debris collecting in small living space. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). **Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Observed touch-up paint needed on doors and door frames throughout. Observed baseboards with damage. Observed painting needed on walls and ceilings in bedroom #4. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Each resident will be provided soap and individual towels. 15A NCAC 18A .1610 (b). Observed no soap in bathrooms. Shower curtains need laundering. General Comment #1: Shower wands in bathrooms need backflow preventers or supply documentation that the shower wands have built-in backflow prevention. Note: Observed multiple razors in a drawer in bathroom, staff didn't know whose they were. This is not a good practice to prevent blood-borne illnesses.

Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).Observed Storage areas and bedroom closets need arranging and organizing. Clothes strewn in several closets.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed walls and ceilings damaged throughout and need cleaning.

All openings to the outer air shall be effectively protected against the entrance of flying insects by screens, closed doors, closed windows, or other effective means. 15A NCAC 18A .1615 (a). Observed top of window missing in 1 bedroom. Needs repair before the rest of it falls out.

Observed baseboard with discoloration apparent rotting in right side bathroom. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.Mr. Travis was present during inspection.

Observed comforters, linen, pillow cases stained, torn, throughout facility. Observed multiple rips in liners of furniture in left living room upon entrance. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed left hallway flooring in disrepair from previous flooring pulled up and small gaps in flooring in kitchen.
