15A NCAC 18A .1611 (d) Residential care facilities shall provide water heating facilities. Hot water shall be provided at temperatures between 105F and 116F at handwashing and bathing facilities. Observed hot water at 119F at bathroom handsink.

15A NCAC 18A .1621 (d) The residential care facility shall have personal protective equipment on-site for employees to use and a written procedure for employees to follow when responding to vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that should be taken to minimize the exposure to vomitus or fecal matter. Facility did not have copy of required procedure plan. EHS left facility copy of plan. -General comment.

Observed observed dresser handles missing in room 3. Observed dresser drawers off track in room 4. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed dusty floor underneath the beds and dresser in all bedrooms. Need general cleaning and keep clean.

Observed a gallon of milk sitting on the counter top thawing. Potentially hazardous frozen food shall be thawed at refrigerator temperatures of 45° F. or below; or quick-thawed as part of the cooking process; or by a method approved by the sanitarian. 15A NCAC 18A .1620 (a).

-Observed stained sheets and/or pillows in bedrooms 2 & 3. Clean. -Observed the dressers in bedroom 6 scuffed showing the raw wood. Repaint or seal. -Observed a knob missing on the dresser in bedroom 1 and the drawers are off track. Repair. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). *12. NO POINTS DEDUCTED: Observed the paint peeling over the handsink in the 1st restroom. Repaint. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed both shower curtain bars in the bathrooms were rusty, and the shower hand rail beginning to rust in the shower. General Comments: #3. Observed the refrigerator thermometer's red indicator was separated and inaccurate; Replace thermomoter. #7. Observed the floor surface in the dining room was discolored / worn. NOTE: Jeannette Dortch was present during the inspection.
