Obserevd grout in the shower in need of cleaning in bathroom 1 and bathroom 2. **15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair.

Observed dresser drawers off track in room 2. **15A NCAC 18A .1617 (a) Furnishings, including furniture, curtains, draperies, and blinds, shall be kept in good repair. Mattresses shall be kept in good repair.

Observed clean linen stored on the floor in linen closet. **15A NCAC 18A .1617 (c) Clean linen shall be stored and handled in a manner to protect from contamination and separate from linen that is not clean.

1611 (c) A residential care facility's water supply plumbing shall not include cross-connections as set out in 15A NCAC 18C .0102(c)(8). Observed no backflow preventers at shower wands in bathrooms.

.1617(a) Furnishings, including furniture, curtains, draperies, and blinds, shall be kept clean and in good repair. Mattresses shall be kept clean, dry, and in good repair. Observed stained mattress with torn plastic protection. Mattress compressed and needs replacing.

.1617(b) Clean bed linen in good repair shall be provided for each resident and shall be changed when no longer clean. Observed one soiled pillowcase.

(c) Clean linen shall be stored and handled in a manner to protect from contamination and separate from linen that is not clean. Observed clean linens on floors in linens closets

.1620(d) All food shall be stored as required by Parts 3-302.11, 3-302.12, 3-305.11, and 3-305.12 of the Food Code, incorporated by reference at 15A NCAC 18A .2650 as amended by 15A NCAC 18A .2653. Observed several opened containers of food not protected from contamination. Keep in lidded containers or baggies.

.1621(c) Residential care facility employees shall comply with the requirements for exclusion from work and restriction due to communicable disease or illness required for food employees as set forth in Parts 2-201.12 and 2-201.13 of the Food Code incorporated by reference at 15A NCAC 18A .2650 as amended by 15A NCAC 18A .2652. New Rule-discussion and handouts

(d) The residential care facility shall have gloves, personal protective equipment, disinfectant, individual disposable towels, and a coagulating agent on-site for employees to use and a written procedure for employees to follow when responding to vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that employees shall take to minimize the exposure of employees, residents, guests, food, and additional surfaces to vomitus or fecal matter. All foods, while being stored, prepared, served, and during transportation, shall be protected from contamination. 15A NCAC 18A .1620 (a). Observed raw shell eggs over Ready to Eat food in refrigerator. An indicating thermometer shall be located in each refrigerator. 15A NCAC 18A .1620 (a). Observed battery dead in refrigerator thermometer.

Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed sink leaking in bathroom. Observed build up on shower head and rusting around shower head wall cap and floor drain. Observed build up around edge of tub. Needs cleaning.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed lower kitchen cabinets in need of cleaning. Observed tears in chairs in bedroom 2 and 4. Needs cleaning/repair.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed scuffs on walls in bedrooms 1 and 3 and in common dining area. REPEAT.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed flooring underneath beds collecting debris in multiple resident rooms in need of general cleaning.

REPEAT The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed walls throughout resident rooms scuffed / stained / damaged in need of touch-up painting.

Ventilation / lighting equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Observed light fixture in group bathroom rusty in need of resurfacing; Observed slight build up on exhaust vents inside bathrooms.

Clean linen shall be stored and handled in a sanitary manner and separate from soiled linen. 15A NCAC 18A .1617 (c ). Observed numerous sheets and linen stored directly on floor of linen closet; store above floor to facilitate cleaning and to prevent potential contamination of clean linen while in storage.

REPEAT The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed areas of walls throughout in need of touch-up painting (ex; wall and light switch area where resident hits wall repeatedly).

An indicating thermometer shall be located in each refrigerator. 15A NCAC 18A .1620 (a). There was not a thermometer located in the refrigerator in kitchen area. CDI.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Repaint walls in living and restroom. Caulk and repaint walls in three residents rooms near kitchen on edges and in corners. Damaged noticed on walls where furniture is located.

Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Clean cobwebs from window sill in resident room in front of facility. Clean dust and mold from window located in front living room area.

Observed stains in holes on shower chair in large bathroom. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed damage to top cushion on single chair. Observed damage to blinds and dresser near large restroom. Observed stains on pillowcases in various rooms. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Observed stains in microwave and on stove. All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).

Observed stains on toilet (lid) in large bathroom. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed hole in wall behind door in restroom on men's side. Observed hole in wall inside closet in bedroom across from large bathroom. Observed stains on wall (near toilet) and on ceiling in large bathroom. Observed damage light switch in bedroom across from large bathroom. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Thoroughly clean base drawer under oven where clean utensils are stored. Clean utensil storage area in cabinet areas. All containers and clean utensils shall be stored in a clean place. 15A NCAC 18A .1618 (f).

Thoroughly clean hand railing in walk-in shower. Clean shower chair in shower. Repair pick plate on base of cabinet in bathroom with walk-in shower. Replace missing floor tiles around floor drain in walk-in shower. Thoroughly clean wall tile and grout in walk-in shower. Repair chipped spot on bathtub. Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Replace torn/damaged chairs in common sitting area. Clean window blinds in resident room. Observed stain on resident bed spread, sheet and pillow. Change linen on resident bed and pillow when visibly soiled. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Repair or replace flooring throughout in hallway area and common sitting area. Tightly secure loose transitional molding in common sitting area. Thoroughly clean or replace carpet throughout house. Replace quarter round shoe molding along edge of flooring in hallway area. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Repair walls throughout where needed. Thoroughly clean walls in hallway, bathroom and residents room. Observed door framing to bathrooms rusted and paint coming off. Repair and repaint metal door framing to bathrooms. Monitor water stain on ceiling in resident room and repair any and all roof leaks, then repaint ceiling. Thoroughly clean ceiling in bathroom. Observed condensation and microbial growth present on ceiling in bathroom where walk-in shower is located. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Repair ventilation in bathroom containing walk-in shower. Observed ventilation over toilet area working, but the ventilation near walk-in shower is not working. Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Observed base of utensil/equipment storage areas(shelving, base of drawer) soiled in kitchen area. Thoroughly clean all utensil/equipment storage areas. All containers and clean utensils shall be stored in a clean place. 15A NCAC 18A .1618 (f). General Comment: Repair extrior of kitchen cabinet from water leak.

Thoroughly clean toilets in all three bathrooms. Observed all three toilets soiled. Thoroughly clean wall in showers. Repair/replace missing tile from around floor drain in shower. Replace missing shower wand in bathroom. Make sure shower wand has a back flow prevention device attached to fixture. Observed soap and paper towels missing from several hand washing sinks throughout facility! PROVIDE SOAP AND PAPER TOWELS AT ALL HAND WASHING SINKS AT ALL TIMES! Monitor and repair base of sink in bathroom/shower near office area. Observed area around drain stopper in base of sink damaged. Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b). Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed damaged covering on red leather couch in common sitting/living room area. Repair or replace damaged couch. Observed broken/damaged window blind in resident room. Observed window blinds soiled in resident room. Replace broken window blind and thorughly clean window blinds. Replace damaged night stand in resident rooms. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed packakges of briefs stored on floor in supply closet. Remove briefs from floor in storage room.. Store extra briefs in plastic bin in storage room or higher shelving. Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

Thoroughly clean floors and base boards throughout facility. Repair damaged flooring throughout facility(hallway, kitchen, bathrooms and residents room). Install quarter round shoe moulding where shoe moulding is missing. REplace missing transitional floor moulding at entrance to bathrooms and residents rooms. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Thoroughly clean and repair ceiling in bathroom with just tile shower near common living room/sitting room. Patch and repair walls throughout in resident room, common sitting room, hallway and bathrooms. Clean walls throughout and repaint. Repair any and all roof leaks, then repair ceiling in hallway area and residents rooms.Replace damaged electrical and light switch covers in hallway, bathrooms and residents rooms. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Repair ventilation in bathroom with tile shower. Ventilation is not properly working due to the amount of condensation and microbial growth on ceiling in bathroom and light fixtures rusting. Replace missing window screen in resident room and try to keep window closed.Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Ms. Chelsea Lovett present at time of inspection.

Observed opened packages of dry food products throughout kitchen cabinets. Store opened packages of dry food products in sealable containers, labeled and kept clean(cookies, flour, sugar, etc.) Observed carton of raw eggs stored on top of vegetable products in refrigeration unit. Store raw meat and raw egg products below and away from other food products.All foods, while being stored, prepared, served, and during transportation, shall be protected from contamination. 15A NCAC 18A .1620 (a).

Observed sealant worn from kitchen cabinet/ doors. Reseal wooden cabinet doors. Repair hinge on cabinet doors. Thoroughly clean base of freezer unit and do not use base of freezer as a foot stool. Observed foot prints in base of freezer unit. Thoroughly clean base of utensil drawer under oven.All containers and clean utensils shall be stored in a clean place. 15A NCAC 18A .1618 (f).All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).

Thoroughly clean shower curtain(cloth and plastic liner) or replace shower curtains. Observed microbial growth on shower curtains. Replace missing floor tiles around floor drain in shower. Observed bolts missing used to secure toilet to the floor in bathroom. Repair toilet in bathroom so toilet will not turn. Replace missing toilet paper holder in bathroom.Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed linen stored on the floor in closet located in the hallway. NO LINEN STORAGE ON THE FLOOR!Clean linen shall be stored and handled in a sanitary manner and separate from soiled linen. 15A NCAC 18A .1617 (c ).

Repair damaged flooring in residents room. Replace missing toe moulding throughout in hallway. Clean carpet in television area.All floors shall be easily cleansable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed base of shelving where baking pans are stored soiled. Thoroughly clean base of utensil shelving. Observed damaged drawers in kitchen area. Repair or replace damaged drawers. Clean exterior of cabinet near dishwasher and keep clean.All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ).

Observed some stains on bed linens in residents room. Change and clean linen on residents bed when visibly soiled. Observed damaged hospital bed mattress on resident bed and urine smell coming from damaged mattress. REPLACE DAMAGED MATTRESS IMMEDIATELY! Observed damaged mattress covers on resident bed. Either replace or remove mattress covers from residents bed.All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Observed damaged flooring throughout facility in shower, entrance to bathroom, residents room and at entrance to resident room. Repair or replace damaged flooring throughout entire facility. Provide a transitional floor moulding where carpet meet wooden floor at entrance to residents room. Tightly secure loose base moulding and replace missing base moulding in resident closet.All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed wall damaged throughout in resident room and bathroom. Properly repair walls throughout and repaint. Replace missing door stopper behind bedroom doors. Observed nails backing out of sheet rock on wall and ceiling in bathroom. Nail back in nails and repaint. Monitor water stains on ceiling throughout and repair any roof leaks if needed.The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed light fixtures not operating in bathroom over shower and handsink vanity. Also observed light fixture not operating in hallway area. Replace blown light bulbs or repair light fixtures. Observed dead bugs present in enclosed light fixtures. Clean dead bugs from within light shield.All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a).

Observed roll out trash can missing lid. Replace trash can with missing lid.All solid wastes shall be kept in durable, rust-resistant, nonabsorbent, watertight, rodent-proof standard waste containers which shall be kept covered when filled or stored or not in continuous use. 15A NCAC 18A .1614 (a).

Observed damaged comforter with tears. Pillow with pink stain. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. See Rule .1617

Observed baseboards damaged throughout facility. Paint peeling in a few areas. **Keep all walls and ceilings clean and in good repair. See Rule .1608
