Observed toilet not operating in the boys restroom near the art room. **Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).

Observed water stained ceiling tiles in room 15, speech and language office and in the cafeteria. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed dead bugs in the light shields in music room, room 14 and the cafeteria. Observed vent cover missing in the boys restroom in the modular. **All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b).

Drinking fountains shall be provided with adequate water pressure and be properly regulated (i.e. water pressure should be regulated so that an individual’s mouth does not come in contact with the nozzle and so that the water does not splash on the floor). 15A NCAC 18A 2406 (b). Observed water pressure too high and in need of adjustment at short fountain in prek hall. REPEAT. No points deducted due to one fountain being in need of adjustment.

Observed damaged tiles at bottom of wall in main hall boys bathroom. (Possible leaking has caused edging damage in music classroom on other side of wall.). Observed damaged bathroom walls in boys 108X bathroom (Custodian stated bathroom is not used.). Observed wall edging damaged in girls bathroom in front trailers and in womens staff bathroom in front trailers. Needs repair.

Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). Observed two broken sink faucets in girls trailer bathrooms. (Custodian states they work but makes loud vibrating noise, so two sinks have been shut off.). Needs repair. REPEAT.

Observed one of hand washing sinks not operating in the girls restroom on the main hall. Observed two of the sinks not operating in the boys restroom in the modular units. Needs to be repaired. **Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).

Observed water stained ceiling tiles in room 10 (main building) and room 3 in the modular unit. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed dead insects in the light shield in room B10 and the hall near the media center entrance. **All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b).

Observed missing baseboard tile in 8th grade girls restroom. Observed cove base needing to be reattached to wall in 6/7 grade girls restroom. Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c).

Observed hand sinks needing to be cleaned (hair) and rust around edges of hand sink in 6/7 grade girls restrom. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).

Reattached baseboards in halls (various). Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed floor damage in girls restroom in mobile. Observed baseboard needing to be reattached to wall in girls restroom in mobile. Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c). Observed disinfectant wipes stored in reach of children in (5th grade) 13, and B7 (multiple on table near exit door). Observed several hand sanitizer stored on counter near hand sink in B5. Keep all hazardous items stored away from children. OBserved garden boxes overgrown. Must maintain to prevent harborage. Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a). The premises of the school under control of the principal shall be kept neat and clean at all times. 15A NCAC 18A .2415 (a).

Thoroughly clean water nozzle, base of fountain, mouth guard and handles on water fountains! Observed pink microbial growth on water nozzle and water fountain basin in Modular 27 unit. WIpe/clean walls and bathroom stall fixtures throughout in bathrooms in hallway area. Repair walls in modular 27 boys bathroom and boys bathroom in modular unit located outside of cafe/gym area. Replace missing ventilation cover in modular unit bathroom and bathroom located near social studies/math room. Clean underside of wall urinal in boys bathroom near office. Tightly secure loose laminate on stall fixture in boys bathroom leading out to bus parking lot. Repair or replace damaged door in girls bathroom near office area. Replace missing door handle on boys bathroom door in modular 27 unit. Thoroughly clean and unclog toilets in outside modular units. Repair or replace damaged flooring in modular 27 boy and girls bathroom. Observed floors damaged in both girls and boys bathroom. Eliminate gap between wall and floor in boys bathroom in modular unit located outside of cafe and gym area. Urine is getting in gap along base of wall and flooring and causing the strong urine smell. Repair stall door in girls bathroom outside of cafe and gym area. Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Observed spring loaded faucet fixtures on hand washing sinks in bathrooms within classrooms with short run time. Spring loaded faucet fixtures shall operate for at least fifteen seconds. Clean hand washing sink basin in classroom area( math, social studies room,etc). General Comment:---> Observed two hand washing sinks missing paper towels. Provide soap and paper towels at all hand washing sinks at all times. Repair loose and damaged laminate ion hand washing sink counter top iin room14. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).

Clean floor under hand washing sink cabinet area in rooms within main school building. Repair damaged floor tiles in modular unit 27 hallway area and classrooms in modular unit. Thoroughly clean floor in modular units classrooms. Replace missing corner molding in hallway area in modular unit 27 near bathrooms. Nail in protruding nail where corner wall molding is missing! Monitor water stained ceiling tiles in modular unit and address any roof or HVAC leaks. Tightly secure loose ceiling tiles in modular unit classrooms. Clean and organize storage closet and areas under hand washing sinks within classroom. Remove storage off the floor in modular unit closets and clean flooring. Storage spaces and custodians' closets shall be kept clean and arranged so as to facilitate cleaning. 15A NCAC 18A .2411. All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.

Observed base of toilet and underside of urinals soiled throughout in all bathrooms in hallway, bathrooms within classroom and modular unit bathrooms. Observed hand washing sinks soiled in all bathrooms in hallway area and in hand washing sink within classroom. Thoroughly clean all hand washing sinks in bathrooms located in hallway area and within classroom area. Recommend not pouring cereal and milk in hand washing sink within classroom. Instead place large trash can in hallway so children can properly dispose of items from breakfast! Observed loose/damaged faucet fixture on hand washing sink in boys modular unit bathroom near front parking lot. Observed soap and paper towels missing from several soap dispenser and paper towel dispensers. Provide soap and paper towels at all hand washing sinks unless operating hand drying device is located near hand washing sink and is working. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).

REPAIR DAMAGED FLOORING IN ROOM M-6 LOCATED IN MODULAR UNIT IMMEDIATELY! Observed floor tile missing and plywood in sub flooring is water damaged and soft. I almost fell through flooring in room! Clean walls throughout in hallway and classroom area. Repair wall paneling and ceiling in outside modular units. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Remove storage from under hand washing sink cabinet area and thoroughly clean floor under sink area. All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.

Observed light bulbs not working in light fixtures in classrooms throughout school building! Replace blown light bulbs in light fixtures throughout entire school building and modular unit. Clean return air ventilation in classroom in main school building and modular units. Clean/dust window blinds, top of shelving, smart board projector and projector braces. Remove broken desk from classrooms and bathrooms. All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b).

Observed air freshener, hand sanitizer, disinfectant spray not properly stored within classrooms. Store cleaning products, bulk container of hand sanitizer and disinfectant spray out of reach of children or locked away. Teacher may keep one container of hand sanitizer in general area where teacher can supervise children getting sanitizer. Observed trash and debris behind, beside and in front of dumpster area. Clean grounds around dumpster! Clean grounds outside of Pre K/ kindergarten hall and playground. Most toilet bowl basins collecting stains and residue. The Health Room toilet in need of flushing. Urinal basins dirty in the Boy's Gym area restroom. The Mod Boy's restroom floor collecting urine, the baseboard damaged, and the flooring damaged/cracked.
