Observed minor black buildup on the grout in the shower located in the hallway bathroom. Clean.

The procedure shall specify the actions that should be taken to minimize the exposure to vomitus or fecal matter. Facility did not have copy of required procedure plan. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. Repair lower section of wall in dining room and install quarter round molding. Highly recommend painting quarter round molding in bed room and recommend placing a high door stopper on back of bathroom door. Wipe/clean walls throughout where needed if needed(front door).

The premises shall be kept neat, clean, adequately drained, and free of litter and vermin harborage. 15A NCAC 18A .1615(a). Remove old boards from behind house. Remove old grills, boards and damaged shed from edge of wood line and property.
