15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair. Observed shower liner in need of replacement in Elm.

15A NCAC 18A .1617 (a) Furnishings, including furniture, curtains, draperies, and blinds, shall be kept in good repair. Mattresses shall be kept in good repair. Observed dresser drawer missing in Room 5.

15A NCAC 18A .1609 (b) Ventilation equipment shall be kept clean and in good repair. Observed dust accumulation to ceiling vents within residence. Clean.

15A NCAC 18A .1617 (b) Clean bed linen shall be changed when no longer clean. Observed staining on pillow case in room 5. Clean.

15A NCAC 18A .1621 (d) The residential care facility shall have personal protective equipment on-site for employees to use and a written procedure for employees to follow when responding to vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that should be taken to minimize the exposure to vomitus or fecal matter. EHS left facility copy of new required written procedure along with copy of employee health policy. General comment.

Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed shower curtain liner torn in bathroom located between rooms 4 and 5. Observed wall spongy material peeling behind toilet in the bathroom of room 6.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed slits in mattress in room 1. Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). Observed staining on bed linen in room 6.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed touch up paint needed to walling, doors, and window sills throughout facility.

An indicating thermometer shall be located in each refrigerator. 15A NCAC 18A .1620 (a). Observed dysfunctional thermometer in the refrigerator. Need batteries replacement or replace a new thermometer.

Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed shower curtain need to be cleaned and keep clean.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed damaged floor around air vent in bedroom #4, need repair or replace. Observed floor in bedroom #2 and laundry room floor need cleaning, and keep clean.

Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed dingy stained shower curtains in bathroom on the right and mildew on both shower curtain and liner in bathroom on the left. Replace or wash shower curtains when visibly dirty.

Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). Observed dirty pillow on bed with no pillow case provided in room #3 and stains on pillow with no pillow case in room #4. Observed dirty sheets on bed in room #5. Wash dirty sheets when visibly dirty. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed dirty area rug in common area and ripped cushions on chairs. Repair or replace damaged cushions on chairs to prevent further damaged.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed loose door knob on laundry room door. Tighten door knob to prevent from falling off door. Observed missing handles on three cabinets and refrigerator in need of cleaning.

Observed black build up around the seals on bathtub in bathroom #2. Observed pink build up around the edge of the wall of the bathroom sink in bathroom #6.

All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a). Replace missing cover on lighting in storage closet. Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Clean dust build up, spider webs and dead bugs out of the window in the storage closet in Rm #6.

-Observed loose faucet in the master restroom. Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). -Observed no soap nor paper towels at the restroom handsinks. Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b).

Observed torn mattress cover in rooms 4 and 6. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

-Observed microbial growth on the ceiling and wall by the hall tub- hall restroom. -Observed the ceiling paint peeling in room 4. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed the paint peeling at the windows and dust buildup on the window sills- storage room. Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Observed torn comforter and stained comforter in room 2 and 3. Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Observed floor tiles needing to be cleaned in restrooms and storage room. Observed cracked floor tile near floor vent in last room. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed duct tape on cushion on couch. Repair/ remove duct tape. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed stains on bed linen in room 6. Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b).

Repair floor tile near vent in room 6. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed crayon marks on wall and counter in room 5. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed refrigerator needs defrosting and needs cleaning of residue build-up,**Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ).

Repeat Non-Compliance:Observed no soap and paper towels in the restrooms. Observed mildewed shower curtain.**Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A.1610 (b).**Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed hole in couch cushion.**All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed storage rooms need cleaning and organizing.**Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

Observed wall damage in several areas. Moldy wall/ceiling in bathroom.**The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

-Observed residue in the refrigerator.-Observed residue collected on the interior of the microwave.Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ).-Observed the refrigerator gasket hanging off.All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).3) GENERAL COMMENT: Observed no thermometer in the refrigerator.An indicating thermometer shall be located in each refrigerator. 15A NCAC 18A .1620 (a).

Repeat Non-Compliance: -Observed no soap and paper towels in the restrooms.Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b).-Observed soap scum and/or mold buildup on the shower curtains.-Observed the washer and dryer with a thick layer of residue on them. Clean.Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

-Observed the wood under the mattress in room 2 splintering. Make smooth and easily cleanable.-Observed debris under the mattress in room 3.-Observed the couch cushion in the den in disrepair.All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

-Repeat Non-Compliance: Observed the ceiling peeling in room 2.-Observed wall paint damage in room 1 and 6.-Observed rust collecting on the top of the door frame in the right restroom.The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed heavy dust buildup on the ceiling vents.Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

Observed the dumpster doors open and one of the lids bent and falling in which may allow vermin access to the garbage. Repair or replace.All solid wastes shall be kept in durable, rust-resistant, non absorbent, watertight, rodent-proof standard waste containers which shall be kept covered when filled or stored or not in continuous use. 15A NCAC 18A .1614 (a).

Repeat Non-Compliance: Observed clothes stored on the floor in rooms 3 & 4.Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

-Repeat Non-Compliance: Observed missing baseboard in the living-room.-Observed the ceiling peeling in room 2.The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.-GENERAL COMMENT 4) Observed hair and debris in the refrigerator. Clean. Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean andsanitary condition. 15A NCAC 18A .1618 (c ). Ms. Joleen present.

Observed some resident clothing stored on the floor in some rooms.**Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

Observed the door frame with rust in the bathroom, and a missing baseboard at room 3/living room.**The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed the ceiling vent in the bathroom with dust build-up.**Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).General Comments:#9 - Observed a large bin outside of the bedrooms with dirty laundry and linen. Ensure that all of the residents dirty clothing and linen are stored in separate areas.John Patton and Joleen Schrock were present during the inspection.
