REPEAT Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).- Observed disrepair urinal covered with plastic bag in boy restroom in Math/Science hallway. Repair needed.

REPEAT Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A .2406 (b).- Observed soiled (pink mildew) on water fountains in front of the gym detail cleaning needed all around surfaces, crevices, and nozzle spouts. Keep all water fountains clean and keep clean throughout the facility. Point remain the same at this time.

Observed broken falling of the wall of toilet tissue dispensers in men/women team sport locker room. Need repair or replace. Observed all hand washing sinks throughout the building need better cleaning and keep clean. Observed faucet at hand sink in girl restroom in front of the cafeteria is not working, need repair. Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).- Observed no paper towel available at the hand sink in men team sport locker room and the cafeteria. Ensure to provide all hand washing supplies at all hand sinks at all time.

REPEAT Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). Observed staff bathrooms left unlocked with aerosols and spray chemicals left accessible to children. Lock door or remove items.

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed multiple water damaged ceiling tiles throughout hallways and classrooms in need of repair. (if leak is present, repair leak and replace ceiling tile).

Observed numerous classrooms having chemicals stored accessible to children; (Recommendation) lock chemicals up when not in use, either a lockable area within classroom and/or locking classroom door when room is vacant.

REPEAT Observed water fountains throughout in need of cleaning. EHS suggested cleaning fountains more frequently to prevent build-up. Observed several water fountains with high water pressure where water splatters onto the floor. Regulate pressure. **Drinking fountains shall be provided with adequate water pressure and be properly regulated (i.e. water pressure should be regulated so that an individual’s mouth does not come in contact with the nozzle and so that the water does not splash on the floor). 15A NCAC 18A 2406 (b). **Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A .2406 (b).

REPEAT Observed loose toilet seats in bathroom stalls throughout. Observed cleaning needed on walls under paper towel dispensers throughout restrooms, on the wall in the last stall of boys upstairs gang bathroom, and last stall of boys gang bathroom on foreign language hall. Observed cleaning needed on the floors in the girls and boys gym lobby restrooms. **Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). **Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). **Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c).

Observed cleaning needed in boys locker room showers and toilets and girls locker room toilets. Observed cleaning needed on carpeted floors on boys and girls locker rooms.
