Observed water stained ceiling tile near back exit, art room and computer room. Observed dust build up on tiles in the music room. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed water stained ceiling tiles on C-Hall, B2 and Media Center. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). EHS observed disinfectant wipes/aerosols/essential oils or other toxic materials labeled “keep out of reach of children” stored accessible to children inB18, B1, B7, A2, and A11. Needs to be stored to avoid health hazards.

Observed stained ceiling tiles in main hall outside bathrooms, first floor main hall stair well, outside A hall bathrooms, and in nurses office. Observed rusty ceiling in boys A hall bathroom. Needs repair. REPEAT.

All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411. Observed art room storage on floor in B hall. Observed storage on the floor in gym storage room. Needs to be stored 15 inches off the floor or arranged to permit thorough cleaning. REPEAT.

Observed storage closet on the second floor with many boxes stored on the floor. **All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.

Observed water stained ceiling tiles in rooms B-19, B-11, B-7, B-55, A-1, A-10, C-8, C-30, Media Center and Tele/Comm closet. Ensure that leaks are not ongoing and replace tiles. Observed rubber covering on steps of gym in need of repairing. Observed dead insects in the electrical closet on B-Hall. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed boxes stored on the floor in the Media Center, room A-4, C-8 and storage closet on C-Hall. **All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.

Observed droppings in the cabinets in rooms C-17, C-26 and Health Room. Observed roach in room C-17 cabinet. Ensure that droppings are cleaned up and pest management is contacted. **There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a). Observed chemicals within reach of children in room A-10, C-30 and C-26. Items were placed within cabinets during inspection. **Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b).

Observed water stained ceiling tile at entrance of boys restroom on A hall and wall damage beside toilet in last stall of boys restroom on C hall. Observed toilet unflushed in boys restroom on A hall. Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).

Observed disinfectant wipes stored in reach if children in classes A19, C15, Band, and nurses office. Please keep all hazardous items stored away from children. Replace broken toilet seat in girls gang bathroom on 2nd floor. Repair out of order toilets in boys bathroom in boys bathroom on 2nd floor. General wiping/cleaning of stall fixtures in bathroom. Clean underside of wall urinals and base of toilets in gang bathrooms on 2nd floor on A and C hall. Clean toilet paper from ceiling in boys gang bathroom on A and C hall. Replace broken base tile cove molding in bathrooms on A hall. Repaint wall near soap dispenser in modular unit bathroom. Toilet fixtures shall be kept clean and in good repair. 15A NCC 18A .2408 (d). Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).

Repair front of hand washing sink counter top in boys bathroom in modular unit. Thoroughly clean area around faucet fixtures on hand washing sink in both girls and boys bathroom in modular unit. Observed area around faucet fixtures soiled. Secure faucet fixture to sink in B1 Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).

Replace damaged floor tile in gym area and in stairway area. Monitor and repair gap along base of wall and floor in B8. Repair any roof or HVAC leaks, then replace water stained ceiling tile in B11, near elevator on first floor, C30 and MC389. Have someone to address water stained ceiling tiles in MC389 as soon as possible. Microbial growth observed on ceiling tiles and moisture observed in and on light shield near ceiling tile in MC389. Clean wall in stairway area. Repaint hand railing in stairway area. Repair corners of walls and repaint throughout in classrooms on A and C hall. Repair wall above wall mounted book shelving in A7. Patch hole in wall near exit door in A7 and repaint

Provide additional shelving in storage rooms so items can be stored off the floor in storage room located at end of A hall and on B and C hall. All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.

Observed roaches in girls and boys gang bathroom on second floor. Please have school exterminated to address roach issue as soon as possible and continue to spray to address roach issue. There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a).

Observed (1) water nozzle soiled on water fountain 2nd and 3rd grade hall. Observed (3) mouth guards on water fountain soiled on 4th-8th grade hall. Thoroughly clean water fountains. Improvement made on cleaning water fountains since last inspection. Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A .2406 (b).

Clean walls throughout in bathrooms. Clean toilet paper from ceiling in bathrooms. Thoroughly clean stall walls and walls in outside modular unit bathrooms(boys and girls). Repair out of order toilet in girls bathroom near gym area. Thoroughly clean base of toilets and underside of wall urinals in bathroom. Repaint lower section of wall in boys bathroom on 4-8 hall in last stall fixture and in B-19 bathroom. General cleaning of floors in bathrooms where needed. Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c). Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).

Thoroughly clean area around handle/faucet fixture on hand washing sinks in gang bathrooms and bathrooms within classrooms. Observed area around handle/faucet fixtures soiled. Observed rust around handsink basin and faucet fixtures on hand washing sinks in modular unit. Replace rusting faucet fixture and rusting basin on hand wash sinks. Replace missing/damaged laminate on front of hand washing sink counter in boys modular unit bathroom. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).

Clean walls and floor throughout in rooms. Touch-up paint/repaint throughout in rooms. Observed water stained ceiling in corner of cafeteria and in stairway at far end of 4-8 hall. Repair any roof leaks and replace water stained ceiling tile in stair way area and repaint corner of ceiling in cafeteria. Replace damaged tile on stairs in stairway. Patch hole on lower section of wall in stairway at far end and repaint. Replace(1) missing floor tile at entrance to modular unit MC717. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Clean floor in cabinet area under hand washing sink in classrooms. Repair wall in can wash area located in custodian closet on C hall. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Clean and organize both book/storage room on 4-8 hall. Clean floor in storage rooms and under cabinet storage area in classrooms. All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.

Observed trash dumpster overflowing with bags of trash and lid could not be closed. Increase the frequency of days dumpster is being emptied to eliminate the overflow issue! Solid waste containers shall be kept clean, in good repair, and emptied when full, but not less than once a week. 15A NCAC 18A .2414 (b).

Observed mouth guard on water fountain soiled. Thoroughly clean mouth guard on water fountain.Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A .2406 (b).

Observed walls soiled in bathrooms(modular unit). General cleaning of walls in bathroom in main building. Clean paper towel from ceiling in bathrooms. Observed loose metal strip on base of stall wall in boys bathroom(modular unit). Tightly secure metal strip on base of stall wall.Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).

Observed two hand sink in bathroom located on C hallway missing handles. Repair faucet fixture on hand sinks. Observed (1) classroom missing paper towel at hand sink in classroom. Provide soap and paper towels at all hand sink at all times! Observed backs plash separating from hand sink counter top in classroom. Caulk/seal back splash to counter top. Observed laminate missing/damaged on hand washing sink counter in modular bathroom. Either replace damaged laminate or paint bare wood surface. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).

Observed storage on the floor in storage rooms and cabinet area under hand sink in classrooms. Remove storage off the floor and store items on shelving or dunage racks,All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.

Observed wasp and wasp nest in open end of pole on swing located on 1st and pre k playground. Kill wasp, remove wasp nest and cap off open end of pole. Observed loose gutter on front and side of school building. Repair gutters and clean out gutters to prevent mosquito breeding places and to prevent water from pooling at base of foundation of building.There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a).

-Repeat Non-Complince: Observed soiled toilet seats. **Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).-Observed handsink knobs missing in the boy's restroom on the B Hall and in the upstairs restroom.-Observed the spring loaded faucets in the mobile restrooms with short running times. Ensure spring loaded faucets run for a minimum of 15 seconds to facilitate in proper handwashing. -Observed paper towel dispensers in disrepair in B1 and B19. **Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).-Observed non-absorbant ceiling tiles in the boy's & girl's restrooms upstairs. **Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).
