15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair.- Observed mildew and build up under at the bottom of shower chairs in bathroom. Detail cleaning needed. Keep clean.

15A NCAC 18A .1607 All floors shall be kept clean and in good repair.- Observed flooring under beds in bedrooms and laundry room behind the machines need detail cleaning and keep clean.

15A NCAC 18A .1609 (b) Ventilation equipment shall be kept clean and in good repair.- Observed AC ceiling vent need cleaning and keep clean.

15A NCAC 18A .1621 (d) The residential care facility shall have gloves, personal protective equipment, disinfectant, individual disposable towels, and a coagulating agent on-site for employees to use and a written procedure for employees to follow when responding to vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that employees shall take to minimize the exposure of employees, residents, guests, food, and additional surfaces to vomitus or fecal matter. - New rule- Observed the facility without vomit and diarrhea written plan. EHS provided the written plan at the time of inspection and the facility will gather the supplies and have available for such an event.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed flooring underneath the beds need detail cleaning, and keep clean.

Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Observed dusts covered the ceiling vents though out the facility need cleaning.

Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed minor black build up inside the lower shower tiles in restroom 2

Observed interior of microwave in need of cleaning. **Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ).

Observed surface of couches peeling in the living room. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed wall damage in bedroom 6. Needs to be repaired. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed vent covers with build up in bedroom 1, 2 and 3 and in the smaller bedroom. Needs to be cleaned. **Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Observed faucet handle off in the hallway bathroom. Needs to be repaired. **Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed handles missing on the dresser in bedroom 3. Handles needs to be replaced. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed sheet torn in bedroom 1. Needs to be replaced. Observed top cover torn in bedroom 4. Needs to be replaced. **Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Observed wall stains in bedroom 1. Needs to be cleaned. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed vent cover in need of cleaning. **Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Observed cleaning needed on cabinet handles. Observed debris build-up in several kitchen cabinets. Observed cleaning needed on refrigerator and freezer shelving. Observed debris in utensil drawer organizer. **Acceptable storage facilities include residential kitchen cabinets, which should be kept clean and free of vermin. 15A NCAC 18A .1618 (i). **All containers and clean utensils shall be stored in a clean place. 15A NCAC 18A .1618 (f). **Utensils shall be handled in such a manner as to prevent contamination. 15A NCAC 18A .1618 (f).

REPEAT Observed caulking needed around the backsplashes of hand sinks and around the base of hand sinks where basin meets counter top. Observed water damage under hand sink in bathroom #1. Observed black build-up in the grout of the shower in bathroom #2. Observed chipping tile around shower drain in bathroom #2. REPEAT Observed several stained pillows in bedrooms. Observed stained sheets with debris in second to the last room on the left. Observed blinds with dust build-up throughout rooms. Observed loose dresser knobs and missing knobs on furniture in several bedrooms. Observed cleaning needed in the chemical closet. Observed build-up on wire rack shelving in kitchen supply closet. **Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

REPEAT Observed build-up and cleaning needed on the floor behind toilets. Observed floor peeling up behind the bed in second to the last bedroom on the left. Observed stained carpets throughout. **All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

REPEAT Observed cleaning on tile wall around toilet area in bathroom #1. Observed walls with scuffs, hand prints, and build-up throughout facility. Observed rusting and chipped wall corner in bathroom #1. Observed brown debris on door jam of last room on left side of hallway. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed heavy dust build-up and rust on ventilation equipment throughout facility. Observed light bulbs out in the kitchen, laundry room, and in bathroom #2. **Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). **All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a).

Observed several plastic bowls with rough/worn surfaces and stained; no longer smooth and easily cleanable. **All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).

Observed no soap at some hand washing sinks. **Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b). Observed shower grout with black residue and some shower tiles were missing, the sink backs plash laminate was separating, and the shower curtain liner had pink and brown residue in bathroom 2. Observed the counter top with damaged caulk and the backs plash laminate was separating in bathroom 1. **Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed that sheets in bedroom 1 smelled of urine, and the body pillow with stains in bedroom 2. some window blinds were dusty. **Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). Observed a computer chair and couches in the living room with torn/worn surfaces. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed carpets were stained throughout, and the floor was damaged in bedroom 6. **All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed the walls were damaged and/or patched in some bedrooms; the baseboard beside the freezer in the kitchen was separated from the wall. **Finish all repaired walls with non-absorbant paint. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed build up residue present at can opener in kitchen. **All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).

Repeat Non Compliance: Observe black residue collecting on tile grout on floor and on corners going up wall. Repeat Non Compliance: Observed A/C vent in restroom collecting debris in need of cleaning. Observed blinds collecting dust debris build up in back right room. **Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Observed corner spinning cabinet shelving damaged in need of repair. **All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).
