15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair.- Observed slow drain at hand sinks hallway bath. Need repair. Keep all hand sinks in good repair.

General Comment: 15A NCAC 18A .1621 (d) The residential care facility shall have personal protective equipment on-site for employees to use and a written procedure for employees to follow when responding to vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that should be taken to minimize the exposure to vomitus or fecal matter.- Observed the facility with vomiting and diarrhea written plan, and not cleaning up kit put together. The facility shall gather all supplies needed and attach a written plan. Have them available for such an event.

The interior walls and ceilings, including doors, window, and window trim, shall be kept clean and in good repair. 15A NCAC 18A .1608 (a). Observed peeling paint on wall and chipping closet doors in third bedroom. Repair.

All hand sinks and baths shall be supplied with hot and cold running water through mixing devices. The residential care facility shall provide each resident with soap and individual hand-drying towels. These hand-drying towels will be stored separately after being used. 15A NCAC 18A .1610 (b). Observed no hand drying towels in third bedroom bathroom.

All foods, while being stored, prepared, served, and during transportation, shall be protected from contamination. 15A NCAC 18A .1620 (a). Observed raw bacon stored on top of cheese. Store raw meats and eggs below and separately from ready to eat foods. An indicating thermometer shall be located in each refrigerator. 15A NCAC 18A .1620 (a). Observed thermometer with multiple breaks in red line, thermometer needs replacing.

All lavatories and baths shall be supplied with hot and cold running water through mixing devices. 15A NCAC 18A .1610 (b). Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b). Repeat-Observed no paper towels in bathrooms. Provide toilet paper as well.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed mattress on back left bedroom in poor repair. Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). Observed soiled sheets, pillowcases and stuffed animal in both rooms on the left.

All solid wastes shall be disposed of with sufficient frequency and in such a manner as to prevent insect breeding and public health nuisances. 15A NCAC 18A .1614 (c ). Observed trash can overflowing. Another can may be needed.

All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a). Observed chest freezer not sealing from over accumulation of ice. Chest freezer needs defrosting.

Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b). Observed bar soap and no paper towels in bathroom.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed front door peeling paint. Door needs repainting.

The premises shall be kept neat, clean, adequately drained, and free of litter and vermin harborage. 15A NCAC 18A .1615(a). Observed trash on ground under outside table.

Observed cabinet door in bathroom offline and peeling at the bottom. Observed hole in the entry door of 2nd bedroom. Observed closet door peeling from the top in 3rd bedroom.

Observed sagging couch and one mattress. Repair/replace when worn.

Observed oven and microwave need cleaning.

Observed no paper towels in restrooms. Observed bathroom cabinet shelving flacking particle board.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed drawers broken and in disrepair in bedroom at the end of the hall. Observed no sheet on bed in the bedroom at the end of the hall.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed floor in the first bedroom and bedroom at the end of the hall in need of cleaning. Observed rug on the floor in common area tv room in need of deep cleaning.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. ***REPEAT*** Observed touch-up paint needed on door frames. Observed damage on front of bedroom and closet door. Observed touch-up paint needed on walls and bedroom door.

All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a). Observed no lighting in laundry room. PIC replaced bulb.

Repeat Observed no thermometer present in refrigerator. **An indicating thermometer shall be located in each refrigerator. 15A NCAC 18A .1620 (a).

Observed cleaning needed in kitchen drawers and cabinets. Observed sticky underside of cabinet and drawer liners to be peeling up and collecting debris. Observed touch-up paint needed on door frames. Observed growth in need of cleaning on exterior of front door. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed cleaning needed on window sills in rooms. Observed cleaning needed in-between window panes. **Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Observed no thermometer in the refrigerator. Provide. An indicating thermometer shall be located in each refrigerator. 15A NCAC 18A .1620 (a).

-Observed black buildup on the shower mat. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). -12. No Points Deducted: Observed 2 wasp nests in the window of the bunk bed bedroom. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

-Observed a rip in the mattress in bedroom by the living room and in the bottom bunk bed and in the mattress cover on the top bunk bed. -Observed a stain on the pillow from the bed on the right side in the left bedroom. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). -Observed stains on the mattress covers in the room by the living room Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Acceptable storage facilities include residential kitchen cabinets, which should be kept clean and free of vermin. 15A NCAC 18A .1618 (i). Observed kitchen cabinets and drawers need wiping out and contact paper removed/replaced. Disposable utensils shall be purchased only in sanitary containers, shall be stored therein in a clean, dry place until used, and shall be handled in a sanitary manner. 15A NCAC 18A .1618 (g). Observed foil and plastic wrap stored with chemicals/cleaners.

Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed slow drain in main bathroom. 1/2 bath needs cleaning and smelled of urine.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed tear in upholstered living room furniture.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed hole in one bedroom door.

Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Observed cracked window in one bedroom. Window sills and ledges need cleaning. A/C vents need cleaning.

Observed buildup in the stove drawer, microwave, and stove hood. Clean. Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ).

-Observed pink buildup on the shower curtain in the hall restroom. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). -Observed several wash clothes stored together in the restrooms. Store separately. Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b).

-Observed a torn mattress cover in the 1st bedroom on the left. -Observed dirt and debris collecting in the bed- room with bunk beds. -Observed some of the dresser drawers off track. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

-Observed the laminate floor in the room with 2 beds in disrepair (torn is places). Repair. -Observed the floor of the chemical closet collecting dirt, dust, and debris. Clean. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

-Observed the blinds in bedrooms and hall restroom in disrepair and/or soiled. Clean and/or replace. -Observed the paint peeling on the window sills. Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).
