15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair. Observed build up on shower head, crack in bottom of tub, and rust stains on wall of shower in bathroom 1.

15A NCAC 18A .1611 (d) Residential care facilities shall provide water heating facilities. Hot water shall be provided at temperatures between 105F and 116F at handwashing and bathing facilities. Observed hot water at 95-97F at bathroom handsinks during the inspection. Needs to be 105-116F.

15A NCAC 18A .1607 All floors shall be kept in good repair. Observed loose tile in bathroom 1. Observed cracked floor tile in kitchen. Needs repair.

15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair. Observed build up on shower head and in toilet in bathroom 1. Toilet and shower head were cleaned by staff during inspection

15A NCAC 18A .1617 (a) Furnishings, including furniture, curtains, draperies, and blinds, shall be kept in good repair. Mattresses shall be kept in good repair. Observed torn and stained chair in bedroom 6. Needs cleaning/repair.

General Comment: 15A NCAC 18A .1621 (c) Residential care facility employees shall comply with the requirements for exclusion from work and restriction due to communicable disease or illness required for food employees as set forth in Parts 2-201.12 and 2-201.13 of the Food Code incorporated by reference at 15A NCAC 18A .2650 as amended by 15A NCAC 18A .2652. Observed facility does not have Employee health policy. EHS emailed a copy to staff.

General Comment: 15A NCAC 18A .1621 (d) The residential care facility shall have personal protective equipment on-site for employees to use and a written procedure for employees to follow when responding to vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that should be taken to minimize the exposure to vomitus or fecal matter. Observed facility does not have vomit and diarrhea cleaning procedure. EHS provided a copy via email.

Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b). Observed automatic paper towel dispenser not operating in bathroom 2. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed shower curtains liners with brown/black staining at the bottom in both bathrooms.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed a couple knobs missing to dresser in room 3.

Observed blinds torn in the dining room. Needs to be replaced. **Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed loose toilet seat located in C hallway restroom. -Observed the inside of the microwave rusting. Replace. -Observed old food debris in the oven drawer. Clean. Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ).

-Observed debris in the made up bed in the bedroom at the end of the hallway. Clean. Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). -General Comment: Observed recliner in bedroom 6 with stains and rips. In the process of replacing. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed toilet with dark staining in the interior in large bathroom. Needs to be cleaned thoroughly. Observed handwashing sink with rust around the edge. Recaulking is needed. **Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed torn linen in room 2. Needs to be replaced. Observed torn mattress covers in rooms 2 and 4. Needs to be replaced. **Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). Observed sunken in mattress in room 4. Staff informed me that residence bounces a lot and a replacement mattress on order. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed rusting around hand sinks in restrooms also in shower of restroom near front door. Observed hair on showerhead in biggest restroom. Observed stain down front of toilet in biggest restroom. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed torn chair in bedroom. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed flies near back door in kitchen. Effective measures shall be taken to keep insects, rodents, and other vermin out of the residential care facility and to prevent their breeding, harborage, or presence on the premises. 15A NCAC 18A .1615 (a).

Observed rusting in tub and around sink in restrooms. Clean down the front bottom of toilet. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed wall damage in restroom. Repair. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed kitchen cabinets and drawers need wiping out throughout. Debris sticking to contact paper glue. Shelving in pantry and large bins need cleaning of food residue and stickiness.**Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ).General Comment #3: Observed some severely dented cans from food bank. Dispose of very dented, leaking or questionable cans.

Observed torn mattress cover, torn comforter, soiled pillow and soiled sham. Dusty lamp shade in one room. Couple of dusty tv's in bedrooms. Dining room table with chipping surface. Stained dining room chairs.**All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed stained carpeting.**All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed back storm door in disrepair at bottom, chipped and chalking. 2 rooms with lighting not working properly.**Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).**All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a).

GC: Observed no thermometer in refrigerator, CDI. Observed rice and flour not in sealed baggie or covered container.**An indicating thermometer shall be located in each refrigerator. 15A NCAC 18A .1620 (a).**All foods, while being stored, prepared, served, and during transportation, shall be protected from contamination. 15A NCAC 18A .1620 (a).

Observed rusted sink drain and medicine cabinet in shower bathroom.**Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

GC: Observed torn sheet on bed in room 3. Observed sagging mattress and soiled chair in bedroom off kitchen. Old mattress in back yard (propped against house). Ripped vinyl chair, rec room. Torn pillow, room 6.**Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).**All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed rusty door jamb in bathroom with tub and room 5. Wall/paint damage in room 6. Rusted/scuffed baseboard, room 5.**The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Repeat Non-Compliance: Observed the sink drain with rust and the shower wall and grout with rust is a resident restroom.**Toilet; hand washing; bathing; and laundry facilities shall be kept clean and in good repair. See rule .1610 (a).

Observed the clothes wasing machine with a damaged gasket. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed the door frame in the bathroom was damaged, and the hallway wall had damage. Finish all repaired walls with paint.GENERAL COMMENTS: John Williams was present during the inspection.#3 - Observed a tray of raw prok on a shelf above ready-to-eat fruits and veggies. **Containers of food shall be stored above the floor, on clean racks, shelves, or other clean surfaces, in such a manner as to be protected from splash (or drip) and other contamination. 15A NCAC 18A .1620 (d).#13 - Observed the ceiling fan was not secure to the ceiling in a resident room. **Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).
