REPEAT Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c). Observed cracked baseboards and floor tiles in boys Mercy 3rd floor bathroom. Repair.

Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). Observed girls bathroom spring loaded faucet not working properly. Needs repair.

Observed several stained ceiling tiles and missing ceiling tile in room 200 Mercy building. Repair/replace. Observed large hole in wall and wall damage around urinals in boys bathroom 3rd floor Mercy building. Observed peeling/tearing ceiling in girls locker room. Observed cracked/damaged floor tiles in boys locker room. REPEAT

Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). Observed storage closet in boys bathroom in admin building contains chemicals and is unlocked. Observed custodial closet in Mercy building floor one unlocked with chemical storage inside. Observed aerosol sprays stored accessible to children in several classrooms. Needs to be locked in order to store toxic materials in a manner which avoids health hazards.

Observed water fountains throughout buildings with pink build-up. Clean water fountain nozzles on a regular basis with a food-grade sanitizer to prevent build-up. **Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A .2406 (b).

Observed cleaning needed on the base of toilets in several restrooms throughout buildings. Observed loose toilet seats throughout girls and boys restrooms of buildings. Observed water stains on ceiling in boys admin building restroom. Observed a large current leak in the girl's locker room. Work is currently being done (hole cut into the ceiling). Observed a dumpster without lids and the doors open. Provide new. Solid waste containers shall be kept clean, in good repair, and emptied when full, but not less than once a week. 15A NCAC 18A .2414 (b).
