Observed dresser drawers outer covering beginning to fray and come lose in front bedroom on right corner. Observed carpet stains in rooms throughout. **All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed the stove drawer with food debris, some stored pans with dried food residue, some cabinets and drawers with missing knobs, and the kitchen backsplash laminate was damaged near the sink.**Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ).**All pots, pans and other utensils used in the preparation or serving of food or drink, and all food storage utensils, shall be thoroughly cleaned after each use. 15A NCAC 18A .1618 (c ).

Observed one sink faucet beginning to corrode, the bathroom grout with dark discoloration, a shower chair with pink and black residue, the wooden toilet seat was worn, and a shower curtain with pink staining.**Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed the sky windows with webs, the exterior wooden tables and benches needed pressure washing, and some resident bed pillows were stained/soiled.**All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed the floor with dust and debris in the storage closets and in the activity room, and the carpet was stained in two resident rooms.**All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

REPEAT - Observed a large wood pile in the front yard from a fallen tree. Remove the piled wood to prevent rodent/animal harborage.**The premises shall be kept neat, clean, adequately drained, and free of litter and vermin harborage. 15A NCAC 18A .1615(a).GENERAL COMMENT: Tameka Coles was present during the inspection.#3 - Observed bottled water was stored on the floor in the living room storage closet. **Keep all food/consumable products stored above of the floor.

Observed the internal temperature of the refrigerator was 46F and several food temps above 45F: opened lunch meat - 48F, opened salsa-50F, and shredded cheese -46F. All potentially hazardous food items (yogurt, milk, cottage cheese, shredded cheese, sour cream) were move to refrigeration at the neighboring facility (2425 Umar Ct). Advised Ms. Coles to discard all opened lunch meat and opened salsa.**All perishable foods shall be stored at such temperatures as will protect against spoilage. All potentially hazardous food shall be maintained at safe temperatures (45° F. or below, or 140° F. or above) except during necessary periods of preparation and serving. 15A NCAC 18A .1620 (a).

Observed a sports water bottle was stored wet/with the lid on. **Completely dry all utensils before storing.Observed some cabinets and drawers with food debris. **Acceptable storage facilities include residential kitchen cabinets, which should be kept clean and free of vermin. 15A NCAC 18A .1618 (i).

Observed one shower curtain with yellow staining, a bathtub surface with rust, and the shower grout with mildew.**Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed some dining room seat cushions and chair backs with stains, a chair arm in the gym was worn/damaged, and the couches/chairs with debris in the living room.**All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed the floor surface near the dining room table was worn, and one resident room with carpet stains.**All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed several mosquitoe larvae in the fountain in the front yard, a fallen tree (w/large hole) in the front yard, and several spider web and gutters full of leaves. **Keep the property free of standing water to prevent mosquito breeding nuisance. **The premises shall be kept neat, clean, adequately drained, and free of litter and vermin harborage. 15A NCAC 18A .1615(a).
