Observed water stained ceiling tiles outside of room 1045, 1024, 3040, 3048, 3004-3009 hallway, in room 1034 and 1061. Observed ceiling in the girls’ restroom on the 1st floor. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed toilets and urinals throughout missing sensor covers. Needs repair. REPEAT. Observed door to stall in boys gym bathroom cracked in half.

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed ceiling peeling/damaged in mens locker room. Observed wall damaged in 2000 hallway. Observed stained ceiling tiles in 2034 and 1026.

Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A .2406 (b). Observed several clogged drinking fountains throughout school building. Needs repair.

Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed toilets not operating in 3rd floor boys bathroom and girls bathroom across from 3002. Observed toilet not working in 2nd floor boys bathroom across from 2002. Observed urinal in boys bathroom in cafeteria broken. Observed toilet in JV field house locker room in need of cleaning. Work orders have been placed for broken toilets/urinal. REPEAT.

All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411. Observed storage on the floor in storage and laundry rooms in field house. Observed gym storage with balls and other items on the floor. Observed boxes stored on the floor in a manner not to permit thorough cleaning in 1006 storage. Needs to be stored 15 inches off the floor or in a manner to permit through cleaning.

Drinking fountains shall be provided with adequate water pressure and be properly regulated (i.e. water pressure should be regulated so that an individual’s mouth does not come in contact with the nozzle and so that the water does not splash on the floor). 15A NCAC 18A 2406 (b). Observed water fountains sporadically throughout school building with too high water pressure. Per Head custodian school is still under warranty, and contractors/plumbers have been made aware of issue.

Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed 1 toilet in girls restroom near cafeteria out of order. Observed 1 toilet in boys restroom out of order on third floor. Work orders have been submitted to have toilets repaired per head custodian. Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Observed toilet tissue stuck to walls in restrooms sporadically throughout school building.

Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed numerous bathrooms having soiled toilets in need of general cleaning / flushing and disinfecting.

REPEAT Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed water damaged ceiling tiles throughout school in need of repair. (note) serious area of concern (media center).

Observed stained ceiling tiles at entrance in D building, damaged arch in C building (paint peeling and black residue on 1 arch near lockers), damaged tile above water fountain in E building, hole in ceiling above display case in F building, stained ceiling in F5, stained ceiling in halls in P building (throughout), damaged floor tiles in P building, wall peeling in P5, damaged floor tiles in J11 at entrance, black residue on ceiling tiles at entrance of boys restroom in A building (top floor), and hole in ceiling tiles near A23 and A25.

Observed microban and disinfectant spray on table in D2, Lysol wipes on desks in B2, B3,E3, E7, and Lysol spray on desk in I1. Keep all hazardous items stored away from children. Adhere to directions on labels. Classrooms not in use today, but are used daily.

-Observed multiple toilet paper dispensers broken or missing in multiple restrooms. Repair or replace. -Observed a toilet shifted over and a puddle of water on the floor in the girl's gym restroom. Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).

REPEAT: Observed cobwebs in the corners in the corners of several buildings. Observed tissue wads on the ceiling in the boy's restroom- "F" building. Observed stained ceiling tiles (some budging, some with microbial buildup, and some falling) in multiple building hallways, in the media center, classrooms, restrooms, storage rooms, etc. Observed parts of the cove bases removed from the wall in boy's "B" building restroom. Observed paint peeling on the window in the upper "A" hall boy's restroom. Observed the floor drain clogged allowing water to puddle on the floor in the "C" building storage room. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

-Observed the HVAC units rusted and peeling in D5 & D6. Repair these and any others in the same condition. -Observed bugs in the light shield in the boy's upper "A" hall restroom. -Observed the wall vents dusty in F8. Clean wall under hand drying devices in all bathrooms. Thoroughly clean floor near wall/ floor mounted urinals in buildings. Clean writing from walls /bathroom stalls in bathrooms. Thoroughly clean bathroom walls in P building. Clean modular unit bathrooms and keep clean even if units are not being used! Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c). Thoroughly clean spring loaded faucet fixture in boys and girls downstairs bathroom in A building. Provide soap in upstairs boys and girls bathroom in A building---> replace broken dispenser. Clean sinks in room A-25, B-7, C-3. Unclog sink in C building girls bathroom. Clean and unclog sinks in classroom areas. Repair hand drying device in girls bathroom in media center and all hand drying devices in girls and boys bathroom in F and P building bathrooms! Repair faucet fixtures throughout in bathrooms with damaged or missing handles. Repair leaking hand wash sink and clogged sink in J-3 art room(film processing and print room) Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).

Repair major roof leak in C, D and E building. Wipe/clean walls throughout in classrooms and hallway area. Repair roof/ceiling leak in band and chorus room and A-21. Replace water stained ceiling tiles throughout in classrooms. Monitor and repair ceiling in hallway area in A building on 2nd floor. Repair/replace damaged floor tiles in modular units. Address any roof leaks in modular unit and replace water stained ceiling tiles. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Replace damaged blinds throughout in classrooms. Clean/dust ventilation covers on heating and cooling units in classrooms. Replace damaged shelving under cabinet area in D-6. Replace or repair damaged covering/pading on weight benches in weight room. Remove broken glass from window in modular unit in MC 731(11). All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b).

Clean grounds under and around ramps to modular units. Empty trash from trash cans at baseball/soft ball field. Clean trash/debris from around dumpster area. Observed missing and damaged boards on railing to ramps leading to modular units. Secure loose faucet fixture to hand washing sink in girls bathroom in c building. Replace missing handle on hand washing sink faucet in girls bathroom in D building and bathroom in E building. Provide soap and paper towels at all hand washing sinks at all times. Observed several hand washing sinks within rooms missing paper towels. Observed water stained ceiling tiles hallway area and classroom area throughout in different building. Repair any and all roof leaks, then replace water stained ceiling tiles. Repair window, wall or roof leak in classrooms above and behind auditorium. Tightly secure loose ceiling tiles throughout in classroom areas. Touch-up paint throughout in classroom areas. Observed signs of serious roof leak in D-6 lab. Repair leaks. Observed damaged floor tiles throughout in modular units. Replace missing and damaged floor tiles throughout in modular units and classrooms. Repair roof leak in MC860 and repair and repaint ceiling. Secure loose wall moulding in MC860. Clean floor under bleachers in gym area. Clean carpet in media center. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Clean heating and cooling units in classroom area. Repair any leaks on heating and cooling units in room H 4. Secure loose cover on heating and cooling units within classrooms. Replace missing drawer doors and cabinet doors on cabinets in classrooms where needed. Wipe/clean window blinds throughout. Replace broken/damaged window blinds or just remove window blinds. Clean top of heating and cooling units located in hallway area. Clean ceiling mounted vent covers throughout in buildings. Replace blown light bulbs in light fixtures throughout in classrooms or repair light fixtures. All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b).

Remove storage off the floor in storage rooms/area and thoroughly clean floor in storage rooms/storage areas!
