Let water run for about 5 minutes. **15A NCAC 18A .1611 (d) Residential care facilities shall provide water heating facilities. Hot water shall be provided at temperatures between 105F and 116F at handwashing and bathing facilities.

Observed black residue on ceiiling above toilet in large restroom (walk in shower). The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Store opened packages of dry food products in sealable container, labeled and kept clean; gold fish stored in pantry closet. Store raw meat and raw egg products below and away from other food products. All foods, while being stored, prepared, served, and during transportation, shall be protected from contamination. 15A NCAC 18A .1620 (a).

Repair leak under kitchen two compartment sink. Observed leak on plumbing fixutre. Observed under counter equipment/equipment storage area and base drawer under stove where sheet pans are stored soiled. Thoroughly clean shelving and areas where clean utensil/equipment is stored and keep clean at all times! Observed paper towels missing from several hand washing sinks throughout residents and no soap at hand washing sink in med closet. Observed broken toilet seat in bathrooms. Replace broken toilet seats throughout facility. Observed slow draining hand washing sinks in several bathrooms. Address slow drain hand washing sinks in bathrooms. Observed shower curtain and shower chair soiled in bathroom. Thoroughly clean microbial growth from shower chair and cracks and crevices of chair. Observed base of shower soiled. Thoroughly clean flooring in tile shower. Regrout top of bathtub where tub basin meet wall tiles. Observed grout missing. Clean area around lid to washer machine. Repair or replace broken dryer. Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b). Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Repair or replace damaged flooring in kitchen area and near dining room/living room area. Clean floors and base boards throughout house. REPAIR FLOOR NEAR ENTRANCE TO HALF BATHROOM NEAR SIDE ENTRANCE DOOR. FLOORING IS ACTUALLY GIVING WAY/ SAGGING! Clean stained carpet in residents room or replace stained carpet in rooms. Repair floor in resident room near closet area. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Clean walls and ceiling throughout kitchen, laundry room, residents rooms and bathrooms! Repair roof leak in bathroom. Observed black microbial growth present on ceiling and sign of roof leak because ceiling is also damaged over shower area. Patch holes in walls in residents room and repaint/touch-up paint throughout. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed old rotten boards stored under deck at end of house. Remove old rotten boards from the premises. Tightly secure loose and missing boards and railing on deck. Store containers(cigarette bucket) so it doen not hold or collect water--> drill small holes in base of bucket/containers so water will drain. The premises shall be kept neat, clean, adequately drained, and free of litter and vermin harborage. 15A NCAC 18A .1615(a). General Comment: #9. Replace dining room table showing wear/ damaged around edge of table top. Replace pillow covering/case with stains present in resident room. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Observed shower chair and covering on shower chair soiled. Thoroughly clean shower chair and covering on chair. Observed hand washing sinks throughout house missing paper towels. Observed sealant/ varnish around edge ofd dining room table worn away and bare wood/compressed wood is exposed and damaged. Recommend just replacing dining room table. Observed damaged chair in common sitting area. Replace damaged chair in living room area. Clean window blinds in kitchen area. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed flooring damaged near door framing between front entrance room and common sitting/living room area. Observed flooring damaged near entrance to dining room, in hallway, kitchen and resident closet. Repair flooring or replace damaged flooring throughout house. Observed soft flooring near entrance to bathroom on kitchen end of house. Repair soft flooring near entrance to bathroom. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Patch small holes in wall in resident room and repaint. Recommend placing door stopper behind doors where door stoppers are missing. Clean ceiling in kitchen. Observed light fixture not working and blown light bulbs in fixtures. Replace blown light bulbs in light fixtures or replace damaged light fixtures. Clean ceiling ventilation in bathroom and return air ventilation(ceiling) throughout in hallway and dining room area. All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a). Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Observed base of utensil shelving, drawer under oven and cabinet area soiled in kitchen. Thoroughly clean utensil/equipment storage area/shelving in kitchen area. Observed bare wood shelving in pantry cabinet. Recommend sealing bare wood shelving with a varnish/sealant. Observed shower chair soiled with microbial growth on underside of chair and damaged head rest on chair. Observed faucet fixture pitting on hand washing sink in bathroom. Thoroughly clean shower chair and replace damaged head rest on chair and replace pitted faucet fixture on hand sink. Observed flooring damaged in office area and between living room and dining room. Repair flooring in office and between dining room and living room. Patch holes in walls, tightly secure wall laminate in can wash, replace missing door stopper and repaint throughout where needed. Thoroughly clean walls in residents rooms. Thoroughly clean ceiling in kitchen and bathroom. Replace damaged electrical outlet covers. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed damaged railing on back porch area, damaged and rotten fencing and containers holding water in back yard area. Repair railing on back porch, repair or remove fencing and store containers in back yard so they do not hold or collect water.The premises shall be kept neat, clean, adequately drained, and free of litter and vermin harborage. 15A NCAC 18A .1615(a).General Comment: Did not observe back flow prevention device on shower wands in both bathrooms. Mr. Jones stated that maintenance man informed him that shower wands had back flow prevention devices built in. Please provide documentation on next inspection stating and showing shower wands have back flow prevention devices built in or install back flow prevention devices on shower wands. Mr. Jones present at time of inspection.

Observed the interior of some kitchen cabinets collecting debris. Observed minor damaged to the interior of the refrigerator. Observed the refrigerator collecting some debris. Observed a rusted hand held can opener. Observed the bottom drawer of the oven collecting debris. Acceptable storage facilities include residential kitchen cabinets, which should be kept clean and free of vermin. 15A NCAC 18A .1618 (i). All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).

Repeat Non-Compliance: Observed shower chairs collecting residue. Observed a corroded bathroom faucet in the tub room. Observed the half restroom caulk at sink collecting residue. Observed some damaged tile grout around the tub. Observed some shower grout collecting residue. Observed the dryer starting to rust. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).Observed the half restroom and the shower room with no disposable towels. Observed the couches collecting debris under the pillows. Observed the leather chair damaged. Observed bed rail padding damaged in one room. Observed multiple items stored on some closet floors and on the floor in the laundry room. Observed medical supplies stored under an exposed sewage line in the office. Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

Observed the half restroom vent collecting dust. Repeat-Non-Compliance. Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).
