15A NCAC 18A .1608 (b) Wall and ceiling attachments, such as light fixtures, fans, and vent covers, shall be kept in good repair. Observed paint peeling on walls and stained ceiling tile in Hornets Nest. Observed cracking bathroom ceiling paint in Hornets next.

15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair. Observed torn shower liner in Lions den and Hornets nest bathroom.

15A NCAC 18A .1617 (a) Furnishings, including furniture, curtains, draperies, and blinds, shall be kept clean. Mattresses shall be kept clean. Observed staining on table in living area of Hornets nest.

15A NCAC 18A .1608 (b) Wall and ceiling attachments, such as light fixtures, fans, and vent covers, shall be kept in good repair. Observed marking on wall behind bed in hornets nest unit room 4.

15A NCAC 18A .1608 (a) The interior walls and ceilings, including doors, window, and window trim, shall be kept clean. Observed stained ceiling tile in living room in Hornets nest unit. Repair.

15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair. Observed staining on shower curtain located in Lions Unit room 6. Observed toilet paper stuck to ceiling in Lions Unit hallway bathroom near room 5.

15A NCAC 18A .1621 (d) The residential care facility shall have personal protective equipment on-site for employees to use and a written procedure for employees to follow when responding to vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that should be taken to minimize the exposure to vomitus or fecal matter. EHS left facility copy of new required written procedure along with copy of employee health policy. General comment.

Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ). Observed shelving of dorm style refrigeration unit in need of detailed cleaning located in Lions Den facility.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed staining on pillow in room 1 in lions den facility. Observed staining on pillow in room 5 of Hornets nest facility. Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). Observed vinyl mattress with slits in room 1 in lions den facility.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed paint needed to walling, doors, and window sills throughout both facility. Observed several stained ceiling tiles in hornets nest facility, EHS advises facility to ensure no active leak is occurring and to have stained ceiling tiles replaced. Observed popcorn ceiling peeling in corner of room 1 in hornets nest facility.

REPEAT Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed bathroom #2 caulking needed along the shower and keep in good repair.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed missing transitional floor strip between rooms and main floor. Need repair/replace transitional floor strips. Observed laundry room floor, need deep cleaning and keep clean.

Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Observed loose ceiling vent in bathroom #2, need to be secure. And keep in good repair.

Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed extremely slow water (clogged) drainage at sink in bathroom on the left. Observed holes in shower line in bathroom in room #6.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). Observed an old dingy & worn pillow on bed in room #4. Observed stains on sofa chair cushions in common area.

Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a). Tidy up storage area in room #6.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed loose floor tile in room #6

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Apply weather stripping between exit door in the common area to prevent insects or other vermin from entry. Cover holes in disabled bolt lock on door in room #6 to prevent insects and to maintain temperature control. Tighten door knob at exit door.

Observed 2 corroded bathroom faucet fixtures. Need replacing.

(Lion's Den bldg): Observed ripped and torn shower curtain liners in both bathrooms. Observed black build up around seal in tub in bathroom #2 (Hornet's Nest bldg): Observed black build up around seal in tub in bathroom #2 Observed dirty shower curtain in bathroom #1, #2 & Rm #6.

Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). (Hornet's Nest bldg): Observed soiled pillow case in Rm #4 & #5.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. (Hornet's Nest bldg): Observed floor tile gap in front of the bathtub in bathroom #2, broken floor tile in the doorway of Rm #3, cracked floor tile in Rm #3.

(Hornet's Nest bldg): Observed chipped wall tile in bathroom #2, loose wall tile in Rm #2, peeling paint on the wall and ceiling in Rm #1. Observed hole in the ceiling tile at the of Rm #1. Observed accumulated dust buildup in ceiling vent in bathroom #2. Observed peeling paint on the wall in the laundry room. SEE COMMENT ADDENDUM FOR Lion's Den bldg:

All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a). Replace burned out bulb in Rm #2, 2nd bed. Cover on lighting in activity kitchen missing.

-Observed the white covering peeling off of the kitchen cabinets- Lion's Den. All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a). -Observed buildup in the ceiling of the microwave- Hornet's Nest. Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ).

-Observed loose faucet in the hall restroom- Lion's Den. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). -Observed no soap nor paper towels at the restroom handsinks. Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b).

-Observed torn mattress covers in room 4- Lion's Den and room 1 in Hornet's Nest. -Observed stained curtains in room 1- Hornet's Nest. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

-Observed the ceiling paint peeling in both of the restrooms in Hornet's Nest. -Observed the ceiling peeling in the master restroom, room 2, and storage room in Lion's Den. -Observed microbial growth in the restroom by the office- Hornets Nest and in the master restroom- Lion's Den. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observede heavy buildup of dust on multiple window sills- Hornet's nest living room & Lion's Den storage room. Clean. Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Observed pink residue in tub and shower curtains stained in Lion's Den restrooms. Urine smell in restrooms. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed stains on bedding in rooms 131 and 128(hornet), pillowcase 130(hornet), and cushions on couches torn (lion). All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). r

Observed cove base/baseboard peeling from wall under sink in big restroom (hornet). All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed chipping paint in rooms and hole in wall in big bathroom in Hornet's nest. Ceiling chipping in both restrooms (hornet's). The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed rusting in microwave in Hornets Nest. All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).

Observed stained sheets in room 2 Lions Den. Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Reseal baseboard to wall in closet in room 5. Repair ceiling around vent in restroom in room 5 Lions Den. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed burned out lights in laundry room in Hornets Nest. All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a).

Observed rusted microwave.**All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).

Observed excessive lint in laundry, make sure dryer vent is secured to wall. Loose bathroom faucet fixture. -Repeat Non-Compliance: Observed no soap and paper towels in the restrooms.**Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).**Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b).

Observed torn living room furniture. Unzipped mattress covers. Soiled linens on beds. Used wash cloths hanging over tub faucet.**Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).**All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed storage areas need cleeaning and organizing.**Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

Observed ceiling damage and wall/paint damage. Moldy bathroom ceiling.**The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed rusted vent covers. Light not working in laundry room.**All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a).**Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Repeat Non-Compliance: Observed the interior of the microwave and the refrigerator collecting debris (hornets nest).Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ).

-Observed rust inside and on the top of the dryer (Lion's Den).-Observed the handsink faucet very loose in the 'master' bedroom (Lion's Den).Repeat Non-Compliance: -Observed no soap and paper towels in the restrooms (Both).Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b).-Observed soap scum and/or mold build up on the shower curtains (Both).Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

-Repeat Non-Compliance: Observed a debris below the couch cushions (Hornet's Nest Unit) & Couch Cushion covers in disrepair.-Observed mattress torn and multiple pairs of dirty socks under the covers in room 2 (Hornet's Nest Unit).-Observed torn comforters in room 1 & 3 (Lion's Den Unit).All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Repeat Non-Compliance: Observed the carpet in the living room frayed (both units).All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

-Repeat Non-Compliance: Observed the paint on the restroom ceiling and vent peeling (Both).-Observed the wall and/or ceiling paint peeling in multiple bedrooms in both units.The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed dusty/clogged ceiling vents in both units.Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).-14) GENERAL COMMENT: Observed what appeared to be a roach's egg sac in the storage room. Facility may want to provide pest control treatment.Effective measures shall be taken to keep insects, rodents, and other vermin out of the residential care facility and to prevent their breeding, harborage, or presence on the premises. 15A NCAC 18A .1615 (a).

Observed the dumpster doors open and one of the lids bent and falling in which may allow vermin access to the garbage. Repair or replace.All solid wastes shall be kept in durable, rust-resistant, non absorbent, watertight, rodent-proof standard waste containers which shall be kept covered when filled or stored or not in continuous use. 15A NCAC 18A .1614 (a).

-Repeat Non-Compliance: Observed the carpet in the livingroom frayed (both units).-Repeat Non-Compliance: Observed storage room floor (in room 5-Lion's Den unit) collecting dust, debris, and dead bugs. Clean.All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed the microwave with food debris accumulation.**Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean andsanitary condition. 15A NCAC 18A .1618 (c ).

Observed the bathtubs with rust, and the shower curtain with soap scum/pink residue.**Toilet; handwashing; bathing; and laundry facilities shall be kept clean and in good repair. See rule .1610 (a).C: Observed no soap or paper towels in bathrooms.**Each resident will be provided soap and individual towels. These towels will be stored separately after being used. See rule .1610 (b).

Observed a sofa cushion cover was torn and no longer fitting the cushion. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).GC: Observed a large bin outside of the bedrooms with dirty laundry and linen. Ensure that all of the residents dirty clothing and linen are stored in separate areas.

Observed resident clothing were stored on the floor.**Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

Observed the living room carpet was damaged and the floor in the storage room was not clean.**All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.General Comment:John Patton and Joleen Schrock were present during the inspection.
