Observed water stained ceiling tiles in the office, cafeteria, K-2nd hall, hall leading to small gym and the storage closet near the small gym. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A .2406 (b). Observed black build up in art couple of water fountain spouts on Prek hall.

Observed missing tiles in small gym and boys downstairs bathroom. REPEAT. General Comment: EHS observed boarded up window in gym building. Custodian stated it had recently been damaged and has been measured and is in the process of getting new pieces for repairs.

Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). EHS observed disinfectant wipes, aerosol lysols and air fresheners, and spray cleaners stored accessible to children in several classrooms. All items state “Keep out of reach of children” Toxic materials shall be stored to avoid health hazards. Follow manufacturer instructions.

REPEAT. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed damaged door frame in MOD199. Observed stained or damaged ceiling tiles throughout facility. Repair/replace ceiling tiles and leaks causing damage.

REPEAT. All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b). Observed dead bugs in light fixtures in E 100 boys bathroom and in stairwell light fixture. Observed broken blinds in E223. Observed light sheild missing in boys small gym bathroom and custodial closet.

Observed water stained ceiling tiles in the hallway near the cafeteria and in room 103A. Ensure that leak is not ongoing and replace ceiling tiles. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed light shield missing in classroom 105. Needs to be replaced. Observed dead insects in the light shields in rooms 121 and the light shield in modular units. **All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b).

Observed droppings in the cabinets in rooms 105, 107, 110 and 217. Ensure that pest management is called to have rooms sprayed for pests. **There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a).

Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Observed wall damaged near closet inside boys restroom in modular. Observed doorframe needing to be reattached in girls restroom in modular.

Observed Lysol wipes (C100), chlorox wipes (C100), basket with microban, Lysol spray, and febreeze stored on mat storage, and Lysol wipes also unlabeled spray bottle with yellow liquid (E217). Keep all hazardous items stored away from children. Suggest storing items in desk or inside cabinets.

Observed urine on floor under urinals in boys restroom in modular. Observed baseboards bulging in boys restroom (modular). Observed counter damaged in girls restroom (modular). Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c).

Replace missing ceiling tiles near office and in multipurpose room. Observed water stained ceiling tiles in hall near multipurpose room. Clean wall under water fountains in modular. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed disinfectant wipes, lysol, or other cleaning items stored in reach of children in E204, E210, E110, and mod 599. Keep hazardous items stored away from children. Observed black residue around mouthpieces of water fountains on 2/3rd grade hall. Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A .2406 (b).

Replace missing ceiling tiles in Gym. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed hazardous items stored in reach of children in classes C104(wipes), C101(ant/roach spray), Tutor room beside E217 and class E217(wipes), and mod 199(lysol). Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Observed hallway restrooms having urine collected on floors on 2-3 hall. Observed walls covered in wet paper towels in restroom located on lower level hallway. Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed broken toilet seat in restroom located inside room #C106.

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed flooring inside room #E204 covered in debris in need of cleaning, Observed baseboards in poor repair in hallway of modular unit; Observed numerous stains on carpet of Media Center.

There shall be no fly or mosquito / insect breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a). Observed large amounts of roach feces collected on text books stored inside cabinet area in need of removal and detailed cleaning needed. (#7) Storage on floor; Observed two rooms having storage on floor #C106, MOD 599; (Keep storage 15 inches off floor).

Observed low pressure on water fountains in modular unit. Repair water fountain in downstairs hall way that is not working and adjust water pressure on water fountains in modular units. Clean area around water nozzle and mouth guard on water fountains in upstairs and downstairs hallway. Drinking fountains shall be provided with adequate water pressure and be properly regulated (i.e. water pressure should be regulated so that an individual’s mouth does not come in contact with the nozzle and so that the water does not splash on the floor). 15A NCAC 18A 2406 (b). Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A .2406 (b).

Wipe/clean walls throughout bathrooms. Clean stall fixture in bathrooms. Remove protruding wire from girls bathroom stall door in downstairs area. Tighten loose toilet seats. Repair loose laminate on counter top in boys bathroom located in modular unit. Replace missing handle on hand washing sink faucet fixture in girls bathroom located near gym. Repair hand washing sink in girls downstairs bathroom. Spring loaded faucet fixture is not operating for at least fifteen seconds. Clean area around handle/faucet fixture in down stairs bathroom. Provide soap and paper towels at all hand washing sinks at all times. Observed broken soap dispenser in downstairs girls and boys bathroom. Observed broken/missing paper towel dispenser in downstairs boys bathroom. Observed leaking plumbing fixture on hand washing sink in boys downstairs bathroom. Tightly secure and seal back splash to wall and counter top behind hand washing sink in 112. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).

Monitor area around window and repaint in hallway where library is located . Observed paint flaking from window framing. Observed water stained ceiling tiles in hallway near library and in cafeteria. Repair any and all roof leaks, then replace water stained ceiling tiles. Repair damaged floor tiles in classroom and hallway area in modular units. Replace missing base tile cove base moulding throughout in modular units. Clean door to modular unit and repaint. Clean walls and floor throughout classrooms. Replace broken floor tile in cafeteria near boys bathroom. Patch holes in wall in hallway leading to gym/ec room and repaint. Replace damaged ceiling tiles in gym area. Replace missing electrical outlet cover over electrical outlet in room 599.Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed storage on the floor in storage room and ec room. Remove storage off the floor. Provide shelving or dunage racks to store items on so custodian can clean the floor in storage room/class. Storage spaces and custodians' closets shall be kept clean and arranged so as to facilitate cleaning. 15A NCAC 18A .2411.All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.

Observed lights fixture not properly working in classrooms throughout entire school including modular unit. Either repair light fixtures or replace blown light bulbs throughout. Clean ventilation cover in rooms located in modular units. Clean dust top of cabinet storage area in classrooms. Observed frebreeze and cleaning products not properly stored in classroom area. Store toxic material either locked away or out of reach of children. Observed frebreeze and cleaning products stored near hand washing sinks in classroom and cabinets under hand washing sinks within classrooms. Observed water fountains not operating on right hand side of pre k / kindergarten hallway and down stairs near gang bathroom. Repair broken water fountains throughout school building. Observed water fountains missing mouth guard on first and downstairs hallway water fountains. Repair/ replace missing mouth guards on water fountains. Secure base covers on water fountains. Clean area behind water fountains in modular unit.Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A .2406 (b).

Observed walls and ceiling soiled in bathrooms throughout school building and modular unit. Thoroughly clean wall and ceiling in bathrooms. Repair floor in modular unit bathrooms. Cove moulding of flooring is damaged and is coming loose from wall. Observed protruding wire on base of girls bathroom stall door located down stairs. Observed stall door that will not properly open. Remove protruding wire from bathroom stall door and repair bathroom stall door so door will properly open. Tighten loose toilet seat in girls bathroom. Replace missing ceiling tiles in bathrooms and clean ceiling tiles! Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). General cleaning of floors, walls and ceiling throughout main school building in classroom and hallway area. Clean ceiling near entrance to cafeteria. Thoroughly clean walls throughout modular unit classrooms and hallway. Replace missing wall moulding in hallway of modular unit and remove protruding nails in wall until moulding is replaced. Patch hole in wall near gym entrance and repaint.Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed storage on the floor in second floor bathroom that was converted to a storage room in room E111, in modular unit classrooms storage rooms and storage room near gym. Remove storage off the floor.Storage spaces and custodians' closets shall be kept clean and arranged so as to facilitate cleaning. 15A NCAC 18A .2411. Observed chemicals and cleaning products not properly stored throughout in several classrooms(C104, C110, E115, E112). Store chemicals/cleaning products locked away or OUT OF REACH OF CHILDREN!Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). General Comment: Tighten loose handles on cabinet doors throughout in different classrooms. Observed wet area around black boarder surrounding playground equipment. Place dirt around black boarder in low areas around black boarder to eliminate wet area. Mr. Cunningham present at time of inspection.

-Repeat Non-Compliance: Observed short running spring loaded faucets in the restroom of C1012, the boy's downstairs & Module restroom.Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).-Repeat non-Compliance: Observed no soap in C101 restroom nor in the girl's downstairs restroom. Paper towels were missing in the boy's downstairs restroom.Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).

-Repeat Non-Compliance: Observed water damaged ceiling tiles in the gym, M299, M399, M899, and M999.-Observed tissue wads on the ceiling of the girl's restroom on 4/5 Hall.-Observed stained rugs in M299 and M399.-Observed baseboards missing in the gym.Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Repeat Non-Compliance: Observed storage on the floor in M299, M399, M899 storage rooms, and the upstairs hall restroom that has been converted to a storage room. These areas are also collecting dust and debris and are in need of cleaning.All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.
