Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed stained ceiling in classrooms in trailer. Leak has been repaired. Repaint/ remove stains. Repeat.

The premises of the school under control of the principal shall be kept neat and clean at all times. 15A NCAC 18A .2415 (a). Observed fence damaged near trailer. Repair/replace. Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). Observed chlorox wipes stored in reach of students on desks, sinks counter, class restrooms, etc. in various classrooms in all buildings. Keep all hazardous items stored according to labels (away from children). Suggestion: store in cabinets until needed for use.

Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c). Observed no soap at prek classroom 1 handsink and kitchen handsink need to have soap. REPEAT.

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed holes in walls of boys bathroom in high school building. Observed stained ceiling tiles in 3rd/4th classroom. Observed stained ceiling in High school English room. Needs repair. REPEAT.

Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c). Observed no paper towels or hand drying device in boys restroom located in outside modular unit. Paper towels were supplied during time of inspection.

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed stained ceiling tile in 4th grade classroom. EHS advises school to ensure no active leak is occurring and to have stained ceiling tile replaced.

Observed water stained ceiling tiles in K5, 1st grade, and 3rd grade classrooms. Observed ceiling torn in Gym. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed chlorox wipes on cabinet in k5, on desk in 1st grade, and chlorox wipes and aerosol Lysol spray on cabinet in 2nd grade. Keep all hazardous items stored away from students. Adhere to storage instructions on labels.

Observed disinfectant wipes stored in reach of children in 2nd, 3rd, multiple in 5th, and 6th grade classes. Follow instructions on labels for storage. Keep out of reach of children. Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b).

-Observed the stall walls rusting in the men's restroom. Repair. -Observed stained ceiling tiles in room K5. -Repeat: Observed the ceiling in disrepair in the gym/fellowship hall. -Observed a hole in the wall behind the door in the boy's modular restroom. -Observed wood panel falling from the ceiling with nails protruding in the girl's modular restroom. -Observed the carpet fraying and stained in the 9th grade room. *Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Repeat: -Observed a large bucket of floor cleaner and a container of Lysol aerosol spray stored within reach if the students. -Observed aerosol spray bottle stored in an open classroom within reach of the students. Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). *Nate Rideout present during the inspection.

Observed cleaning needed on the base of the toilet in men's restroom near 2nd grade classroom. Observed water stained ceiling in girls modular building restroom. Observed wall partition of men's restroom near 2nd grade classroom with rust. **Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). **Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).

Observed cleaning needed behind faucet in men's and women's restroom of modular building. Observed re-caulking needed behind hand sinks in restrooms of modular building. **Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).

Repeat Observed ceiling in gym with damage. Observed staining on carpet in rooms M1, M3, science room, and M5. Observed carpet with rips in science room and M5. Observed wall damage in M5. Observed wall damage in closet of cafeteria. Observed ceiling damage in M2 and M3. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Repeat: Observed aerosol sprays, disinfectant wipes, cleaners, etc stored unlocked and within reach of the children in several classes. Keep locked or stored out of reach of the children. Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b).
