Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). Observed chemicals in spray bottles labeled “Keep out of Reach of Children” stored accessible to children in multiple classrooms. Needs to be stored as directed by manufacturer to avoid health hazards.

Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed some chipping/pitting to girls toilet seats outside cafeteria. Needs repair/replacement.

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed three minor stained ceiling tiles in middle school work room and 2302 office. EHS advises school to ensure no active leak is occurring and to have stained ceiling tiles replaced. REPEAT. Points not escalated due to nature of violation and repairs of all tiles from last inspection.

Observed a few minor stained ceiling tiles within school building to include stairwell near middle school hallway and upper level near electrical closet. EHS advises school to ensure no active leak is occurring and to have stained ceiling tiles replaced. *NOTE: Facilities Manager, Mr. Lloyd was present during time of inspection. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed stained carpets in classrooms 2307 and 2314. Replace missing floor outlet cover in TLC room. Patch holes and touch up paint in girls toilet on second floor.

Observed the spring loaded faucets not running for a minimum of 15 seconds to facilitate in proper handwashing- high school hallway- girl's, boy's restroom by 2210, girl's restroom by lower school. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).

Observed stained ceiling tiles in the stairwell by room 2317 and in tutor room #7. Repair the leak and replace stained tiles. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed toilets not flushed in boys room at the gym and girls room on 1st grade hall. Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).

Observed vents dusty in men's room in gym and in girl's room near multi purpose room. Observed blinds damaged/ bent in girl's room on 1st grade hall. All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b).

Observed floors need sweeping in server rooms (locked areas). **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed a couple of empty soap dispensers in bathrooms. Gym had 1 empty soap dispenser and one non working soap dispenser in bathrooms. Make sure soap is dispensing when replacing soap containers.**Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).

Observed storage (cardboard boxes) on floor in 2nd floor book storage.**All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.
