Observed underside of shower chair in need of cleaning. **15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair.

15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair. Observed rusting and peeling legs on the shower chair. Needs repair.

General Comment: 15A NCAC 18A .1611 (d) Residential care facilities shall provide water heating facilities. Hot water shall be provided at temperatures between 105F and 116F at handwashing and bathing facilities. Observed hot water at 120F at bathroom handsink. Needs to be 105-116F.

General Comment: 15A NCAC 18A .1621 (c) Residential care facility employees shall comply with the requirements for exclusion from work and restriction due to communicable disease or illness required for food employees as set forth in Parts 2-201.12 and 2-201.13 of the Food Code incorporated by reference at 15A NCAC 18A .2650 as amended by 15A NCAC 18A .2652. Observed facility does not have Employee Health Policy. EHS emailed a copy.

General Comment: 15A NCAC 18A .1621 (d) The residential care facility shall have personal protective equipment on-site for employees to use and a written procedure for employees to follow when responding to vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that should be taken to minimize the exposure to vomitus or fecal matter. Observed facility does not have vomit and diarrhea cleaning procedure. Needs vomit and diarrhea cleaning procedure. EHS emailed a copy.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed debris on mattress in bedroom #5. Observed shelf rusting in bathroom #1. Needs cleaning. REPEAT.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed small holes in walls of bedroom #3. Observed scuffs on walls and build up on door in bedroom #2. Observed damage to wall from door knob in bathroom #2. Needs repair.

Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Observed dusty vents and fans. Need cleaning and keep clean. REPEAT.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed damaged leather bench on the entry hall way. Repair or replace the bench.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed dust collected underneath the bed in one room, and dust all around the corners of rooms. Need general cleaning and keep floor clean.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed some black mildew on the wall and ceiling in bathroom #1 and 2. Need cleaning and remove mildew. Observed damaged wall in bathroom#2, need repair and touch up paint.

Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Observed dusty bathroom vents. Need cleaning and keep clean.

Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a). Observed bedroom #2 #5 having large amounts of storage overflowing out of closets onto floor in need of proper storing to facilitate cleaning.

Effective measures shall be taken to keep insects, rodents, and other vermin out of the residential care facility and to prevent their breeding, harborage, or presence on the premises. 15A NCAC 18A .1615 (a). Observed numerous fruit flies throughout kitchen in need of removal; (trash can is not covered and fruit flies seemed to be collecting in that area, recommend getting lid for trash can).

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed wall behind toilet in group bathroom damaged in need of repair. (recommendation to install water proof wall covering to prevent disinfectant damaging paint).

Observed mattress heavily stained in far bedroom on back side of house. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed stained carpet in far bedroom on front side of house. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed counter in disrepair near toilet in bathroom 1. Observed build up in crevices on counter in bathroom 1. Observed build up/residue on shower tiles in bathroom 2. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed black residue in bathtub in small restroom. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Repair or replace damaged vanity counter top in bathroom containing the shower/tub fixture. Observed vanity counter top damaged near shower/tub fixture. Replace damaged toilet seat in bathroom with tub/shower fixture. Clean shower curtain in bathroom with walk-in shower. Clean underside of shower chair. Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Clean resident pillow covering with stains present. Replace resident damaged pillow covering. Dust shelving in resident room and high fixture. Tighten handles on furniture in resident room. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Tightly secure loose transitional floor molding near dining room area and living room area. Highly recommend installing transitional floor molding at entrance to resident room to prevent carpet from being kicked up and fraying along edge. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Touch-up paint throughout home(living room, dining room and resident room). Repaint wall under window in room #2 and monitor wall under window to make sure area around window is not leaking. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. General Comment: #4. The two pitted bowls discarded at time of inspection. #13. Clean/dust ventilation in bathroom with shower/tub fixture.

Tighten loose toilet seat in bathroom. Observed base corner of shower, shower curtain and underside of shower chair soiled in bathroom in hallway area. Thoroughly clean base of shower, shower curtain and underside of shower chair. Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed bags of stuff stored on the floor in outside storage closet. Store items in rubber bin. Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

Tightly secure loose transitional floor molding at entrance to laundry room. Install transitional floor molding at entrance to resident room where flooring in hallway area meets the carpet in residents room. Observed carpet being kicked up at entrance to resident room and carpet is starting to fray. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Louise Dennis present at time of inspection. Wipe/clean wall near light switch in hallway and residents room.

Observed base of utensil/equipment drawer under oven/stove soiled. Thoroughly clean base of drawer under oven/stove. Utensils shall be handled in such a manner as to prevent contamination. 15A NCAC 18A .1618 (f).General Comment: Clean ice from top of door of up-right freezer unit. Monitor freezer unit. Ice should not be forming around top of door. Gasket on refrigeration unit is in good repair. Make sure items in freezer unit are properly stored so they not prevent the freezer door from properly closing.

Observed stains on residents pillows. Thoroughly clean or replace pillows.All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Monitor and replace cracked floor tiles at front entrance. Tightly secure floor transitional moulding at laundry room area. Recommend installing transitional floor moulding at entrance to residents room. Observed edge of carpet starting to fray at entrance to resident rooms. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Replace blown light bulb in light fixture above dining room table and in bathroom at end of hallway on the right. Clean ventilation in bathroom at end of hallway on the right.All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a). Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Observed opened packages of grits, crackers, noodles and mash potatoes stored in pantry area. Store opened packages of dry food products in sealable container, labeled and kept clean. **Corrected at time of inspection**All foods, while being stored, prepared, served, and during transportation, shall be protected from contamination. 15A NCAC 18A .1620 (a).

Observed base of shower curtain soiled in bathroom located on left side of hallway. Thoroughly clean or replace soiled shower curtain.Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).General Comment: Observed dust on bathroom fan cover located on left side at end of hallway.
