15A NCAC 18A .1621 (d) The residential care facility shall have gloves, personal protective equipment, disinfectant, individual disposable towels, and a coagulating agent on-site for employees to use and a written procedure for employees to follow when responding to vomitus or fecal matter on facility surfaces. The procedure shall specify the actions that employees shall take to minimize the exposure of employees, residents, guests, food, and additional surfaces to vomitus or fecal matter. EHS observed facility does not have a vomit and diarrhea cleaning plan. EHS emailed a copy

All foods, while being stored, prepared, served, and during transportation, shall be protected from contamination. 15A NCAC 18A .1620 (a). Observed boxes of individual wrapped snacks stored on the floor in the garage. Needs to be stored off the floor.

The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Observed broken window in shared bathroom. Needs repair.

Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b). Observed no hand drying device (paper towels, towels etc) in bathrooms throughout.

Observed sticky residue on kitchen cabinets and dust and debris on dishwasher door when opened. Handle on over missing. Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ).

Replace non fitting toilet lid with proper fitting lid in middle bedroom's bathroom. Adjust offline bathroom cabinet door so that it will shut securely when closed in middle bedroom's bathroom. Observed sunken and damaged (cracked/peeling) sofa in common area. Observed chair in common area in need of deep cleaning, sunken arm rest and badly worn. Observed sunken mattress in bedroom on the right. Observed damage to top dresser drawer in bedroom on the right. Observed dirty pillow case on pillow in bedroom on the left. Chair seating in the dining room needs deep cleaning. Dining room table top needs to be refinished or replaced. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed damaged wood laminate flooring in dining room. Observed dingy/dirty rug in common area. Sweep debris out of the corners of the common area. Observed gap in wood laminate flooring in hall. Floor also appeared to be weak when pressing foot on the gapped area. Laminate flooring in main bathroom bulking up, needs to be glued down. Flooring throughout needs to be repaired. All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed a hole in the wall behind the TV in the common area and behind the front door. Observed white substance on wall in kitchen. Replace broken light switch cover at the front door. Observed peeling popcorn ceiling in bathroom shower located in middle bedroom. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed wall near shower head in the hallway bathroom in need of painting. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Store raw meat and raw egg products below and away from other food products. Store opened packages of dry food products in sealable container labeled and kept clean. Observed raw hamburger and raw eggs stored above other food products in refrigeration unit. Observed opened bag of sugar in kitchen cabinet. Repair flooring near exit door in dining room area. Repair flooring beside kitchen refrigeration unit. Install quarter round shoe molding along edge of flooring in 1st bedroom on the left. General Comment: #12. Touch-up paint throughout facility where needed.

Repair gate entering into backyard and replace damaged boards on gate. Remove leaves and boards from backyard area. Repair fencing in backyard area. Clean out gutters. Remove from damaged tent from back yard. The premises shall be kept neat, clean, adequately drained, and free of litter and vermin harborage. 15A NCAC 18A .1615(a).

All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. General Coment: #12. Touch-up paint wall near window in first room on the left. Only inspected the first room on the right and first room on the left. The back corner bedroom was locked and was informed by staff that the one locked room is being used as an office. Last year (10/15/2016)inspected the back corner bed room and bathroom/shower in bed room. Clean leaves from gutter.Ms. Shairaz McCullen present at time of inspection.

Observed leaking plumbing fixture under two compartment sink in kitchen. Repair leak under sink. Observed loose hinge on cabinet door. Repair hinge on cabinet door. Observed hood over oven/stove not tightly secure. Tightly secure hood system. Clean base of drawer under oven. Observed cup shelving in cabinet area damaged. Observed laminate missing from base shelving in cabinet and particle wood flaking and breaking apart. Repair shelving in cabinet area. Wipe/clean utensil shelving in lower cabinet in kitchen.All containers and clean utensils shall be stored in a clean place. 15A NCAC 18A .1618 (f).All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).

Clean window blinds in residents room. Clean chair or replace chair in resident room. Clean top of furniture and base of drawer in night stands and dressers in resident room. Repair floor in dining room area. Repair flooring in shared bathroom near hand washing sink. Provide transitional floor moulding near coat closet and at entrance to laundry room/area. Repair rug or replace rug in living room area.All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Tightly secure wall paper in kitchen behind two compartment sink. Patch hole in wall in hallway area and repaint. Repaint wall where patch work has taken place. Observed can opener blade rusting, damaged inside bowls (rough inside), and food debris collecting on serving spoon and rust on spatula stored as clean.All multi-use eating and drinking utensils shall be thoroughly cleaned after each usage, and the facilities needed for the operations of washing and rinsing shall be provided. 15A NCAC 18A .1618 (b).
