REPEAT 15A NCAC 18A .1607 All floors shall be kept clean and in good repair.- Observed dust and debris on flooring throughout the facility (rooms, closets, laundry closets, underneath beds and furniture). Detail cleaning needed. Keep clean.

REPEAT 15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair.- Observed soiled/stained/mildew on the shower stall bathroom hallway 1. Cleaning needed and keep clean.

REPEAT 15A NCAC 18A .1621 (d) The residential care facility shall have personal protective equipment on-site for employees to use and a written procedure for employees to follow when responding to vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that should be taken to minimize the exposure to vomitus or fecal matter. -Observed the facility with a vomit and diarrhea written plan, however without a kit for clean up. Ensure to obtain a vomit/ diarrhea kit and supplies. Have them available for such an event.

15A NCAC 18A .1607 All floors shall be kept clean and in good repair.- Observed dusty flooring throughout the facility (closet, laundry closet, underneath the beds, along the wall, hard to reach corners). Observed black carpet in living room at the front need cleaning.Clean and keep clean.

15A NCAC 18A .1610 (a) Residential care facilities shall provide toilet, handwashing, and bathing facilities that are available for use by residents and employees that are kept clean and in good repair.- Observed soiled toilets in both bathroom, one soiled/stained hand sink in one of the bathroom. Detail cleaning needed and keep clean.

15A NCAC 18A .1617 (a) Furnishings, including furniture, curtains, draperies, and blinds, shall be kept in good repair. Mattresses shall be kept in good repair.- Observed one side damage closet door at the bottom in one of the resident room toward the back side of the building. Repair or replace needed.

REPEAT Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).- Observed black/pink buildup along the caulking tiles of shower/tub and inside the basin of hand washing sinks. Observed soiled/stained toilets inside the bowl and surrounding. Need detailed cleaning and maintain clean.

REPEAT All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. -Observed flooring throughout (under beds, closets, laundry room, etc..) Detail cleaning needed, and keep clean.

Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).- Observed ceiling AC filter cover covered with dusts. Need details cleaning and keep clean.

All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).- Observed rusted microwave. Need repair or replace, keep in good repair. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. - Observed damaged door frame at sink closet, need repair.

Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ). Observed brown accumulation to flooring of reach in freezer.

Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b). Observed no soap at handsink located in bathroom #1. Employee supplied soap to handsink during time of inspection (no points for violation). Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed residue build up under toilet lid and surrounding outside of toilets. Observed black build up inside to caulking of shower tiles. Observed brown build up under shower chair in restroom #2. EHS advises facility to complete detailed cleaning to toileting and bathing equipment throughout facility.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed night stand in room 2 with doors missing. Observed mattress cover torn on bed in room #3. Observed brown staining on mattress in room #3. Observed some vinyl blinds torn/in need of repair sporadically throughout facility.

Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a). Observed cleaning needed to closet in room #6.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed touch up paint needed to walls, wall joints, and baseboards throughout facility. Observed no soap or paper towels at hand sink in side B restroom. Per conversation with employee the residents on side B have their own soap and paper towels. Observed toilets in both residential bathroom with build up under toilet lid and within toilet bowl, cleaning frequency to toilets needs to be increased. Observed shower curtain liner on side A (closets to front door) with staining on the bottom of liner. EHS advises facility to replace/clean liner. Observed heavy black build up inside the tile grout of shower in bathroom on B side of facility. EHS advises facility to clean shower.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed hole in black mattress located in the second room on side A. Observed mattress cover in room 3 on side A torn. EHS advises facility to repair/replace mattress and to replace torn mattress cover. Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). Observed staining on black blanket in room 2 on side A.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed white scuffing to flooring throughout facility.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed minor scuff markings to walls in bedrooms throughout facility.

Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).Observed 2 high ceiling/wall vents in main walkway with dust accumulation. Cleaning to vents is encouraged.

REPEAT Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed toilet seats loose in both bathrooms in need of tightening.

REPEAT The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed walls throuhgout in need of touch-up painting and some areas of walls inside bedrooms in need of general cleaning.

REPEAT entilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Observed vents inside all bathrooms having heavy dust build-up in need of general cleaning.

All foods, while being stored, prepared, served, and during transportation, shall be protected from contamination. 15A NCAC 18A .1620 (a). Observed several cartons of eggs stored with and above ready to eat foods. Store in drawer or container that is spill proof. Containers of food shall be stored above the floor, on clean racks, shelves, or other clean surfaces, in such a manner as to be protected from splash and other contamination. 15A NCAC 18A .1620 (d). Observed bottled water and bags of potatoes stored on floor in pantry. Scoop handle down in brown sugar. Keep handles out of contact with food. Once opened, store bags of cereal, rice, etc. in a sealed container or sealed baggie. Don't store any food on the floor

Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed shower tiles need cleaning. Lots of build-up. Resecure loose sink to wall. Tighten loose toilet seat. Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b). Observed soap towels and toilet paper not supplied to bathroom.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed furniture needs dusting. Some mattresses stained or saggy. Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). Observed sheet torn and soiled.

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. Observed floors need cleaning in closets, behind stuff stored on floor.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed walls and doors with grime build-up. Wall damage around mop sink. Recommend FRP or othe non-absorbent material.

Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Observed window sills and window areas need cleaning.

Thoroughly clean underside of shower chairs and legs on shower chair. Replace damaged shower curtain. Thoroughly clean area around lid on clothes washer machine. Provide soap and paper towels at all hand washing sinks at all times. Tightly secure and caulk hand washing sink to wall in bathroom. Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). All residential care facilities shall be provided with approved sanitary toilet, hand washing and bathing facilities complying with state licensure requirements. 15A NCAC 18A .1610 (a).

Repair damaged arm rest on wheel chair, replace or remove from premises. Thoroughly clean pillows on residents beds or replace. Clean spots and stains on mattresses and box springs. Replace damaged mattresses and box springs. Replace damaged chairs in common living room area. Repair damaged drawers on night stands or replace. General cleaning of furniture and fixture in resident room that smears items. Clean chairs to dining room tables and legs on tables. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Repair damaged chair molding and door framing in dining room area. Thoroughly clean walls and closet doors throughout in residents rooms. Thoroughly clean walls in hallway and bathrooms. Repair wall in can wash area. Repaint wall and closet doors where needed. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Clean ceiling ventilation in bathrooms. Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b).

All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a). Observed microwave with rusted interior. Refrigerator storage bins cracked and duct taped.

Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Observed roll in shower with damaged tile/grout in back corner of wall and floor. Hand sink needs to be recalked to wall.

All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Observed couches with damaged exterior. Dusting of furniture items needed in some bedrooms.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed crescent shaped dents in ceiling of one bedroom. Holes in some bedroom/bathroom walls behind door. Lots of smudges from hands on doors throughout.

Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1609 (b). Observed window areas and sills dusty. Several ceiling vents dusty.

All solid wastes shall be kept in durable, rust-resistant, non-absorbent, watertight, rodent-proof standard waste containers which shall be kept covered when filled or stored or not in continuous use. 15A NCAC 18A .1614 (a). Observed one rolling garbage missing its lid.

Repeat non-compliance: Observed manual and mechanical can openers collecting residue on blade. Observed residue in oven drawer.All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).

Repeat non-compliance: Observed damaged shower chair back hall bathroom and residue collecting under chair. Observed shower curtain collecting residue in front hall restroom.Toilet, handwashing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a).

Observed baseboard in back restroom with pink and black residue along wall next to shower basin. Observed damgaed walls in the back schower room behind door. Observed several closet doors damged in resident rooms.The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed can opener blade collecting residue.All multi-use eating and drinking utensils shall be thoroughly cleaned after each usage, and the facilities needed for the operations of washing and rinsing shall be provided. 15A NCAC 18A .1618 (b).

Observed box spring with soil present in last hall bedroom to the right of the bathroom. Observed wall damaged in rooms throughout facility. Observed wall over mop sink basin with chipping paint.The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.Recommend placing FRP plastic on walls to create a non absorbent surface around basin to protect wall from splash.
